Description:
*Does pre-visit planning for scheduled patients.
*Communicates the patient needs and recommended treatment to the staff on their team.
*Maintains & updates the team's patient panel.
*Identifies, creates, & maintains panels of high risk sub populations for care management.
*Tracks performance measures monthly and shares with the team, staff and patients.
*Participates in ongoing quality improvement activities using the Model for Improvement.
*Contacts patients for follow up diagnostic tests, referrals, appointments, or medical follow up as
appropriate.
*Schedules nursing visits for follow up.
*Tracks and follows up on referrals.
*Provides patient education and self-management support.
*Triages patients on the team's panels who call or walk in and directs them appropriately.
*Documents observations, assessments, nursing interventions, treatments, and referrals according
to CN documentation standards.
*Provides direct nursing care back up as the team needs dictate.
Requirements
Good written and verbal communication skills
Meet pre-employment credentialing clearances (Background Check, Drug Screen, etc.)
ORIENTATION SCHEDULE: Dept. Specific and Cerner Training will be provided on your first day. Department Manager will review Badging Process. (Note- Will need to take 2 forms of ID to get badge printed.)

VOM was founded in 1967 by Pastor Richard Wurmbrand, who was imprisoned 14 years in Communist Romania for his faith in Christ. His wife, Sabina, was imprisoned for three years. In the 1960s, they were ransomed out of Romania and came to the United States. Through their travels, the Wurmbrands spread the message of the atrocities that Christians face in restricted nations, while establishing a network of offices dedicated to assisting the persecuted church. The Voice of the Martyrs continues in this mission around the world today.

Summary. The International Ministry Project Management Office (IM PMO) serves to support the International Ministry staff, who manage more than 1500 projects in more than 75 countries. The PMO Coordinator helps promote project management best practices, compliance with the organization's project management standards, and develop efficient PM systems and processes. The PMO Project Manager works at the direction of the IM PMO Director, managing projects, tasks and initiatives which fall outside of the PMO's regular operational duties. At key stages in the year, this position also helps meet operational demands as needed.
Subordinate Titles and Scope of Supervisory Responsibility. None.
Core Duties
- Assists in promoting the Project Management Standards defined in the IM Handbook.
- Manages and collaborates on various PMO tasks, projects, and initiatives, especially related to:
- Developing processes and procedures that facilitate active project management.
- Fielding and implementing effective project management tools and systems.
- Creating and/or analyzing dynamic project reporting at the request of the IM PMO Director and IM Executives.
- Promote healthy dialogue with various stakeholders and departments to maximize our services.
- At key stages in International Ministry's annual project cycle, assists with the PMO's operational and administrative functions.
Skills and Experience
- (Required) Bachelor's degree from an accredited institution.
- (Required) 3 years of experience in Project Management.
- (Required) Strong computer skills, including proficiency in Microsoft Suite and the ability to quickly learn new software and applications.
- (Required) Excellent communication, conflict resolution, and problem-solving skills.
- (Required) Outstanding interpersonal and teamwork skills, organizational capability, and strong attention to detail.
- (Required) Exceptional time management talents and ability to manage multiple tasks.

Working Conditions
- Typical office environment and hours.
- (Required) Must live in the area and work at VOM’s headquarters.
- Requires sitting for long periods. Work processes are predominantly executed on a computer with long periods of screen time.
