Job Duties

  • Coordinates the implementation and evaluation of clinical standards of practice, performance improvement projects and service standards.
  • Seeks input from management and implements recommendations in the process of developing and administering budgets; understands key financial terms and budget concepts; understands the expense and revenue aspects of the budgets; controls operational costs.
  • Manages the budgets on a day-to-day basis.
  • Responsible for recruitment, interviewing, and selection of staff at assigned sites.

  • Serves as resource to Clinical Nutrition staff.

  • Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance to physician orders.
  • Oversees and assures development, implementation, and updates to the individualized patient plan of care, as appropriate.
  • Manages and documents phone calls from physicians, clinicians, patients, referral sources, and communicates patient updates/new orders to clinicians. Uses coordination notes to document, as needed and appropriate.
  • Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate.
  • Coordinates all aspects of care with all disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and any other applicable healthcare providers.
  • Follows-up on lab and other clinical diagnostic test, physician contact, and significant changes in the patient condition to ensure adequate physician notification, follow-up, and needed plan of care modifications and communicates such to clinicians.
  • Schedules, prepares for, facilitates, and documents case conference/SOC reports and facilitates effective exchange of information across disciplines especially with adverse findings, changes in patient condition, daily and urgent updates, as necessary.
  • Assists clinicians in coordinating the transfer and discharge of patients from agency services as indicated by the physician.
  • Receives report from field clinicians prior to scheduled days off on patient status and ongoing needs.
  • Processes new orders and updates the visit frequency, as appropriate, when the oncall RN takes supplemental verbal orders which alter frequency going forward.
  • Writes and processes orders when taking verbal orders directly from the physician and communicates such to field clinicians.
  • Assures payer change documentation is completed properly and timely, as required.
  • Reviews clinician visit notes weekly to ensure timely, complete, appropriate, and accurate submission of all documentation by field staff. Takes necessary action to correct adverse findings and communicates trending to clinical director.
  • Reviews, evaluates, and supervises service delivery to ensure appropriateness of care and utilization of services, equipment, and supplies through activities such as random patient visits, medical record reviews and case conferences.
  • Enters infections and incidents/occurrences into the online Risk Management Incident Reporting System, as specified by policy.
  • Assists in the orientation of new agency personnel.
  • Provides direction and leadership to clinical team members in collaboration with the clinical director.
  • Provides direct patient care, as necessary, in accordance to scope of practice and physician orders.
  • Participates in QAPI program.
  • Assures compliance with and ensures timely follow up on daily clinical and coding edits.
  • Directs clinicians in utilizing best practice interventions when finalizing Plan of Care for all patients.
  • Participates in on-call rotation.
  • Follows-up with On-Call events daily.
  • Receives report from weekend and after-hours clinicians admitting new patients.

Summary
Responsible for coordinating and directing the delivery of care for an assigned case load of patients who are receiving services in the home setting in collaboration with the interdisciplinary care team and Home Health Care Navigator. Job Duties

  • Coordinates and directs the delivery of care for an assigned case load of patients who are receiving services in the home setting in collaboration with the interdisciplinary care team and Home Health Care Navigator. Formulates an individualized plan of care according to physician orders that incorporates the analysis of assessment data and current scientific findings. Collaborates with the physician and Home Health Care Navigator. Determines home health as the appropriate level of care for the patient as well as skilled need for services ordered based on home health Conditions of Participation. Relays significant changes in patient status to the physician and other members of the interdisciplinary care team in a time period consistent with patient needs.
  • Delivers patient care based on the medical plan of treatment established by the physician and protocols using a patient family centered approach. Provides educational opportunities for patients, families, and clinical staff focusing on end-of-life issues, palliative care, advance directives, chronic disease management, pain management, symptom control, home care, hospice, and discharge planning. Promotes patient/caregiver autonomy. Evaluates effectiveness of teaching and modifies education based on patient needs and goals.


Minimum Qualifications

  • Specialized Diploma Nursing or
  • Associate's Degree Nursing
  • 1 year recent experience providing care in a Home Health care setting and demonstrate skills in OASIS data set collection. and
  • 1 year Demonstrated ability to coordinate and manage a patient caseload. or

Summary

Ensures and/or provides patient care in accordance with the RN job description. Assumes 24-hour accountability for the direction and management of patient care within assigned area. Manages material, financial, and human resources within the assigned unit.

Job Duties

  • Oversees administrative and operational issues for the designated care delivery team.
  • Functions as a department team leader to support unit and hospital goals and objectives.
  • Uses effective and appropriate communication styles with patients, significant others, visitors, staff, and professional colleagues.
  • Supports the Culture of Safety by following established policies and procedures which include identification and timely patient safety event reporting.
  • Ensures staffing to meet patient care needs and operational guidelines.
  • Fosters accountability of staff for adherence to policies/procedures/professional practice.
  • Participates in coaching, counseling, and mentoring of unit personnel.
  • Supports the Professional Practice Model and participates in efforts related to the key components.
  • Assumes unit, clinical, and administrative responsibilities in the absence of the Director and under the supervision of the Patient Care Services management staff.
  • Ensures and/or provides patient care in accordance with all responsibilities in the RN job description.
  • Ensures completion of the unit's schedule in a timely and equitable manner.
  • Utilizes evidence based research findings within patient care delivery, communicates research findings to co-workers and/or the professional community, attends research continuing education offerings, assists with data collections, and serves as a participant in ongoing research studies.
  • Provides clinical, consultative, technical, or informational services as needed to staff, other disciplines, patients, and/or families.

Minimum Qualifications
  • Bachelor's Degree Nursing
  • 3 years Registered Nurse in an acute care hospital setting.
  • Ability to multi-task and prioritize work.
  • Proficient in clinical practice protocols for medical procedures and patient care.
  • Skilled in monitoring/assessing the performance of self, other individuals, or organizations to make improvements or take corrective action.
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire

Preferred Qualifications
  • Master's Degree Nursing

Physical Demands

Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
