About the Role:
- Under the immediate supervision of the Clinical Manager, the Case Manager is responsible for each component of the client's treatment planning, client's behavioral health plan, including all programmatic implementation.
- Case managers will also coordinate aftercare plans such as MAT service appointments, mental health continuums, primary physician appointments, relapse prevention support within the community including NA/AA meetings and identify transportation for client at time of discharge. The Case Managers will also meet with their assigned case load individually every 7 days to adhere to documentation requirements through Medicaid standards.
