Duties:

- Designing and implementing comprehensive, evidence-based programs that support individuals across the continuum of care for substance abuse and mental health conditions.
- Enhancing access to integrated behavioral health services, including education, prevention, treatment, recovery, support, and crisis intervention.
- Expanding and improving clinical services for patients and families with a focus on quality, safety, and outcomes.
- Clinical oversight over direct medical services and multidisciplinary teams including supervision of clinical protocols, coordination of integrated care pathways, compliance with regulatory standards and promoting continuous quality improvement.

Premium Pay
In addition to the hourly salary, eligible applicants may receive premium pay for the items listed below:
* Bilingual pay:Qualified employees who meet the criteria found (page 16)shall receive an additional seventy $0.70 cents per hour for all hours paid.

Adult Correctional Health Services (ACHS)
The ACHS program provides medical, dental, nursing, infection control, health education and pharmaceutical services at a community standard of care. Mental health services include 24-hour emergency triage and crisis intervention, suicide prevention, treatment beds, individual and group therapy, substance use counseling, medication assessment and discharge planning to all adult inmates in the County's five (5) correctional facilities.
The Comprehensive Care Nurse II assigned to ACHS will perform the following specialized duties:
* Obtain vital signs such as temperature, blood pressure, pulse, respiration, and pain scale assessment
* Perform comprehensive and focused physical assessments and refer to higher level of care as necessary, including but not limited to:
* Comprehensive history taking
* Interpreting vital signs and physical assessment results
* Wound Care
* Documenting in Subjective-Objective-Assessment-Plan (SOAP)format on electronic health record
* Implementing standardized nursing protocols
* Monitoring outcomes
* Review of health records, summarize and synthesize client information, and give comprehensive hand off at shift change or outside facility transfers utilizing discharge summary form or verbal report

The ACHS facility is a 24-hour, 7 days a week facility. Therefore, incumbents must be able to work on weekends, evenings or night shift, and holidays.
Nursing staff work six (6), twelve (12) hour shifts, and one eight (8) hour shift per two (2) week pay period. Days off are every other weekend and rotating week days.

Veteran Healthcare Navigator
35 hours per week / $25.00 - $6.00 per hour
(The actual compensation will be determined based on experience and other factors permitted by law.)
Summary: Under direct supervision of the Program Director provides services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. Primary duties include case management, care coordination, health education, consultation, administrative duties, collaboration with the VA, COC and other healthcare service providers. Candidates must be familiar with the Los Angeles County health care system, specifically within the United States Department of Veterans Affairs.

Summary: The Director of Welcoming Team Services provides leadership and oversight of programs composed of multiple, multi-disciplinary service teams that both enroll and place members into FSP teams. These teams are Department of Mental Health (DMH) - funded Full Service Partnership (FSP) teams and provide integrated, wrap around services to individuals with mental health needs and other potential co-occurring conditions who are experiencing poverty. - Maintain an outreach log of prospective veterans in SSVF program and provide the number of Veterans outreached in the community on a bi-weekly basis to program intake staff.
- Provide information and resource linkage for veterans in the service area by direction to Veteran Resource Center
- Work as part of a team, makes regular visits to local institutions and other community sites where homeless veterans are known to be present (i.e., community shelters, community access centers, local veteran's services, hospital emergency rooms, local jail, soup kitchens, community parks, other social service agencies, etc).
- Establish and maintain good rapport and regular contact with local agency workers and administrators (i.e., police, fire, MET team, emergency room workers, parks & recreation workers, etc.) who are a source of referrals of homeless veterans in need of assistance and services
- Participate actively in the program's established system(s) for accepting and responding to community-based requests to provide assistance for homeless veterans.
- Implement strategies for and conducts outreach to hard-to-engage homeless veterans who are identified by local community referral sources, or who are living on the streets in the identified community/service area
- Assists SSVF Team with Veterans in locating requested services to include: housing, food, health care, benefits, transportation and entitlements, etc. on a case by case basis or at Program Manager or Director's request.
- Provide identification of and prescreening for eligibility of potential applicants.
- Coordinate initial screening and assessment with case management team, participates in Stand Downs and all other VA/community outreach activities as directed by Program Manager
- Provide information about SSVF services and information on community resources to current and potential participants
- Work cooperatively with team members to help ensure smooth and seamless delivery of comprehensive services to program participants
- Participate in staff and community meetings and in the day-to-day operations of the program as needed
- Maintain all records and documentation needed for program compliance and accountability in accordance with MHALA standards and SSVF requirements, and all billing and supporting documentation shall be entered into LAHSA's HMIS (data record system) within 24 hours from the date of service delivery

- Help problem solve with the Veteran to identify an immediate safe place, even if temporary, to stay within the Veteran's network of family, friends, or other social supports.
