• Conduct comprehensive needs assessments with clients and their families to identify specific care requirements and goals.
• Coordinate the delivery of a wide range of services, including in-home support, medical appointments, transportation, and social activities.
• Act as the primary point of contact for clients and their families, providing ongoing support, education, and advocacy.
• Liaise with healthcare professionals, social workers, and other community agencies to ensure integrated and holistic care.

• Bachelor's degree in Social Work, Nursing, Gerontology, or a related field. Master's degree preferred.
