Essential Functions

• Performs clinical, technical, administrative and clerical duties as directed by the physician and within the scope of training and certification, including point of care testing. Receives critical lab values, reports value to physician and documents notification in medical record.

• Prepares patients for examination and treatment. Obtains and records patients' chief complaints, current medications, vital signs, pain screening and allergies. Pends orders for physician signature.

• Prepares exam and treatment rooms. Prepares and maintains supplies and equipment for treatments, cleaning/sterilizing instruments as needed.

• Assists physician with examinations, tests, procedures, and office-based surgery. Apply splints, casts, dressings and remove sutures with competency. Report and assist during emergencies.

• Prepares and administers medications and immunizations as ordered by the physician, including p.o., vaginal and rectal suppositories, intradermal injection and intramuscular injection, after demonstrating competency and passing medication administration test.

• Receives and returns clinically related telephone calls after communicating with physician or registered nurse regarding medical concerns of the patient.

• Teaches patient and provide printed information, on physician request, about diagnosis, self-care within the scope of assigned duties, health promotion, disease prevention and community healthcare resources. Patient education must be documented in the medical record.

• Documents data and cares in the patient medical record.

• Completes medical insurance and other related forms for physician review. Communicates as needed with the patient, referral source and payor between visits. Completes patient registration, scheduling and billing tasks, pre-authorization and other general office/clerical duties as assigned. Schedules tests and treatments, as ordered by the physician.

• Performs other duties as assigned.

Supervisory/Management Responsibilities

• This is a non-management job that will report to a supervisor, manager, director, or executive.

Minimum Requirements

• Education - Completion of an accredited medical assistant program preferred.

• Experience - No experience required.

• In lieu of the education requirement stated above, will accept confirmation of the successful completion of the Prisma Health Certification Institute (PHCI) Fast Track Certified Medical Assistant (CMA) program.

Essential Duties and Responsibilities:
+ Responsibilities of the Certified Nursing Assistant/Patient Care Assistant include, but are not limited to, the following:
+ Bathing (sponge bath, shower, tub, shampoo)
+ Back rubs
+ Grooming (nail filing, hair care, oral hygiene)

Minimum Requirements:
+ Successful completion of a nursing assistant/home health aide or competency training and evaluation program as applicable, per State/Federal regulations
+ Successful completion of Maxim's Clinical Competency Program for home health aides, including successful completion of examinations with score of 80% or higher, and demonstration of all required skills per State/Federal regulations
+ Ability to read and follow written instructions and document care given
+ Meets Maxim and applicable State Background Investigation requirements
+ Possession of current CPR certification and First Aid training/certification as applicable, per State regulations

**Position Purpose:**
Works with members and providers to close care gaps, ensure barriers to care are removed, and improve the overall member and provider experience through outreach and face-to-face interaction with members and providers at large IPA and/or group practices. Serves to collaborate with providers in the field, to improve HEDIS measures and provides education for HEDIS measures and coding. Supports the implementation of quality improvement interventions and audits in relation to plan providers. Assists in resolving deficiencies impacting plan compliance to meeting State and Federal standards for HEDIS. Conducts telephonic outreach, while embedded in the providers' offices, to members who are identified as needing preventive services in support of quality initiatives and regulatory/contractual requirements. Provides education to members regarding the care gaps they have when in the providers office for medical appointments. Schedules doctor appointments on behalf of the practitioner and assists member with wraparound services such as arranging transportation, connecting them with community-based resources and other affinity programs as available. Maintains confidentiality of business and protected health information.
+ Acts as a liaison and member advocate between the member/family, physician and facilities/agencies.
+ Acts as the face of WellCare in the provider community with the provider and office staff where their services are embedded.
+ Advises and educates Provider practices in appropriate HEDIS measures, and HEDIS ICD-10 /CPT coding in accordance with NCQA requirements.
+ Assesses provider performance data to identify and strategizes opportunities for provider improvement.
+ Collaborates with Provider Relations to improve provider performance in areas of Quality, Risk Adjustment, Operations (claims and encounters).
+ Schedules doctor appointments for members with care gaps to access needed preventive care services and close gaps in care in the provider's office.
+ Conducts face-to-face education with the member and their family, in the provider's office, about care gaps identified, and barriers to care.
+ Conducts telephonic outreach and health coaching to members to support quality improvement, regulatory and contractual requirements.
+ Arranges transportation and follow-up appointments for member as needed.

**Education/Experience:**

Pay Range: $26.50 - $47.59 per hour
