The Registered Nurse Case Manager is a (RN) who oversees and participates in the provision of appropriate clinical interventions and support to an assigned caseload of hospice patients.

QUALIFICATIONS
Education: Graduation from an accredited school of nursing; Bachelor of Science in Nursing (BSN) preferred.
Experience: Minimum of one year nursing experience in a hospital, home health agency, hospice, or related program.
Licensure: Licensed as a Registered Nurse (RN) in Arkansas.
Additional Requirements: Valid Arkansas driver's license, personal automobile, automobile insurance required.

PRIMARY RESPONSIBILITIES
  1. Manages the patient care functions after the initial admission and assignment to a designated RN Case Manager.
  2. Assists in the admission process as requested.
  3. Interprets, when necessary, Arkansas Hospice, Inc. policies and procedures for nursing staff and others to assure understanding and compliance.
  4. Coordinates and collaborates with interdisciplinary team members for the management of the plan of care on an ongoing basis to meet patient/family needs.
  5. Updates the Plan of Care on an ongoing basis keeping other team members current regarding the progress of the patient's physical status and any changing needs for care.
  6. Facilitates effective written and verbal communication with patients/families and other healthcare providers.
  7. Maintains communication with attending physician and community organizations concerning resources for assigned patients/families.
  8. Monitors pain and symptom control to assure optimal patient comfort.
  9. Coordinates CNA scheduling needs with Patient Care Coordinator/Area Manager or designee.
  10. Initiates referrals for other hospice related care and services.
  11. Serves as a preceptor for nurses in orientation, students, and trainees.
  12. Reviews documentation of staff as directed; assures documentation is complete, accurate, and timely according to organizational policies and practice standards.
  13. Participates in interdisciplinary team meetings, case conferences, care planning activities, and other committee meetings as assigned.
  14. Actively participates in Performance Improvement activities.
  15. Participates in community outreach activities to educate the public about Arkansas Hospice, Inc. services.
  16. Coordinates and provides education on hospice and hospice services for patients/families, other team members, and the community.
  17. Rotates on-call duty to provide 24-hour service to patients and families as assigned.
  18. Maintains strict confidentiality at all times.
  19. Utilizes and models excellent customer service skills at all times; seeks opportunities to assist patients, families and co-workers; demonstrates teamwork and cooperation.
  20. Accepts direction and follows instructions from supervisor; seeks additional information as needed; works with minimal supervision.
  21. Adheres to all organizational and departmental policies and procedures.

Qualifications

  • License or Certification:
    • Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
    • If licensure is required for one’s discipline within the state, individual must hold an active license.
    • Must meet eligibility requirements for CCM® or ACM™ certification upon entry into this position OR within two years of entry into the position.
    • CCM® or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position.

·    Minimum Qualifications :

    •  For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
    •  For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor’s degree and graduate degree is preferred.
    • 2 years of rehabilitation experience preferred.

CDM Smith currently has a new opportunity for a Senior Project Manager with previous experience managing industrial water and/or wastewater projects to join our Industrial Business unit. In this position, you will be the main point of contact for planning and design of water, water reuse, and wastewater projects and will assist with business development activities throughout the U.S. CDM Smith’s Industrial Business unit services clients in a variety of industrial sectors including; High Tech, Data Centers, Chemicals, Rubber & Plastics, Petrochemical, Oil & Gas, Food & Beverage, Mining and Manufacturing.

*** This position is hybrid with a minimum of two days in the office and can be based at any of our Industrial offices in the United States. Some of those cities include Houston, Irvine, Fairfax VA, Portland, New Orleans, Raleigh, Atlanta, Chicago, Phoenix, Austin, Concord CA, Edison, Albany, Boston, and Wadsworth, OH ***

As a member of this team, you would contribute to CDM Smith's mission by:

• Managing and serving as the lead Project Manager on water/wastewater projects

• Providing technical guidance, senior leadership, and mentoring engineering staff to enhance our water/wastewater design capabilities

• Being responsible for scope, schedule, budget development, monitoring and adherence for projects managed

• Completing Quality Assurance/Quality Control of key deliverables and assisting on projects managed by others in the office

• Assisting Client Service Leaders with technical marketing for clients throughout the U.S.

• Participating in professional societies relevant to the industry.

• Building and maintaining positive working relationships

with key decision makers in our clients’ organizations

Minimum Qualifications:

• Bachelor's Degree

• 10 years of related experience

• PMP (PMI), CCM or DBIA certification is required (within 12 months of hire or promotion onto the Approved Project Manager list)

• Domestic and/or international travel may be required. The frequency of travel is contingent on specific duties, responsibilities, and the essential functions of the position, which may vary depending on workload and project demands

Equivalent additional directly related experience will be considered in lieu of a degree.

Preferred Qualifications:

• Professional engineering (PE) license, strongly preferred.

• Bachelor's degree in civil, environmental, or chemical engineering, or related degree.

• Previous experience working on and managing projects for Industrial water/water reuse/wastewater clients.

• Previous experience managing multi-discipline project teams.

• Excellent communication, networking and team building skills.

• Previous experience working directly with clients

• Master’s degree.

• Previous business development experience including preparation of proposals and scopes of work and costs estimates for industrial clients

Pay Range Minimum:

$119,829

Pay Range Maximum:

$209,726

Additional Compensation:

All bonuses at CDM Smith are discretionary and may or may not apply to this position.

Work Location Options:

Hybrid Work Options may be considered for successful candidate.
