This position provides assistive care to patients. The incumbent facilitates the efficient delivery of patient care, supports the professional role of the licensed staff, and provides support and assistance as directed by licensed staff in rendering basic healthcare to the patient. ResponsibilitiesPrepares patients for examinations and diagnostic procedures according to clinic procedures such as recording temperatures, pulse rate, blood pressure, and responses to standardized medical history questions.
Reports all observed patient concerns and changes in condition to a licensed staff member.
Performs Clinical Laboratory Improvement Amendments (Clia) Waved Lab tests as identified by department and reports results to the appropriate personnel.
Performs additional skills such as suture removal, straight catheterization, and injections (Im, Subq, and intra-dermal).
Acts as a chaperone for health care providers during patient examination as requested and assists with procedures.
Monitors and cleans assigned patient examination rooms, unit areas, and unit equipment reporting outdated supplies, stock levels, and restock set levels of items and necessary equipment repairs.
Answers telephones, forwards telephone messages and under the direction of licensed staff schedules appointments, procedures, and diagnostic tests.
Assists in routine clerical functions such as filing of medical information in patient's records, locating and compiling department charts, obtaining medical records from other providers, and entering charges for billing purposes.
Attends and completes mandatory education including infection prevention and control, safety, CPR, equipment usage, and procedures according to hospital and department policies.
May perform patient phlebotomy in accordance with clinic procedures and physician orders after specialized training.
May apply leads and monitors for diagnostic procedures such as EKG and ultrasound scan under the direct supervision of licensed personnel.
May assist inpatient and family education by providing educational material and general health information to patients and family members.
May assist with transportation of patients to ancillary departments or inpatient units as directed by licensed staff.
May instruct in Basic Life Support (Bls) requirements.
May perform IV access.
May perform other duties based on department assigned.
Knowledge / Skills / AbilitiesThis hiring department is responsible for assessing new employee's skills, identifying gaps, and coordinating further education.
Ability to perform the essential functions of the job as outlined above.
Demonstrated human relation and effective communication skills.
Demonstrated proficiency in the specific clinic's specialty skills within six weeks of hire. *Specialty skills may not be performed until after the new hire completed basic competency testing.*
Ability to withstand the physical demand of being on your feet for long periods of time, of moving and lifting patients, and of reaching for and handling equipment.
QualificationsRequired
Verifiable completion of one (1) of the following:
- State approved EMT course.
- One (1) year of medical assistant experience in an ambulatory care clinic or other healthcare setting.
- Medical assisting education program.
- University hospital medical assistant certification program.
Licenses Required
Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.
Additional license requirements as determined by the hiring department.
Working Conditions and Physical DemandsEmployee must be able to meet the following requirements with or without an accommodation.

Reporting Relationship
Intake Manager
Responsibilities include the following:
1. Responsible for transcribing all applicable information from the Intake Referral Form and patient information received from the referral source into the computer system correctly.
2. Handles all faxes incoming to Intake Department and distributes appropriately.
3. Calls referral sources to acknowledge receipt of faxes as applicable.
4. Logs all new referrals according to the current process.
5. Verifying patient demographics on restart patients as requested.
6. Enters patients info in CPR+
7. Creates invoices and charges credit cards as applicable.
8. Makes outbound calls to follow up on a patient discharge, follow up on any missing information needed to process a referral such as an H&P, H&W, and address or obtain orders from a hospital or MDs office.
9. Follow-up on patient consent and financial obligation forms.
Minimum Qualifications:
- Must possess excellent oral and written communication skills, with the ability to express technical issues in “layman” terms. Fluency in a second language is a plus.
- Must be friendly professional and cooperative with a good aptitude for customer service and problem solving.
Education and/or Experience:
- Must have a High School diploma or Graduation Equivalent Diploma (G.E.D.) or Higher.
- Prior experience in a pharmacy or home health company is of benefit.

Responsibilities:
+ Ensure efficient patient flow and prepare treatment rooms and equipment for patient examinations.
+ Provide appropriate procedural support following department-specific guidelines.
+ Perform initial intake of patients, including interviewing patients and families to obtain chief complaints, vital signs, and allergy information.
+ Accurately record and update relevant patient information.
+ Perform daily Quality Controls (QC) for all Point of Care (POC) testing and obtain specimens.

+ Clean and stock patient care rooms between patients and maintain appropriate inventories of all medical supplies.
+ Stock and maintain all patient-related forms and educational materials.
+ Utilize equipment appropriately and notify the Practice Manager in a timely manner of equipment/facility breakdown or damage.
+ Ensure documentation is legible and falls within the appropriate MA scope and organizational documentation guidelines.
+ Demonstrate accuracy in basic data entry and data retrieval of paper/electronic patient records.

Work Environment
The candidate will work in two offices located in Smithtown and West Islip. The shift is Monday to Friday, either 8 AM to 4 PM or 9 AM to 5 PM, with an 8-hour shift including a half-hour unpaid lunch (paid for 7.5 hours per day). No weekend work is required. The role requires familiarity with taking manual vitals, using holter monitors, and performing EKGs.
Pay and Benefits
The pay range for this position is $18.00 - $23.00/hr.

Ensures that pertinent data is obtained, updated, verified, and accurately entered into the computer system on all patients at each visit.
Secures appropriate signatures on all mandatory hospital documentation and consents.
Interviews all patients in a professional and courteous manner.
Serves as an interdepartmental liaison.
Verifies third party information.
Ensures referrals are obtained and collected; and that pre-authorization for services is verified.
Answers phone calls, takes accurate messages, and refers caller or message to appropriate party.
Researches non-routine questions and responds back to the caller in a timely manner.
Ensures entries pertaining to the patient are made in the electronic record.
As directed, communicates with various hospitals and physician offices regarding patient related information and documents.
Provides information in accordance with patient confidentiality standards.
Covers the new patient intake telephone line.
Enters patient intake information into the transplant electronic medical record.
Obtains medical records as required.
Notifies patient navigator and nurse reviewer regarding necessary follow-ups.
Performs initial intakes for inpatient transfers and promptly notifies patient navigator.
Mails out information packets and directions.
Makes referrals to the financial coordinators as needed.
Review and follow-up with MyChart folders i.e., patient scheduling request, patient advice.
Maintains an accurate filing system to meet the needs of the department, following established guidelines. Ensures that lab slips, x-ray reports, etc. are filed and/or prepped for scanning and/or scanned in a timely manner for Medical Records.
Assists with billing procedures and forms, assists in the preparation of documents and forms sent to/received from referring physician directed.
Prepares information for billing/processing by Finance.
Collects statistics as directed for administrative and regulatory purposes and produces reports as required.
Collects payments for services and follows established policies for cash collection and reconciliation.
Provides additional support to our Physician Referral System forms process.
**Qualifications:**
· Minimum of 2 years' experience working as a Medical Office Assistant or Patient Care Coordinator.
· Must have experience working with EPIC.
· Spanish speaking is preferred but not required.
