**PURPOSE** **AND** **SCOPE:**
Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with organization policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP).
**PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**
**Patient Related**
Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.
+ Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.
+ Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient prior to discharge.
+ Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.
+ Monitors patients performing self-care under the supervision of RN.
**Staff Related**
+ Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.
**Education/Communication:**
+ Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
+ Initiate Solution Delivery System (SDS) system.
**RECORD KEEPING:**
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
+ Enters all treatment data into the designated clinical application in an accurate and timely manner.
+ Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure
omitted entries are completed or corrected by appropriate staff.
+ Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
+ Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
+ Collect, label, appropriately prepare and store lab samples according required laboratory specifications.
+ Ensure collection of lab specimens by appropriate lab courier.
**INFECTION CONTROL:**
+ Assists in collecting information for infection control audits.
+ Supports staff and patient Adherence to infection control practices.
+ Follows infection control Policies and Procedures
+ Participates in and reinforces infection control education of patients and families under the direction of the CM or designee as allowed by state law.
**TRANSITIONAL CARE UNIT:**
+ Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care
+ Reinforces all education and care related matters as it relates to the new patient as allowed by state law
+ Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.
+ Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.
+ Sets and preforms a treatment on all machines used in the transitional care unit.
+ Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.
+ Assists with coordination of transition to patient's modality choice.
+ Assists with supply ordering and inventory for TCU

    The Riverside County Fire Department is seeking to fill a vacancy for an Emergency Medical Services Coordinator in Riverside. The Incumbent will plan, coordinate and implement the certification and training component of the Riverside County Emergency Medical Services (EMS) system or the Riverside County Fire Department EMS program; maintains the quality assurance components of the EMS system; monitors, evaluates and investigates the transport and emergency care provided to trauma victims; develops plans, policies, and procedures to enhance coordination and response to the public health and medical components of emergencies; performs other related duties as required.

    The Emergency Medical Services Coordinator is the journey level classification in the Emergency Medical Services series and reports to an appropriate supervisory or manager level position. Incumbents plan, coordinate, and implement training, quality assurance, and the trauma monitoring components of the County's EMS systems or programs for the Riverside County Fire Department EMS programs, including the development of standards and protocols, investigations of violations and evaluations regarding the appropriateness of emergency medical services and/or transport. Incumbents also coordinate with public health and medical system stakeholders to develop response plans, implement training, and conduct disaster exercises. The Emergency Medical Services Coordinator is distinguished from the Senior Emergency Medical Services Coordinator in that the latter is the advanced journey level classification that provides technical supervision over subordinate Emergency Medical Services personnel.

    •Provide staff services to assigned EMS advisory committees; assist other EMS agency staff by interpreting policies and procedures; administer certification and recertification of EMS personnel.

    •Monitor field performance of EMS personnel to identify deficiencies and determine training needs; evaluate qualifications of hospitals applying for Base Station Hospital certification based on established criteria and make recommendations on certification; monitor Base Station Hospital operations for compliance with medical control protocols and training requirements.

    •Review and evaluate suitability of basic training and continuing education programs to ensure compliance with all legislative requirements.

    •Act for the Senior Emergency Medical Services Coordinator in their absence.

    •Implement, conduct, and develop training programs and curriculum for the County Fire Department for emergency medical programs such as First Responder and EMT-1.

    •Address and provide staff work for all EMS issues concerning operations and personnel; participate in training plan; participate in Safety Committees; serve as contact person between the Fire Department and Department of Public Health.

    •Develop all-hazards emergency response plans, policies, and procedures for public health and medical system stakeholders.

    •Conduct training on emergency response plans, policies, and procedures.

    •Conduct all-hazards, multi-agency exercises that include the development and distribution of exercise documents (e.g., exercise plan, situation manual, exercise evaluation guides, after action reports, improvement plans). OPTION I

    License: Possession of a valid license to practice as a Registered Nurse in the State of California.

    Experience: Two years as a Registered Nurse in an acute care hospital setting, one year of which must have been in an emergency department or as a Disaster Coordinator.

    OPTION II

    Certificate: Possession of a valid Paramedic license and Advanced Cardiac Life Support certificate as defined by the American Heart Association or REMSA approved equivalent.

    Experience: Four years as a Paramedic.

    ALL OPTIONS

    License: Possession of a valid California Driver's License.

    Knowledge of: State of California statutes and regulations pertaining to emergency medical services; basic medical terminology and equipment relating to the certification procedures for Emergency Medical Technicians; group dynamics and community organizations; techniques of collecting and compiling data pertinent to emergency medical care systems; principles and practices of training; methods of program evaluation; curriculum development and statistical record keeping.

