Registered Dietician

JOB SUMMARY  

Assesses nutritional status of PACE participants. Uses pertinent data to plan and implement appropriate nutrition interventions and communicate the information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met. Functions as a member of the Interdisciplinary Team. Provides nutrition counseling, education, and therapy to participants; educates and counsels family and other caregivers regarding dietary issues; and conducts initial assessments and periodic reassessments.  Participates in the development of the plan of care, assists in the coordination of 24-hour care delivery, and regularly informs the Interdisciplinary Team (IDT) of the nutritional needs of each participant. Provides education to other health care professionals as appropriate.   

ESSENTIAL DUTIES AND RESPONSIBILITIES  

  • Provide high quality nutritional care and services and serves as a member of the PACE interdisciplinary team (IDT) 
  • Conduct an initial comprehensive nutritional assessment upon enrollment and as needed according to PACE procedures regulations. 
  • Collaborate with the interdisciplinary team to develop a comprehensive care plan for each participant. Revise goals and approaches to participants’ care in coordination with interdisciplinary team. 
  • Provide nutritional counseling to participants, care givers and family members to promote health and wellbeing of participants. 
  • Facilitate nutrition-related care plan interventions, such as home delivered meals and supplements.  
  • Monitor weight, medical diagnosis, drug nutrient interactions, changes in chewing or swallowing, mood changes, hydration status, intake, skin breakdown and report changes to the interdisciplinary team with recommendations for changes. 
  • Works collaboratively with the interdisciplinary team (IDT) to develop and implement the comprehensive plans of care for participants.  
  • Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.  
  • Participate in all interdisciplinary team meetings.
  • Assist the interdisciplinary team members, contractors and center staff in understanding the significant nutritional status and needs related to the participant’s health problems.  
  • Incorporate cultural and ethnic preferences of participants into individual care plans. Provide direction and input into planning and implementation of nutritional services regarding the cultural and ethnic preferences of participants. 
  • Performs the duties of other IDT members based on professional licensing, competencies, and experience as needed. 
  • Performs the duties in the role of Food Service Supervisor as assigned. 
  • Actively participates in Quality Improvement projects / meetings.  

  • Participates in family meetings, staff meetings, in-service and training and orientation programs as required. 
  • Follows all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines. 
  • Protects privacy and maintains confidentiality of all company procedures and information about employees, participants and families. 
  • Documentation of care that reflects excellence as demonstrated by clear, concise, comprehensive data. 
  • Practices standard precautions and follows PACE Program Infection Control protocols.   
  • Position specific competencies for the Dietitian will be met prior to assuming participant care.
  • **NJ: Dietary counseling which falls outside of the scope of practice defined by the State of New Jersey Nursing Practice Act and the New Jersey State Board of Nursing shall be provided by a dietitian.
  • Performs other duties as required or requested. 

EXPERIENCE AND EDUCATION 

  • Bachelor’s degree from an accredited college with major studies in food, nutrition, or dietetics. 
  • Passing the Commission on Dietic Registration (CDR) exam.
  • Minimum of 1 year working with the elderly population required.  If this is not present, training will be provided upon hire (If applicable for the role).  
  • Minimum of two (2) years clinical nutrition experience preferred 
  • Serve Safe Certified preferred 
  • **NJ: Each dietitian shall be registered or eligible for registration by the Commission on Dietetic Registration.

PRE-EMPLOYMENT REQUIREMENTS:   

  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.   
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.   

  • Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.

JOB SUMMARY: Responsible for overseeing and ensuring consistent high-quality clinical care for participants across all settings. The Director of Clinical Services role focuses on delivering participant-centered care in alignment with PACE policies, regulatory standards, and best practices for elder care. As a key member of the PACE program leadership team, this position contributes to policy development, census growth, staff training, and achieving outstanding participant outcomes and satisfaction.

