• Provide direct personal care and support to elderly clients in their homes, including assistance with daily living activities (bathing, dressing, feeding, mobility).
• Develop and implement personalized care plans in collaboration with clients, families, and healthcare professionals.
• Monitor clients' health status and report any changes or concerns to the care manager or appropriate medical personnel.
• Administer medication as prescribed and ensure proper documentation.
• Assist with light housekeeping, meal preparation, and companionship.
• Organize and facilitate social and recreational activities to promote engagement and well-being.
• Supervise and mentor junior support staff, providing guidance and training.
• Conduct home safety assessments and implement necessary modifications.
• Maintain accurate and confidential client records and documentation.
• Liaise with families, doctors, and other healthcare providers to ensure coordinated care.
• Participate in team meetings and contribute to the continuous improvement of care services.
• Handle on-call duties and emergency situations as required.

• High School Diploma or equivalent; relevant certifications (e.g., CNA, HHA) preferred.
• Minimum of 3 years of experience in elder care, home health, or a related social care field.
• Demonstrated experience in a senior or supervisory support role.
• Excellent communication, interpersonal, and empathy skills.
• Ability to work independently and manage time effectively.
• Proficiency in basic medical monitoring and first aid.
• Understanding of elderly care needs and best practices.
• Must be reliable, punctual, and possess a strong sense of responsibility.
• Willingness to undergo background checks and provide references.
• Familiarity with care planning and documentation systems.
