Located on the Community Regional campus, Burnett Extended Care Center is our brand-new 154-bed transitional care facility. The RN Case Manager is responsible for providing support and clinical expertise across the continuum of care, from admission through discharge or transition to the next level of care. Collaborates with physicians and the interdisciplinary team to coordinate patient care, enhance clinical outcomes, support patient and family engagement, and manage the cost and efficiency of care delivery.
