Maximus is seeking a dedicated Clinical Services Manager to support the Colbert Consent Decree Illinois Outreach and Assessment team. In this role, you will act as a liaison to serve as a single point of contact for all transition entities. This would include communicating with Illinois state agencies, MCO's (managed care organizations), court appointed staff, and transitional prime agencies.

**This position requires up to 15% travel annually to Illinois.**

About the Program:

We have contracted with the State of Illinois Department of Healthcare and Family Services (HFS) to manage comprehensive assessment processes for various screening and assessment services, specifically the Williams and Colbert Consent Decrees Outreach and Transitional Assessment. Outreach includes in-person visits with Colbert Class Members in Cook County Nursing facilities and Williams Class Members in SMHRF facilities which provide education on their rights and evaluate their level of interest in potential transition to the community.

Transitional Assessments are conducted in-person with Colbert Class Members in Cook County Nursing facilities and Williams Class Members in SMHRF facilities interested in transitioning to the community. Evaluators assess individuals' functional needs in several areas to determine the level of support necessary for successful community transition.

Minimum Requirements

- BA/BS

+ Assisting with activities of daily living, including but not limited to: feeding, bathing, toileting, providing water, answering the phone, and oral care. Within the Patient Care Technician duties and responsibilities and dependant on the patient population, assists the Registered Nurse with patient monitoring, including but not limited to: obtaining, recording and reporting vital signs, weights, and measuring and recording inputs/outputs. May perform EKGs as required.
+ In coordination with the Registered Nurse and other medical center staff, will maintain a clean and safe environment by providing a newly admitted patient with supplies (e.g. ICS/ Admission Kit), ensuring a clean and tidy bedside, and neat linens. Performs other related duties as necessary.
+ Under the direction of the Nurse Manager or designee, the Patient Care Technician may act as a sitter to ensure that patient safety needs are met. As a sitter, the Patient Care Technician will assist the registered nurse assigned to the patient in providing care including hygiene, nutrition, mobility, monitoring and engage the patient in diversional and therapeutic activities.

Position Description:
  • Telephonic and field-based outreach to engage clients in our care management program.

HCBA PATIENT CARE NAVIGATOR

The Center for Elders' Independence is a PACE (Program of All-Inclusive Care for the elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, and promoting autonomy, quality of life and the ability of individuals to live in their communities.

The Position: Reporting to the Director of HCBA, the Patient Care Navigator performs as Home and Community-Based Care Based Alternatives (HCBA) department generalist and first point of contact. Independently performs assigned duties. Interacts with all levels within and outside the organization to release or receive information and handles issues within scope of the role. Requires extensive knowledge of related vendors operations; communicates directly with participants and facilitates appointments.

Pay rate range for the Patient Care Navigator at CEI is $22.96 - $34.43 per hr. Pay rate range based on market for the Patient Care Navigator position as well as experience, skills, abilities and work history.

Duties and Responsibilities:

  • Acts as initial point of contact for all department calls and triages calls appropriately.
  • Provides administrative support for non-PACE care coordination functions including printing/faxing/scanning of participant documents.
  • Orders office supplies for HCBA.
  • Manages ordering and installation of emergency alert system for participants.
  • Initiates calls for clinical or administrative information as requested.
  • Receives information and assures delivery to correct recipient.
  • Sends clinical information as directed. Forwards incoming calls and or arranges call appointments as needed.
  • Arranges for discharge transport, follow up internal and external appointments with associated transport.
  • Receives SNF acceptance information and notifies case manager. Receives MD orders and assures distribution to appropriate parties. Communicates with SNF's regarding CEI transfer paperwork and appropriate actions to be taken. Confirms receipt of orders and arrival of participant at SNF.
  • Open to best practice model suggestions. Offers input and complies with department standards of care and practice as well as quality improvement activities.
  • Listens to and documents participant concerns and facilitates satisfactory resolution.
  • Interfaces with participants for data collection, scheduling and care coordination needs.
  • Performs other duties as needed to support the CCSC team and assist with provision of care coordination services.
Qualifications:
  • Associate degree or higher, and 3-5 years of progressively responsible experience related to health plan operations preferred
  • Demonstrated excellent customer service and oral communication skills required
  • Health plan experience with member coordination/communication and clerical management experience required (usually 3-5 years) preferred.
  • Basic knowledge of PC's and related software required. Accuracy, productivity, dependability and good attendance is critical.
  • Ability to perform duties in heavy workload environment.