ESSESNTIAL DUTIES AND RESPONSIBILITIES:  

  • Integrate organizational values, mission, and vision into program culture, policies, and daily operations.
  • Oversee consistent high-quality clinical care for participants across multiple counties.
  • Monitor participant experience and outcome data, guiding Care Teams to ensure high-quality care.
  • Collaborate with HR and management for staff recruitment, training, and performance oversight.
  • Ensure timely and accurate participant assessments and care plans by interdisciplinary teams.
  • Establish and oversee an effective 24/7 on-call process with Medical Director.
  • Collaborate with leadership to maintain efficient, high-quality services.
  • Ensure clinical staff actively engage in team meetings and care planning.
  • Develop and monitor budgets, key performance indicators, and financial reports for resource utilization.
  • Participate in Quality Improvement programs, track data, and lead improvement initiatives.
  • Ensure clinical compliance with federal, state, and PACE requirements.
  • Develop job descriptions and assign duties aligned with competencies.
  • Assist in development of Standard Operating Procedures to ensure policy compliance in line with PACE standards in collaboration with leadership.

  • Collaborate with the Medical Director, Operations Director, and program management teams to ensure high-quality, efficient, and effective service delivery under the direction of the Executive Director.
  • Participate in family and staff meetings, in-service training, and departmental activities as needed.
  • Works in collaboration with Clinic Practice Manager to effectively maintain and manage clinic operations.
  • Follow all PACE policies, OSHA safety guidelines, and maintain confidentiality.
  • Lead infection control efforts to ensure best practices in all settings.
  • Attend continuing education and training as required.
  • Maintain required certifications and professional affiliations.
  • Perform other duties as needed.

EXPERIENCE EDUCATION AND CERTIFICATIONS :

  • State RN license with leadership skills and experience in healthcare delivery and management. Strong desire to work with the elderly in an innovative, highly interactive, interdisciplinary environment.
  • Compact License Required (KY/IN)
  • Bachelor of Science in Nursing Degree required. Master’s degree in management or a healthcare related field preferred. Professional Certification preferred. 
  • Minimum 3-years’ experience working with the frail and elderly in acute, long-term care or community based setting.
  • 3 years progressive management experience required.
  • Experience in home care, managed care, or long-term care preferred.

How you make a difference

As a Licensed Mental Health Professional working in a correctional facility, your responsibilities include conducting intake evaluations, providing therapeutic interventions and suicide prevention techniques, and developing treatment plans for inmates. You will also be responsible for monitoring the clinical needs of inmates on your caseload, providing staff training on mental health topics, and documenting all findings in accordance with company and facility policies. Your role is critical in ensuring that inmates receive appropriate mental health services and care while incarcerated.

Key Responsibilities

  • Conduct screening, assessments, and triages for inmates referred by security, healthcare staff, and other community sources, and provide appropriate recommendations for their level of care.

  • Develop and recommend treatment plans, conduct essential treatment services such as psychoeducational and transitional groups, and provide suicide prevention interventions and management techniques.

  • Respond to sick call requests, provide medication adherence, and coordinate with prescribing providers for medication evaluations.

  • Provide crisis intervention, oversee the clinical needs of patients on caseload, and participate in treatment team multidisciplinary meetings and administrative meetings.

  • Provide staff training on relevant mental health topics, document all findings in the patient's health record, and ensure compliance with facility and company policies and procedures.

Additional Details

Qualified applicants will have one of the following active licenses in the state of Wisconsin:

Licensed Professional Counselor (LPC)

Licensed Clinical Social Worker ()

Advanced Practice Social Worker (APSW)

Licensed Professional Counselor In-Training (LPC-IT)

Independent licensure is preferred (LPC or LCSW) though LPC-IT and APSW licenses will be considered with relevant work experience.

Schedule for this position is M-F with rotating weekend (one Saturday & Sunday per month).

Qualifications & Requirements

Education

Experience

  • One (1) or more years of experience in a Mental Health treatment program, preferably in a Correctional or Hospital setting

Licenses/Certifications
