Position Details

The RN Case Manager assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient or member's health status. Manages utilization and practice metrics to further refine the delivery of care model to maximize clinical, quality, and fiscal outcomes.

  • Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for the identified population.
  • Develops systems of care that monitor progress and promote early intervention in acute care situations.
  • Assists with the design, implementation, and evaluation of the advanced patient centered care model.
  • Assesses the healthcare, educational and psychosocial needs of patients or members.
  • Designs an individualized plan of care and fosters a team approach by working collaboratively with the patient or member, family, primary care provider, and other members of the health care team to ensure coordination of services.
  • Continuously evaluates laboratory results, diagnostic tests, utilization patterns and other metrics to monitor quality and efficiency results for assigned population.
  • Works to appropriately apply benefits and utilization management serving as a resource to the patient or member and healthcare team.
  • Maintains required documentation for all case management activities.
  • Collects required data and utilizes this data to adjust the treatment plan when indicated.

Job Duties

The Trauma RN Case Manger is responsible for ensuring optimal quality, clinical and financial outcomes; networking, assuring availability and coordinating needed patient and family services, identifying and advocating for individual needs of patients, identifying and resolving patterns of variance having a negative quality, clinical and financial impact, and creating action plans and systems to optimize clinical and system outcomes.

Job Duties:

  • Assumes the role of clinical expert.
  • Coordinates patient care placing rehabilitation and social service consults per protocol.
  • Facilitates weekly multidisciplinary team meetings.
  • Participates in direct patient care in a preceptor role.
  • Acts as a preceptor or adjunct faculty for student nurses as needed.
  • Participates in discharge planning in conjunction with other healthcare members.
  • Documents care management and progress report in each trauma service patient's chart on a weekly basis and as needed.

  • Coordinates data collection for formal quality improvement focus audits.
  • Develops and revises quality improvement data collection instruments and documentation guidelines.
  • Provides immediate one-on-one education concerning patient care and system.

  • Coordinates the implementation and evaluation of clinical standards of practice, performance improvement projects and service standards.
  • Seeks input from management and implements recommendations in the process of developing and administering budgets; understands key financial terms and budget concepts; understands the expense and revenue aspects of the budgets; controls operational costs.
  • Manages the budgets on a day-to-day basis.
  • Responsible for recruitment, interviewing, and selection of staff at assigned sites.
  • Coordinates and conducts orientation programs.
  • Schedules staff for inpatient and outpatient services; maintains personnel files.
  • Serves as resource to Clinical Nutrition staff.
  • Develops and implements performance improvement activities; reports these activities to designated committees.
  • Represents and participates in performance improvement teams as needed.
  • Designs clinical nutrition policies, protocols, standards and work systems for inpatients and outpatients.
  • Seeks approval when needed from administration and medical staff.

Job Duties

The Trauma RN Case Manger is responsible for ensuring optimal quality, clinical and financial outcomes; networking, assuring availability and coordinating needed patient and family services, identifying and advocating for individual needs of patients, identifying and resolving patterns of variance having a negative quality, clinical and financial impact, and creating action plans and systems to optimize clinical and system outcomes.

This role is full-time, 40 hours weekly; Monday through Friday; 7:00 a.m.- 3:30 p.m. A minimum of 3 years RN work experience is required.

Job Duties:

  • Assumes the role of clinical expert.
  • Coordinates patient care placing rehabilitation and social service consults per protocol.
  • Facilitates weekly multidisciplinary team meetings.
  • Participates in direct patient care in a preceptor role.
  • Acts as a preceptor or adjunct faculty for student nurses as needed.
  • Participates in discharge planning in conjunction with other healthcare members.
  • Documents care management and progress report in each trauma service patient's chart on a weekly basis and as needed.
  • Provides one-on-one consultation with multidisciplinary team members identifying and implementing strategies to enhance patient care and outcomes.
  • Assists in collection and interpretation of data pertinent to the Trauma PI Program and Trauma Registry.
  • Maintains trauma roster and updates on concurrent basis.
  • Summarizes concurrent daily chart rounding information into monthly quality improvement report and forwards initial review to leadership for secondary review and action.
  • Coordinates data collection for formal quality improvement focus audits.
  • Develops and revises quality improvement data collection instruments and documentation guidelines.
  • Provides immediate one-on-one education concerning patient care and system.

Education Graduate from Specialty Training Program- (Required), Bachelor's Degree-Nursing (Preferred)
