Schedule - Monday to Friday Flexible schedule Floating between Various Imaging and Speciality Practice in Nassau and some parts of Suffolk County
Primary Responsibilities:
- Prepares and positions patients for diagnostic imaging procedures
- Explains procedures and process to patients
- Moves imaging equipment into the specified position; determines exposure factors on the basis of height, weight, the portion of the body involved, and the degree of penetration required; and adjusts equipment controls to set exposure factors and produce images of proper detail, density, and accuracy
- Practices radiation protection techniques to minimize radiation to patients and staff
- Performs specialized and routine diagnostic radiology procedures
- Processes images and reviews them for proper identification and quality control
- Prepares images for reading by the radiologist or requesting physician
- Performs routine maintenance, diagnoses malfunctions, and makes minor repairs on radiologic equipment, arranging for repairs as needed

Required Qualifications:

What you will do.
The Care Specialist works in patient’s homes and community 90% of the time and virtually the other 10% to deliver chronic care management to high complexity patients. During initial outreach the Care Specialist informs patients about our services and helps them get enrolled. Reaching out via phone is our top strategy for outreach and it’s important that the Care Specialist is comfortable and confident communicating by phone. Field-based approaches are utilized as well, and the Care Specialist should be prepared to use whatever strategy is most effective. Once the patient is enrolled, the Care Specialist will facilitate virtual visits from the patient’s home to our providers remotely to support our integrated care delivery model, focused on the following goals: Promote timely access to appropriate care; increase utilization of preventative care; reduce emergency room utilization and hospital readmissions; create and promote adherence to a care plan developed in coordination with the patient, primary care provider, and family/caregiver(s); increase patient’s ability for self-management and shared decision-making; provide medication reconciliation; connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs
KEY RESPONSIBILITIES:
- Conduct direct outreach to patients via phone, in-person, mailings, and other strategies including cold calling and unscheduled door knocks.
- Enroll patients into Upward Health’s program and collect key data about patients during the enrollment process through continuous and on-going phone and in-person interactions.
- Meets patients in their home or in the community to conduct a needs assessment, including helping patients to set health goals.

- Explain technical medical information to educate the patients.
- Cultivate and support the primary care providers with timely communication, inquiry follow-up, and integration of information into the care plan regarding transitions-in-care and referral.
- Builds rapport with Upward Health patients utilizing motivational interviewing techniques.
- Conduct one-on-one extended in-person patient appointments.
- Makes follow-up calls and home visits to patients per Upward Health policy.

• Perform procedures, e.g.: hot & cold applications, placement/ removal of orthopedic appliances, casts, splints, suture and staple removal, urinary bladder irrigation w/ and w/o chemotherapy, insertion/removal/catheter care of urinary catheter, ear irrigation.
• Applies and removes dressings, and performs wound care
• Assists with Minor and Major Procedures
• Performs patient monitoring including pre, intra, post procedure, testing, medication administration, medical/surgical treatment and/or sedation
• Provides medication administration: (IM, PO, SQ, ID, IV topical, inhalation, ocular, vaginal, rectal) as per provider orders or protocols to patients in practice, following nursing rights of medication administration and organizational policies. This includes oxygen therapy.
• Facilitates medication refills by reviewing clinical information and preparing order for provider (prospective) authorization in EMR.
• Administers IV therapy: including blood, blood products, IV fluids, chemotherapy (with proper certification), IV medications via peripheral or central venous devices.
• Starts and discontinues peripheral IV lines.
• Maintains peripheral and central IV lines and manages complications with appropriate certification
Minimum Qualifications:
To qualify you must have a A Baccalaureate degree in Nursing is highly preferred; a Diploma or Associates Degree in Nursing requires a minimum of one year experience in conjunction with plans to earn a baccalaureate degree required. Current Registered Professional Nurse in New York licensure is required. BCLS is also required. Emergency room or critical care nursing experience and membership in pertinent professional organizations preferred. Membership in pertinent professional organization preferred. Fluent written and spoken English language skills necessary. Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities needed also required.
Required Licenses: Registered Nurse License-NYS, Basic Life Support Cert
Preferred Qualifications:
ACLS certification. Spoken mastery of a second language strongly preferred.

Minimum Qualifications:
To qualify you must have a A Baccalaureate degree in Nursing is highly preferred; a Diploma or Associates Degree in Nursing requires a minimum of one year experience in conjunction with plans to earn a baccalaureate degree required. Current Registered Professional Nurse in New York licensure is required. BCLS is also required. Emergency room or critical care nursing experience and membership in pertinent professional organizations preferred. Membership in pertinent professional organization preferred. Fluent written and spoken English language skills necessary. Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities needed also required.

Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city.
Responsibilities:
Schedule: Full-time: Mon-Fri, 4pm-12am (RNDC)
Under the supervision of the Clinical Supervisor and the Mental Health Unit Chief on Rikers Island, the MHC will provide quality mental health care to incarcerated individuals who need and/or want services. This position is responsible for providing assessment and counseling services to inmates.
- Conducts mental health evaluations, crisis intervention, assessments, and follow-up care.
- Determines and coordinates disposition of patients for appropriate level of mental health care.
- Coordinates with Reentry Social Workers for appropriate discharge planning services.
- Completes required statistical reports.
- Collaborates with the Department of Correction to ensure access to patients, proper housing, and treatment disposition.
- Coordinates patient care with other institutional programs.
