Summary The position is eligible for Education Debt Reduction Program (EDRP), student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Responsibilities The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) in Home Based Primary Care (HBPC) practices in Extended Care and Palliative Medicine Service to provide a full range of health care services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. The major duties and responsibilities include, but are not limited to: Independently manages a panel of patients with acute and chronic medical, social, and behavioral conditions, with an emphasis on caring for older, frail, homebound patients. Visits patients in their homes in the community. Assesses, identifies, and diagnoses patients' general medical problems and defines the medical management. Develops and implements an individualized treatment plan to achieve optimal patient health outcomes. Demonstrates leadership and works collaboratively with HBPC interdisciplinary team members. Summary Home Base Primary Care Long Beach is expanding and is seeking a well-qualified Registered Nurse to serve as a Home Based Primary Care (HBPC) Registered Nurse in Geriatric Extended Services. The RN role is to provide case management and nursing care to a caseload of veterans in the HBPC program. Care is provided to patients with the goal of achieving quality patient care outcomes, increased continuity of care and minimizing health care cost. Currently there are two (2) vacancies. Responsibilities The HBPC RN is expected to demonstrate leadership in delivering and improving holistic care through collaborative strategies with others. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.) NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification. Grade Determinations: The following Scope, Education and Dimension criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade. An ADN or Diploma in Nursing AND 2 years of professional nursing experience in which one year is equivalent to Nurse I, Level 2; OR a BSN and 1 year of professional nursing experience equivalent to the Nurse I, Level 2; OR a Master's degree in nursing (MSN) and no additional professional nursing experience; OR a Master's degree in a *related field with a BSN and no additional professional nursing experience. Nurse II Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. A BSN with 2 years of professional nursing equivalent to Nurse I, Level 3; OR an MSN with one year of specialized nursing experience equivalent to Nurse I, Level 3; OR a Master's degree in a *related field with a BSN and one year of specialized nursing experience equivalent to Nurse I, Level 3; OR a Doctoral degree in Nursing with no professional nursing experience; OR a Doctoral degree in a *related field with a BSN with no additional professional nursing experience. Nurse III Executes position responsibilities that demonstrate leadership, experience and creative approaches to management of complex client care beyond the immediate practice setting. What You’ll Do:

As a Developmental Play Partner (also referred to as a Developmental Paraprofessional (DPP), you’ll work one-on-one with children, to help them develop communication, social, and daily living skills. Your role is hands-on—helping kids learn through play and conversation to grow in their ability to think, create, communicate and meaningfully connect with others. Your day-to-day responsibilities will be focused on working directly with children in their homes or in our therapy centers using fun and engaging interaction to help them grow. You will be expected to document session notes and progress toward goals. You will also work directly with clinicians and supervisors to gain skills and allow for career growth.

Compensation & Schedules:

  • Pay Range: $22-$2 per hour, based on experience and education
  • Thoughtfully designed schedules: We offer a range of predictable schedules with a single, direct rate of pay, including part-time roles with flexible 5-, 3-, or 2-day-per-week options
  • Opportunities to pick up additional hours on Saturdays

  • A valid driver's license and reliable transportation (within a 30–40-minute drive radius)
  • The ability to complete certain physical tasks
    • Lift and carry up to 40 pounds
    • Physically position your body with clients including sitting, kneeling, or crouching.
    • Respond to client needs, including running or adjusting positioning quickly to ensure safety and well-being.
