Responsibilities: The Behavioral Health Patient Navigator (BHPN) position is part of the Pediatric Behavioral Health and Wellness, Integrated Behavioral Health Satellite based in Golisano Children's Hospital Pediatric Practice (GCHPP), which is a part of the clinical service enterprise of the Department of Pediatrics. GCHPP, a primary care practice, provides outpatient medical care and related- family based services to patients (0-21 years) who reside in the greater Rochester region. The service accepts Medicaid, Managed Care Medicaid, and private insurance plans and offers a sliding fee schedule to families with low income. The Integrated Behavioral Health Satellite clinician's support: (1) the care of all patients through mental and behavioral health consultation, resource recommendation, and referral support, (2) the development of treatment and safety planning for identified patients, and (3) short-term solution focused treatment to patients and their families. The BHPN ensures patients receive the best care available to them by guiding patients through all the various complexities of the healthcare system. The role supports GCHPP patients and their families through consultation regarding community resources, preparation and follow up on referrals, and participation in appointments and community-based visits when clinically indicated. The BHPN provides field-based case management services, may serve as a main point of contact for patients, builds strong relationships with patients in order to help patients stay engaged in behavioral and medical care and adhere to their care plan. The BHPN Identifies critical resources, helps navigate through health care services and systems, and promotes patient health. The BHPN works closely with the care team, which includes medical (e.g., physicians, nurses), mental health (e.g., psychologists, mental health therapists) and non-clinical staff (e.g., care coordinators) to support positive patient health outcomes. Individuals must subscribe to UR Medicine ICARE values including creating a culturally competent environment by treating patients, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

Essential Functions:

  • Provide consultation to clinicians regarding community resources for their patients and patients' families
  • Meet with patients and/or caregivers to assist with paperwork, correspondence, etc., necessary to ensure linkage to appropriate resources; as well as assist with access to urgently needed resources (e.g., food, housing, medical care)
  • Participate in evaluation and treatment appointments with mental health clinicians and patients/families as needed (e.g., to share information about community resources and programs)
  • Responsible for documenting patient and family contacts and patient progress in the electronic medical record
  • For patients in acute crisis, collect information, prepare referrals, and follow-up on referrals to necessary services
  • Make collateral contacts for new and existing patients and families. Collateral contacts may include Child Protective Caseworkers, Foster Care Caseworkers, other DSS Caseworkers, probation officers, pediatricians and nurse practitioners, and school personnel.
  • Make community-based visits when clinically indicated to support youth and family advocacy efforts (e.g., school visits, CSE meetings, DHS)
  • Provide telephone screens/triage to families seeking GCHPP behavioral health services for their child or adolescent.
  • Participate in designated service meetings

Minimum Education & Experience:

  • Bachelors' degree in health-related field, 2-3 years' experience as a care manager for; or an equivalent combination of education and experience required.
  • 2-3 years' experience as a psychiatric case manager; or an equivalent combination of education and experience, preferably with children, adolescents and families required
  • 1 year experience in community health, human services, and/or patient liaison/advocate roles preferred.
  • Lived experience working with the mental health system related to a family member.

Knowledge, Skills and Abilities:

  • Must possess a valid New York State Driver's License, satisfactory driving record, and access to a reliable automobile required.

Licenses and Certifications:

  • BCPA-Board Certified Patient Advocate upon hire preferred

Responsibilities:

ACT services are provided to patients who cannot engage with traditional mental health clinics; thus, the ACT Team Clinical Coordinator engages with multiple government agencies such as the jail, mental health court, homeless shelter providers, etc. to facilitate meeting regulatory requirements and high-quality clinical care. The Clinical Coordinator oversees the daily clinical operations of the interdisciplinary team including making assignments, triaging cases, and prioritizing staff activities to meet acute patient needs. The Clinical Coordinator strives to maintain regulatory required staffing levels through recruitment, onboarding and retention strategies for this team who interact with the most challenging and vulnerable patient population in Behavioral Health clinical services. The Clinical Coordinator is also an integral member of the ACT team, providing comprehensive integrated mental health and substance abuse treatment to enrolled clients and provides administrative and clinical supervision of an interdisciplinary group of clinical staff including multidisciplinary mental health therapists, CASAC's, Rehabilitation Therapists, Case Managers, and other team members.

RESPONSIBILITIES:

Administrative:

Ensures that the ACT team adheres to all aspects of the OMH regulatory requirements for the ACT team in terms of service delivery, documentation that the team completes OMH reporting in a timely and comprehensive manner (i.e., CAIRS, AOT, etc.), billing and data reporting.

Work with Strong Ties leadership and Dept of Psychiatry Utilization Review team to address documentations concerns in a timely way and problem-solving reduction in risk of noncompliance going forward.

Prepares for and participates in regulatory surveys for ACT with the Strong Ties Leadership team and engages in the development, monitoring and evaluation of Performance Improvement Plans required as part of the survey process.

Approval of purchases for patients directly related to patient care as WRAP funding. Ensure appropriate process for procurement card acquisition, purchases, completion of SOEs, reconciliation, and doumentation of WRAP utilized per patient.

Facilitates daily team meetings to assign cases to team members and reviews days work responsibilities

Accepts referrals for ACT and completes necessary collaboration with community providers in the assessment and determination for enrollment in ACT. Works with the team to complete the enrollment process as indicated

Supervisory:

Is responsible for the hiring, onboarding, and supporting new clinicians and ACT team staff.

