Qualifications
Certifications: Basic Life Support (BLS) within 30 days - American Heart Association (AHA), Education: H.S. Diploma/GED (Required), Work Experience: Relevant medical military training; Completion of CNA coursework within past 24 months; Certified Nursing Assistant (CNA); At least one semester of nursing clinical experience; Certified Medical Assistant at Carle Health; Worked as CNA within past 24 months; Successful completion of Carle's HCT In Training Program
Responsibilities
Assists with the maintenance of unit supplies to ensure the smooth function of the unit, including stocking nurse servers, cleaning instruments and equipment, ordering supplies, and making up charts. Collects data on assigned patients as directed by Registered Nurse (RN) and ensures accurate documentation. Demonstrates competency in selected psychomotor skills as outlined in the skills checklist including: measurement of vital signs, blood glucose monitoring, and measuring and recording intake and output. Performs or assists patients with the activities of daily living. Prepares patients for transportation and/or transport. Completes the unit specific HCT Safety Checklist at the beginning of each shift Receives delegated responsibilities from RN each shift and maintains close communication during shift regarding completion of tasks/responsibilities Completes and documents basic patient care independently: including but not limited to baths, vital signs, accuchecks and meal trays Maintains patient safety as the highest priority level: ensuring safety measure compliance such as fall precautions, pressure ulcer prevention, isolation precautions and patient ambulation safety Maintains a positive attitude at all times on unit that promotes a healthy work environment and a healing patient environment Keeps a diligent focus on prompt answering of patient call lights to ensure effective triage and completion of patient requests/medical needs

Perform comprehensive nutrition assessments for patients identified at nutritional risk, adhering to facility policies and proceduresDevelop, implement and monitor individualized and evidence-based nutrition care plans for patients with complex medical conditionsOffer nutrition education and counseling to patients and families regarding acute disease management, discharge planning, and prevention of readmission.Proactively collaborate and communicate effectively with physicians, nurses, pharmacists, speech-language pathologists, case managers, and other members of the interdisciplinary care team to optimize patient outcomes and facilitate timely discharge.Ensure compliance with all hospital policies, regulatory standards (e.g., TJC, CMS), and best practice guidelines for acute care nutrition.Maintain thorough and timely electronic medical record documentation in a fast-paced environment, reflecting medical necessity and intervention effectiveness.Routinely monitor patients for acute changes in weight, labs, fluid status, and clinical condition, adjusting nutrition interventions promptly.Actively participate in quality improvement initiatives, clinical rounds etc to enhance nutrition care services and overall patient safety within the acute care setting.Serve as a nutrition expert and resource for hospital staff, contributing to staff education and development.Perform other tasks as assigned and within scope of practice as neededRequirementsExperience & Qualifications:
Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program.Registered by the Commission on Dietetic Registration (RD/RDN).Current state licensure/certification (if mandated by the state).Minimum of 2-3 years of recent clinical experience in an acute care hospital setting (required).Demonstrated experience managing patients with diverse, complex medical conditions.Proficiency in enteral and parenteral nutrition support.Exceptional communication and interpersonal skills for effective interaction with patients, families, and high-performing healthcare teams in high-pressure situations.Possesses superior critical thinking, problem-solving, and decision-making abilities to assess rapidly changing objectives, develop, and implement effective, time-sensitive plans.Strong familiarity with acute care workflows, electronic health records (EHR), and regulatory standards relevant to hospital accreditation.Ability to work autonomously and prioritize effectively in a dynamic environment.Why Join Us?
