Lung transplant patients at Cedars-Sinai have access to all of the services and programs for which the Medical Center is renowned. These include the S. Mark Taper Imaging Center, with groundbreaking diagnostic technologies; state-of-the-art surgical suites; and the latest in new techniques such as video-assisted thoracoscopic surgery, lung volume reduction surgery and minimally invasive surgical techniques and interventional pulmonology.
Essential Job Duties:
* The Lung Transplant Coordinator is responsible for the planning and coordination of all aspects of care for the patients throughout the continuum of services-from initial referral to follow-up care.
* Serves as the primary coordinator for the interdisciplinary team, including: physicians, nurses, social workers, dietitians, pharmacists, financial analysts, case managers, and support staff.
* Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel, and the community.
* Collects/documents data to meet clinical, financial, and regulatory requirements.
* Participates in the development of guidelines, protocols, pathways, and other performance improvement and research activities.
* Serves as a patient advocate throughout the patient care process.

* BSN degree required
License/Certifications:
* Current California RN License required
* BLS from the American Heart Association required
* CCRN Acute/Critical Care Nursing, PCCN Progressive Care Nursing or other specialty certification required
* Certified Clinical Transplant Nurse (CCTN) preferred (or willing to obtain within one year from start date)
Experience:
* A minimum of 3 years of acute care nursing experience required
* At least 1 year of Critical Care experience required
* Organ transplantation experience preferred
* Demonstrated knowledge of case management required
* Experience in data management preferred
* Excellent organizational skills and ability to effectively coordinate all aspects of patient care required

Primary Duties:Serves as the initial contact person at the medical practice or hospital department and greets patients in a courteous and professional manner.The Patient Service Coordinator manages the intake of patients into the practice, screens for emergent conditions, registers, verifies and explains patient responsibility for payment.Performs check in and check out procedures by inputting patient charges and verifying demographic and insurance information.Coordinates efficient patient flow through the practice or hospitalize department assigned.Supports organization initiatives related to new technology, clinical programs and improving the patient experience.Qualifications:6 months - 1 year of providing high quality customer service required, preferably in a health care setting.
