**JOB PURPOSE:**
To provide and coordinate the provision of skilled nursing care under physician supervision, through competent application of nursing process.
**KEY RESPONSIBILITIES:**
- Provides competent nursing care & interventions to home health patients according to state and industry and profession
- Supervises care provided by Home Health Aides, Licensed Practical Nurses (LPN) assigned to the care team (completing supervisory visits of HHAs as required)
- Helps maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan of care, making referrals to other disciplines as needed
- Demonstrates intermediate knowledge of home care and competency in discipline for specific patient care skills, required for the provision of care
- Conducts field visits to (1) evaluate & initiate start of care and (2) conduct clinical outcomes monitoring, follow-up and care performance improvement initiatives
- Utilizes accepted company teaching materials in patient/family education
- Assesses, monitors, documents and reports progress of patient's health and condition using required documents via clinician preferred technology, and within established timelines
- Completes documentation timely, accurately, and at the point of care, according to industry standards (e.g., OASIS assessments, SOC/Admissions, Recertifications, ABN's, subsequent visits, physician orders, care coordination, etc.)
- Honors patients/residents' rights to be notified in advance of care, treatment and services to be provided and works with patient team to adhere to physician-approved care plan. Ensures equitable treatment, self-determination, individuality, privacy, property and civil rights.
- Complies with corporate compliance program
- Meets agency productivity standards and utilizes time and resources effectively and efficiently
- Participates in center/agency surveys (Licensure/Joint Commission), mandatory in-services, case conferences, staff meetings, and programs (e.g., Performance Improvement Program) as required

Home Health Aide provides patients with assistance in activities in daily living. The Home Health Aide performs varied activities and moderately complex administrative/operational/customer support assignments. Assists patients within the home with daily living activities. Typically works on semi-routine assignments.
A **Home Health Aide** ( HHA ):
+ Provides direct patient care to patient under direction of the RN and according to the Aide Plan of Care (POC).
+ Correctly assists the patient with self-administered medications by opening bottle caps for the patient, reading medication labels to the patient, checking the dose being self-administered against the prescribed dose on the container label and observing the patient takes the medication
+ Consistently takes accurate temperature, pulse and blood pressure measurements and recognizes and reports abnormal results to supervisor
+ Helps patient maintain good personal hygiene by performing or supervising bathing, grooming, skin care, shaving, oral care, nail/foot care and other activities
+ Assists in feeding patients. Is able to communicate basic principles of nutrition, observe and record food and fluid intake when necessary. Safely positions patient for meals and feeds or assists in self feeding
+ Assists with patient toileting including use of bed pan/urinal, change and position catheter bags and bag change procedures on well-regulated ostomies
+ Provides necessary skills to safely assist the patient with patient mobility, exercises, positioning/turning, transfers and ambulation per Plan of Care and CenterWell Home Health policy
+ Provides necessary skills to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care. Documents interactions with patients, caregivers, doctors and other staff members appropriately, legibly, thoroughly and in the amount of time allowed
+ Practice acceptable infection control principles. Provide a clean, safe and comfortable environment
+ Willingly assists with other household duties including light laundry, bed changing and bed making, light meal preparation, light housekeeping and shopping (if no other assistance is available and an MD order is present).

As a **Home Health Occupational Therapist Assistant** , you will:
+ Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient's Plan of Treatment.
+ Assist in the implementation of vocational/education programs and activities established by registered Occupational Therapist designed to restore, reinforce, and enhance task performances, diminish or correct pathology, and to promote/maintain health and self-sufficiency.
+ Design/adapt equipment and working and/or living environment. Fabricate devices to assist and improve function and independence and participation in the program and/or community where possible.
+ Provide therapeutic treatment and instruction to patients as directed by the qualified Occupational Therapist and in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
+ Report information and observations to Occupational Therapist and/or Clinical supervisor, document observed information in patient records and prepares clinical notes. Assist with preparation of progress reports. Maintain and submit documentation as required by the Company and/or facility.
+ Instruct patients and family members regarding home programs as well as care and use of adaptive equipment.
+ Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's needs.
+ Design community reintegration activities, as appropriate, to assist the client in the physical reconditioning effort, and/or the psychological adjustment and coordinate the plan with members of the interdisciplinary team.

As a **Home Health RN Case Manager** , you will:
+ Provide admission, case management, and follow-up skilled nursing visits for home health patients.
+ Administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
+ Confer with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision. Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation.
+ Coordinate appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers).
+ Report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis.
+ Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
+ Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflects current plan of care. Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
+ Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
+ Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records and confers with other health care disciplines in providing optimum patient care
