GENERAL OVERVIEW:
The Mental Health Professional will provide clinical treatment services to the targeted populations. This is an outpatient treatment program designed to service patients with a mental health diagnosis who meet dimensions for outpatient and/or partial hospitalization treatment criteria.
ESSENTIAL RESPONSIBILITIES:

Creates a treatment plan for physicians that includes an effective continuum of therapeutic, supportive and other services for the patients with a variety of needs and diagnoses serviced by the program. (40%)

Discusses need for additional services with the clinical team to determine appropriateness of recommendations. Refers patients to other levels of care, community based self-help programs for recovery or other community social or human services as needed. (10%)

General Overview:
Under the direction of the provider and as a member of the health care team, assists in coordinating the examination, treatment and health care planning for patients. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP).
**PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**
**Patient Related**
Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.
+ Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.

+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient prior to discharge.
+ Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.

**Position Purpose:**
Hospital Services Technicians travel between customer healthcare facilities providing on-site proactive collection and exchange of waste containers for various Stericycle programs, including but not limited to: Sharps Management Service, Pharmaceutical Waste, Controlled Substances, and Document Management. This service is provided at regularly scheduled intervals for assigned facilities.
**Key Job Activities:**
- Travels between customer healthcare facilities to proactively monitor, collect, and exchange waste containers for various Stericycle programs and services, including but not limited to: Sharps Management Service, Pharmaceutical Waste, Controlled Substance and Document Management.
- Interacts professionally with onsite customer staff to ensure service expectations are met or exceeded.
- Maintains a rigid collection and service schedule.
- Communicates duty status on a defined scheduled.
- Perform other duties and responsibilities, as assigned.
- Position requires extensive lifting, walking, bending, and pushing. Must be able to lift up to 55 pounds and push and/or pull carts weighing as much as 500 pounds.
**Experience:**
- 2+ years' work experience or Highschool diploma or equivalent.
- Demonstrate strong communication skills and tact in providing superior. customer service in a variety of settings and situations.
- May require mandatory immunizations and credentialing based on customer requirements.
- Must possess a valid driver's license. Must possess reliable transportation to travel between customer facilities.
- Ability to work independently and with little supervision.
- Knowledge of computer/handheld of software applications including spreadsheets, and application-based programs to track daily KPI's.
**Schedule:**
Start Time: Between 5:00AM and 6:00AM (Tues/Fri)
Mon: OFF
Tues: Uniontown Hospital (Uniontown, PA)
Wed: OFF
Thurs: OFF
Fri: Uniontown Hosptial (Uniontown, PA) AND Highlands Hospital- **Bi-weekly** (Connellsville, PA)