    The County of Riverside Behavioral Health Department's (HHOPE) Homeless Housing Opportunities, Partnerships & Education program is currently seeking a Behavioral Health Services Supervisor. The program is dedicated to reducing and preventing homelessness in Riverside County through a comprehensive, integrated approach that combines housing and behavioral health services. Our objective is to engage, stabilize, and support individuals and families experiencing homelessness by connecting them to permanent housing, healthcare, and community-based supports.
    Supervisors within HHOPE play a vital role in advancing this mission by leading multidisciplinary teams that deliver high-quality, person-centered care. They ensure program fidelity, promote collaboration with internal and external partners, and uphold HHOPE's commitment to equity, innovation, and outcomes that foster long-term housing stability and wellness for all participants. The position requires occasional fieldwork; therefore, a valid California Driver's License is mandatory. The work schedule follows a 9/80 format, with hours from 8:00 a.m. to 5:30 p.m.
    We are looking for candidates who

    • Monitor staff performance through regular supervision, coaching, and performance evaluations, ensuring alignment with departmental goals and professional development plans.
    • Coordinate workflow and staffing coverage to maintain consistent field presence and office operations, including scheduling, approving timecards, and managing time-off requests.
    • Oversee documentation and reporting in systems such as HMIS, ELMR, and EPIC to ensure accuracy, timeliness, and compliance with funding and regulatory requirements.
    • Track grant objectives and expenditures , working collaboratively with leadership to ensure programs operate within budget and meet outcome targets.
    • Facilitate communication across interdisciplinary teams, community partners, and internal departments to promote coordinated care and efficient service delivery.
    • Respond to field and program needs by providing guidance on complex cases, crisis intervention, and linkage to appropriate internal or community resources.

    Additional Duties
    Plan, assign, direct, supervise, and coordinate the work of a multidisciplinary staff providing behavioral health treatment services, social and rehabilitation services, and consultation and education services to consumers, public and private agencies, and the community.

    Evaluate overall unit activities, including methods and procedures for effectiveness and consistency with program goals and objectives,; recommending and implementing changes in program, policies, and procedures as necessary.

    Evaluate performance of personnel and take or recommend appropriate course of action; interview, select, and recommend the hiring of new staff members.

    Provide guidance to subordinates on departmental philosophy, goals and objectives, and policies and procedures through casework conferences, staff meetings, and work review; assist and participate in the development and coordination of in-service training and staff development programs.

    Make or review decisions on difficult consumer assessment and casework matters.

    Work with other public, private, and community organizations providing health and human services to mentally and emotionally disabled individuals to gain and ensure cooperation in order to achieve program goals; participate in behavioral health conferences and local area committees; may participate in regional or countywide community needs assessment and program planning development and evaluation activities.

    Assist in preparing annual budget requirements and monitor expenditures to ensure unit is within funding provisions; prepare reports and correspondence regarding workload statistics, casework and unit methods, and procedures. Experience: Two years of post-license professional experience in a psychiatric or behavioral health setting providing direct client services which must have included: client assessment and evaluation, development and implementation of appropriate treatment plans, providing psychotherapy or behavioral health counseling, and providing indirect services involving information, consultation, and educational services to staff and community.

    License: Possession of a valid license to practice as a Licensed Clinical Social Worker, Licensed Professional Clinical Counselor, or Licensed Marriage and Family Therapist issued by the California State Board of Behavioral Science Examiners.

    Candidates recruited from out-of-state must possess an out-of-state license to practice in one of the above stated options. Individuals hired under this option shall obtain their license to practice issued by the California Board of Behavioral Sciences within three years of date of hire. During the interim three years, the incumbent shall be eligible for a Mental Health Professional Licensing Waiver Request approved by the State of California Health and Human Services Agency.

    Candidates qualifying with the Licensed Professional Clinical Counselor option must have completed additional education and training as defined below:

    1) Six semester units or nine quarter units specifically focused on the theory and application of marriage and family therapy, OR a named specialization or emphasis area on the qualifying degree in marriage and family therapy, marital and family therapy, marriage, family, and child counseling, or couple and family therapy;

    2) No less than 500 hours of documented supervised experience working directly with couples, families, or children; and

    PURPOSE AND SCOPE:

    Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with organization policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP).

    PRINCIPAL DUTIES AND RESPONSIBILITIES:

    Patient Related

    Education:

    • Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.

    Treatment:

    • Welcome assigned patients and inquire as to their wellbeing since their last treatment.
    • Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state Law.
    • Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
    • Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).   
    • Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
    • Monitor patients’ response to dialysis therapy. 
    • Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
    • Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure
    • Obtain Hemostasis and apply appropriate dressings.   
    • Evaluate the patient prior to discharge.
    • Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification   
    • Responsible for calculating and entry of individual patients’ dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.   
    • Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.   
    • Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification   
    • Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.   
    • Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.   
    • Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.  
    • Perform and document any intervention for unusual patient status and document patients' response to intervention.
    • Evaluate patients’ vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.
    • Obtain all prescribed laboratory testing and prepare specimens for collection.   
    • Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.   
    • Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.   
    • Monitors patients performing self-care under the supervision of RN

    Staff Related

    • Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.
    • Collaborates with the Clinical Manager and Education Coordinator in driving organization quality standards through education regarding all ESRD regulatory and legal requirements and the practice of Continuous Quality Improvement. 
    • Provides developmental support to applicable clinical and facility staff. 