Provides weekly clinical supervision to all direct care staff on the team per OMH guidelines.

Works with Strong Ties leadership and HRBP regarding any staff disciplinary issues.

Provides feedback to the treatment team regarding patient progress in recovery and facilitates and supervises the placement of patient in other community treatment programs as necessary.

Clinical:

Serves as liaison between the ACT program and other community agencies that provide chemical dependence or MICA treatment programs for ACT clients who choose additional or inpatient chemical dependence treatment services.

Serves as liaison between the ACT program and the courts, probation, and parole.

Transports clients as needed to appointments and assists clients in moving or shopping.

Participates in a client on-call phone rotation with other ACT team members once every 5-8 weeks.

The Team Leader also provides back-up (when not directly on call) to any clinical staff on the team that is on call as needed in the event of patient acuity.

Works with clients individually and in group format to help initiate and maintain recovery from substances of abuse and other addictive behaviors.

Engages clients in both individual and group psychoeducational to minimize the incidence of relapse using recovery-based principles, collaboration and shared decision making.

Provides cognitive behavioral therapy for psychosis including education about stress-vulnerability and symptom management.

  • A physician-led platform that spans neurology, neurosurgery, stroke , and PM&R  across 8 hospitals and 600+ care sites
  • dyad governance model  built on trust, visibility, and operational alignment
  • A growing team of neurologists with subspecialty depth in stroke, epilepsy, memory, headache, movement, and neurohospitalist care
  • Neurosurgeons trained in DBS, LITT, RNS, and complex cerebrovascular and spinal procedures
  • A regional PM&R infrastructure anchored by the Golisano Restorative Neurology & Rehabilitation Center , CARF-accredited and best-in-class

Job Summary

The Care Manager II services youth and adults in their community setting with the goal of reducing and preventing emergency room visits, hospitalizations, and decreasing overall Medicaid costs as a lead member of the team responsible for the client. This role utilizes assessments, care planning, linkage to services and community resources, advocacy, and support to coordinate services for adults, youth, and families using person centered and family driven care strategies.

Essential Job functions
  • Responsible for, but not limited to comprehensive assessments, outreach and engagement, service and treatment linkage and coordination for assigned youth, adults and families.
  • Partner with referral sources to engage youth, adults and families and build an interdisciplinary care team to support the member in meeting their needs.
  • Facilitate monthly face-to-face visits with adults, youth, family, and their interdisciplinary care team, for assigned caseload.
  • Conduct initial and ongoing assessments to assist with accomplishing member's goals and needs within program requirements.
  • Create, implement, evaluate, and modify, as required and needed, individual service plans to meet assessed client's unique needs as a member of a multi-disciplinary team.
  • Refer members to applicable agency services and community programs, such as outpatient counseling, dental provider, and primary care providers.
  • Support members who transition between systems and services (i.e. hospitalizations, inpatient stays, residential settings, housing needs, etc.).
  • Establish and maintain productive working relationships with community service providers to facilitate referrals and service evaluations.
  • Maintain required contact with members, families, and the interdisciplinary team and facilitate team meetings.
  • Serve as a liaison between the program and other internal and external resources, ensuring information is shared with the Care Team.
  • Maintain and update all necessary records, forms, reports, and summaries in member files according to agency and funders standards.
  • Travel across different counties within respective region to serve members, as required.
  • While this job description covers many aspects of the role, employees may be required to perform other duties as assigned.
Education & Experience
  • Bachelor's degrees required.
  • Minimum 2 years of experience working in a human services or related position supporting youth, adults and families required.
SPECIAL REQUIREMENTS
  • Unrestricted, valid NYS driver's license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.
  • Children's Health Home only: Must receive CANS (Child and Adolescent Needs and Strengths Assessment) certification score of 70 within 3 months of hire and annually thereafter.
Knowledge, Skills & Abilities

In addition to demonstrating the Hillside Professional Competencies of Communicates Effectively, Personal Excellence, Cultural Competence, Builds and Leverages Relationships, and Optimizes Decision Making, the following occupational competencies must be demonstrated:
  • Demonstrate the highest standards for ethical and professional conduct at all times
  • Knowledge of all federal, state, and local statutes, regulatory agency standards and Hillside policies.
  • Ability to manage multiple tasks and large caseloads simultaneously.
  • Ability to manage scheduling visits with high volume caseload and complete tasks by funder deadlines.
  • Ability to de-escalate and manage crisis situations both in-person and by phone.
Physical Demands & Work Environment

  • Must be able to work a variable scheduling including evenings and weekends
  • The following daily physical demands are required:
    • Sitting (up to 6 hours)
    • Occasional standing (up to 2 hour)

    • Driving (several times a week up to 6 hours)
    • Continuous balancing (up to 8 hours)
    • Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours)
    • Weekly lifting up to 10-15 lbs. on a daily basis
    • Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)
    • Occasional exposure to dust, fumes, gases, chemicals, or smoke is apparent
    • Ability to change positions as needed

SPECIAL CONSIDERATIONS

While Hillside is a restraint free environment, there may be times in a "life or limb" situation, that staff may be required to physically restrain clients weighing between 50 lbs. and 300 lbs. and guide them safely to the floor. In some circumstances, restraints can last up to 15 minutes and may require repetition as necessary.

$21.40 Minimum pay rate, $31.50 Maximum pay rate, based on experience.
