1. Patient Assessment and Monitoring: Conduct thorough assessments of patients' health conditions, including vital signs, symptoms, and medical histories. Continuously monitor patients' progress and respond promptly to changes in their condition.
2. Medication Administration: Administer medications and treatments as prescribed by physicians, ensuring accurate dosages, routes, and timing. Maintain accurate medication records and assist in medication reconciliation.
3. Wound Care: Provide wound care and dressing changes for patients with complex wounds. Monitor and report any signs of infection or deterioration in wound status.
4. Collaborative Care: Work closely with the interdisciplinary healthcare team, including physicians, registered nurses, therapists, and other healthcare professionals, to develop and implement comprehensive care plans.
5. Patient Education: Educate patients and their families on medical conditions, treatment plans, and self-care techniques. Provide emotional support and address any questions or concerns.
6. Documentation: Maintain accurate and thorough electronic health records (EHR) of patient assessments, interventions, and outcomes. Ensure compliance with all legal and regulatory requirements.
7. Infection Control: Follow strict infection control protocols to prevent the spread of infections within the facility. Adhere to established safety procedures and protocols.
8. Emergency Response: Respond effectively to medical emergencies within the facility, including initiating CPR and other life-saving measures as necessary.
9. Quality Assurance: Participate in quality improvement initiatives and assist in data collection and analysis to enhance patient care and outcomes.

• Current state LPN licensure
• Graduation from an accredited practical nursing program
• American Heart Association BLS (Basic Life Support) in-person certification
• Strong clinical assessment skills
• Excellent communication and interpersonal abilities
• Attention to detail and strong organizational skills
• Ability to work collaboratively within a healthcare team
• Min 1yr recent experience in long-term acute care or critical care setting

Salary: $2215 / Week

The Long-Term Acute Care Registered Nurse provides planningand delivery of direct and indirect patient care to medically complex patientsin long-term acute settings.

Minimum Requirements:

• Current RN license for the state in which the nursepractices in

• One (1) year nursing experience in a long-term acute carefacility preferred

• Experience in acute rehabilitation for vents, wound care,cardiac, and stable post trauma patients

• Current CPR if applicable

• TB questionnaire, PPD or chest x-ray if applicable

• Current Health certificate (per contract or stateregulation)

• Must meet all federal, state and local requirements

• Must be at least 18 years of age

• Active RN License
• Degree from accredited nursing program

• Eighteen months of recent experience in an Long Term Acute Care setting
• Experience with Vents, Trachs, Drips, EG Tubes and able to read Tele-strips
• Other requirements to be determined by our client facility

• Provide care for 5-6 patients.
• Adjust and perform in fast paced environment
• Keep up with many admissions and discharges
• Be able to monitor and administer medications (orally, subcutaneously, and through an IV)
• Educate patients and family on surgical processes and how to manage their illness/injury
• Monitor and adjust specialized equipment used on patients, and record and interpret electronic displays
• Interpret and understand telemetry readings, notifying appropriate medical team members of any irregularities
• Perform the duties required of a registered nurse at all times
• Care for patients with pulmonary HTN, respiratory failure, COPD, CHF, and A-Fib
• Use computer equipment to monitor cardiac and other vital information
• Start IV's, draw blood, see the patient from start to finish