    Education/Communication:

    • Participates in implementing  educational programs for PCT staff including contests and other team-based programs.
    • Assists in monitoring processes in the delivery of patient care through regular Continuous Quality Improvement (CQI) activities.
    • Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices

    • Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
    • Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
    • Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
    • Initiate Solution Delivery System (SDS) system.

    RECORD KEEPING:

    • Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
    • Enters all treatment data into the designated clinical application in an accurate and timely manner.
    • Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure

    omitted entries are completed or corrected by appropriate staff.

    • Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.   
    • Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.   
    • Collect, label, appropriately prepare and store lab samples according required laboratory specifications.   
    • Ensure collection of lab specimens by appropriate lab courier.   

    INFECTION CONTROL:

    • Assists in collecting  information for infection control audits.
    • Acts as a change agent for infection control practices within clinic. Under the direction of the CM or designee monitors staff and patient adherence to infection control practices.   
    • Reinforces new hire employee adherence to infection control Policies and Procedures
    • Supports ongoing staff and patient engagement on infection control practices under the direction of CM or designee for example participation in patient lobby days and staff meetings to reinforce infection control practice.
    • Participates in and reinforces infection control education of patients and families under the direction of the CM or designee as allowed by state law   

    TRANSITIONAL CARE UNIT:

    • Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care   
    • Reinforces all education and care related matters as it relates to the new patient as allowed by state law
    • Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.   
    • Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.   
    • Sets and preforms a treatment on all machines used in the transitional care unit.
    • Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.
    • Assists with coordination of transition to patient’s modality choice.   
    • Assists with supply ordering and inventory for TCU

    • Day to day work includes desk and personal computer work and interaction with patients, facility staff, and physicians. The position requires travel between assigned facilities and various locations within the community. Travel to regional, Business Unit and Corporate meetings may be required.
    • The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/materials  
    • The position provides direct patient care that regularly involves heavy lifting and moving of patients and assisting with ambulation. Equipment aids and/or coworkers may provide assistance. This position requires frequent, prolonged periods of standing and the employee must be able to bend over. The employee may occasionally be required to push and/or pull equipment, exerting up to 15 pounds of force. The employee may be required to lift and to lower solutions on a frequent basis of up to 30 lbs., and on an occasional lift basis up to 40 lbs., as high as 5 feet. There is a two-person assist program and "material assist” devices for the heavier items.

    EDUCATION AND LICENSES:

    • High School diploma or G.E.D. required.
    • Must have a valid PCT national certification or state specific certification as defined by Center for Medicaid/Medicare Services (CMS)All appropriate state licensure, education, and training (if any) required.

    EXPERIENCE AND REQUIRED SKILLS :

    • At least two years previous patient care experience in a hospital setting or a related facility preferred.
    • Continued employment is dependent on successful completion of the organization dialysis training program and successful completion of CPR certification.

    ADDENDUM:

    Home Therapies Program (Not Home Assist)

    This list pertains to task performed in the Home Therapies program under the direct supervision of the Home Therapies registered nurse (HT RN)  and in compliance with state law.

    • May assist HT RN on Home Visit as directed   
    • Reinforces PD/HHD patient education under supervision of HT Registered Nurse as permitted under state law   
    • Schedules and contacts patients regarding appointments   
    • Weigh patient and obtain vital signs   
    • Collect patient’s treatment records and review for completion. Notify RN of incomplete / missing records.
    • Cleaning and prepping treatment room; preparing charts prior to clinic visit   
    • Preparing lab tubes and requisitions & assisting with lab draw days both pre and post draw (PCT certified phlebotomy or per state regulations)   
    • Clerical duties as assigned (faxing, mailing to physician offices etc.)   
    • Set-up of the home hemodialysis machine and PD Cycler   
    • Assist with exit site care when directed by HT RN   
    • Obtain home hemodialysis water and dialysate samples and process for testing as directed by HT RN
    • Perform water dialysate collection and processing for testing as directed by the HT RN   
    • Creates and sends patient reminders (supply inventory for order, Kt/V collection, bringing meds to clinic visit for review)   
    • Prepare initial patient chart for admission. Complies and maintains medical records according to company policy and procedure and in compliance with all appropriate regulatory requirements   
    • Maintains logs as directed and applicable
    • Support patient registration and use of connected health   
    • Provide patient home support as indicated and permissible under state law and applicable regulations

    Inventory of Home Program

    • Maintains par levels of home department supplies   
    • Files home patient packing slips/invoices   

    Inventory for Patients

    • Assists patient with supply management and contacting customer service

    The rate of pay for this position will depend on the successful candidate’s work location and qualifications, including relevant education, work experience, skills, and competencies.

    Hourly Rate: $24 - $30
