POSITION SUMMARY

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Documents all team, physician and patient/family communication and concerns pertaining to coordination of care and services.
  • Screens every patient to identify need for further assessment of medical necessity or discharge planning (standard description of which patients are seen). 11. Adheres to the Care Management Department policies and procedures.
  • Participates in the Quality and Performance Improvement Plan for the Care Management Department.
  • Considers the patient population served, age-specific criteria and the Watson Model of Care in all patient/family care and interaction.
  • Utilizes Milliman Care Guideline's best practices to determine patient disposition. Collaborates with on site Hospitalists, Intensivists, Laborists and Emergency Department physicians in this process.


POSITION REQUIREMENTS

A. Education
  • Bachelor of Science degree in nursing preferred
  • Associates degree in nursing required

B. Qualifications/Experience
  • Minimum of one (1) to three (3) years of hospital or related experience required. Internals with at least 18 months acute care case management/coordination experience will be considered in lieu of nursing clinical experience.
  • Able to navigate and connect successfully with outside provider networks (Health Plans, IPA's, and FQHC's).

C. Special Skills/Knowledge

Overview

Registered Nurse will support the ER initiative and provide quality patient care Manages individualized; goal directed nursing care through use of the nursing process and the principles of primary nursing in accordance with departmental and Hospital policies and procedures. The Registered Nurse provides care, treatment, counseling and health education to the hospitalized patient. The RN is responsible for planning and coordination of care across the continuum. Responsibilities

Assesses the patient and family needs in order to provide quality care to assigned patients.

Develops, discusses and communicates a realistic problem list (plan of care) for each patient, in collaboration with each patient/family/significant other in order to address all identified needs.

Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions and procedures as necessary for the care of the patient.

Evaluates the identified problems, care provided and patient's responses are reassessed in order to meet patient and family needs to assure overall quality of care delivered.

Maintains up-to-date and accurate documentation of nursing care provided to assure the integration of information for use by the health care team, in order to assure quality care.

Maintains current knowledge/certification/licensure. Pursues professional growth and development.

Functions as a role model for current and new staff, and performs patient/customer initiatives to keep patients and families happy.

Performs all responsibilities/duties required by the critical care division as defined in the scope of service, to assure that the unique nature of the client is addressed. This includes, but is not limited to the age of the client served.

Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, specific job-related hazards.

Responsibly follows the hospital exposure control plans/bloodborne and airborne pathogens.

Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.

Promotes effective working relations and works effectively as part of a department/unit team inter and intra departmentally to facilitate the department's/unit's ability to meet its goals and objectives.

Essential Responsibilities:

+ Exhibits excellent customer relations with patients, visitors, providers, and co-workers; shows courtesy, compassion, and respect; conforms to the critical service standards established for the department.

+ Provides therapeutic patient care in accordance with the patients established by physician/advanced practice providers and/or a registered nurse within the scope of practice.

+ Adheres to high level of disinfectant, point of use, peel packs policies, and procedures.

+ Perform related patient care functions such as taking temperature, pulse, respiration and blood pressure readings, wound care, may instruct patient in various self-administered treatment or procedures; obtain specimens; assures correct culture collection.

+ Assist physician/advance practice providers or other medical staff personnel with examinations and treatments by selecting and passing instruments, providing support and restraint to the patient, draping, and other assistance as directed or required.

+ Within scope observes and documents patients condition by gathering signs, symptoms, and reactions to therapy, to identify unexpected and/or significant findings, initiates appropriate emergency procedures as necessary, and reports unusual conditions to the registered nurse/advanced practice providers as established by policy and procedure in a timely manner.

+ May volunteer to start and maintain intravenous flow rate with Certifications, notifies registered nurse/advanced practice/physician of any problems. May discontinue IV therapy except intra-cath and central venous lines.

+ May volunteer to start perform venipuncture/blood withdrawal with Certifications.

+ Collects and processes specimens per policy, procedures, guidelines, and protocols.

+ Provides non-compoicated teaching following the registered nurse/advanced practice provider/physician plan for education. Report any identified learning needs and discharge barriers.
