- Monitor client well-being and report any significant changes or concerns to the appropriate authorities.
- Collaborate with families, caregivers, and healthcare professionals to ensure a coordinated approach to care.
- Organize and lead community engagement activities and support groups.
- Maintain accurate and confidential client records in compliance with organizational policies.
- Advocate for client rights and ensure they receive appropriate services and support.
- Respond to client emergencies and provide immediate assistance as needed.
- Continuously seek professional development opportunities to stay abreast of best practices in community and social care.
- High school diploma or equivalent; Associate's or Bachelor's degree in Social Work, Psychology, Sociology, or a related field is preferred.
- Previous experience in a similar role, preferably within elderly care or community services.
- Excellent interpersonal and communication skills, with the ability to build rapport with diverse individuals.
- Strong empathy, patience, and a non-judgmental attitude.
- Proficiency in record-keeping and basic computer skills.
- Knowledge of local community resources and support systems.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation are required.
- CPR and First Aid certification is a plus.
- Demonstrated commitment to ethical conduct and client confidentiality.
