Position Summary: Eventus WholeHealth is dedicated to providing an integrated model of care for adults residing in skilled nursing and assisted living facilities. We work with an extensive network of healthcare professionals, including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.

Essential Duties and Responsibilities: As a Medical Director, your primary focus will be overseeing medical responsibilities at skilled nursing facilities (SNFs), which includes conducting History & Physicals (H&Ps) and regulatory visits. You will collaborate with other Eventus WholeHealth doctors for oversight visits at assisted living facilities (ALFs) within your territory, aiming for meaningful MD exposure. This role involves:

  • Meeting with Regional Care Coordinators (RCC) and Administration
  • Being present at facilities during NP/PA rounds
  • Guiding quality care for our providers while meeting oversight visit expectations
  • Supervising all Nurse Practitioners in your region
  • Developing a systems-based approach with facility staff and providers
  • Participating in monthly Primary Care Conference Calls
  • Working with our Audit team to comply with standard care and billing criteria

SNF Duties: Your responsibilities will also include administrative duties for SNFs, signing MD-only paperwork (e.g., FL2s, Rehab Certifications, etc.), attending QA meetings, and rounding with patients. You will be the attending physician responsible for every other 60-day regulatory visit, while the NP will see patients every 30 days. Each SNF will have a full-time NP managing acutes, discharges, tuck-ins, and providing regular follow-ups for your care plan.

Qualifications/Education:

  • Licensed MD or DO
  • Minimum of 3 years experience in Long Term Care/Skilled Nursing Facility settings
  • AMDA Certification is a plus, but not mandatory
  • Experience in Medical Directorship is a plus
  • Strong leadership and organizational skills
  • Excellent written and verbal communication skills

What You'll Do

As the Medical Director , you will provide day-to-day leadership for the clinic teams and deliver high-quality care, an exceptional patient experience and a best-in-class culture, acting as a Champion of the Hopscotch Care Model and a Culture advocate for your team and your patients. Specifically, you will:
  • Provide leadership, oversight and support for the team to drive day-to-day execution against critical priorities to enable high-quality care, a great patient experience, engagement across the team and a best-in-class culture
    • Define objective goals and top priorities for the clinic and for each member of the team, in partnership with Hopscotch central team members, to support clinical, growth, and culture

    • Actively participate in all Clinical Forums , providing feedback, driving action and ensuring that key discussion points and takeaways are cascaded to your clinic Providers and other team members as appropriate

    • Partner with the Care Center Manager to outperform against target metrics and outcomes, including patient experience, STARS/HEDIS, care coordination, utilization and cost of care, through collaboration and engagement of your clinical care team

About You

You'll be a great fit for this position if you have 2+ years of post-Residency experience and have experience in managing teams. You must be Board Certified in Internal Medicine or Family Medicine, have an active, non-probationary North Carolina state medical license, and be authorized to work in the US. An ideal candidate will also have:
  • Familiarity with innovative clinical models, specifically those that serve high acuity members
  • Experience with the Medicare population, including in-home care as well as documentation and compliance requirements
  • Experience caring for "at-risk" panel of patients

**JOB SUMMARY**
Responsible for registering and screening donors under the direction of the Center Manager. Operates within the scope of EU Guidance on Good Manufacturing Practice and FDA cGMP 21CFR.
**ESSENTIAL JOB FUNCTIONS**

**Core**
+ Facilitates donor flow throughout the plasma collection center with a sense of urgency, minimizing wait time by working carefully and efficiently.
+ Registers applicant donors.
+ Assists qualified donors in completing the screening process.
+ Assesses the self-administered health history, refers donors to medical staff when appropriate and performs health screening procedures such as blood pressure, pulse, weight, temperature.
+ Performs finger sticks to obtain a sample to obtain hematocrit and total protein levels.
+ Prepares equipment by performing PQ/calibrations and verifying required maintenance has been performed as outlined in the SOP.
+ Performs troubleshooting of equipment within scope of training.
+ Performs Haemonetics Autopheresis setup and programming with understanding of alarm signals during operation, shut down or in the event of power failure.
+ Responds and resolves equipment alarms.
+ Responds to donor issues related to collection.
+ Notifies management or medical personnel if an alarm or donor issue requires needle adjustments and/or resticks.
+ Prepares collection machines for the plasmapheresis procedure and plasma collection, including donor set-up, monitoring and disconnect in compliance with Standard Procedures and FDA guidelines, as needed.
+ Monitors the safety and care of the donor during the plasmapheresis process.
+ Maintains alertness and awareness to any reaction a donor may have during or after the plasmapheresis process and notifies appropriate medical staff.
+ Ensures accurate recording of donor data in the electronic donor information management system as outlined in the SOPs.
+ Records incidents that occur during the pheresis process, such as machine alerts and alarms, volume variances and donor adverse events.
+ Maintains a clean, orderly, and efficient work environment and ensures sufficient operating supplies are available as needed.
+ Records any required documentation accurately and in compliance with company SOPs.
+ Maintains data integrity and confidentiality of all personnel, donor, and plasma collection center information.
+ Monitors donors for possible issues with their donation, understands how to handle donor adverse events and collection exceptions.
+ Explains plasma collection center policies & rules to new donors about the donation process, proper nutrition, and any other information pertinent to the donor or donor safety.
+ Works with plasma collection center's staff to meet production goals.
+ Ensures all donor questions are answered timely, accurately, and professionally and alerts management of any issues or concerns.
+ Assists plasma collection center for audits.
+ Follows all SOPs and regulatory policies and procedures, including the employee handbook.
+ Remains current on SOPs changes.
+ Follows all Health Safety & Environmental and Occupational Safety and Health Administration (OSHA) policies and procedures.
+ Operates computer systems including National Donor Deferral Registry (NDDR) system and Cross Donation Check System (CDCS) within approved guidelines.

**JOB SUMMARY**
Responsible for registering and screening donors under the direction of the Center Manager. Operates within the scope of EU Guidance on Good Manufacturing Practice and FDA cGMP 21CFR.
**ESSENTIAL JOB FUNCTIONS**

**Core**
+ Facilitates donor flow throughout the plasma collection center with a sense of urgency, minimizing wait time by working carefully and efficiently.
+ Registers applicant donors.
+ Assists qualified donors in completing the screening process.
+ Assesses the self-administered health history, refers donors to medical staff when appropriate and performs health screening procedures such as blood pressure, pulse, weight, temperature.
+ Performs finger sticks to obtain a sample to obtain hematocrit and total protein levels.
+ Prepares equipment by performing PQ/calibrations and verifying required maintenance has been performed as outlined in the SOP.
+ Performs troubleshooting of equipment within scope of training.
+ Performs Haemonetics Autopheresis setup and programming with understanding of alarm signals during operation, shut down or in the event of power failure.
+ Responds and resolves equipment alarms.
+ Responds to donor issues related to collection.
+ Notifies management or medical personnel if an alarm or donor issue requires needle adjustments and/or resticks.
+ Prepares collection machines for the plasmapheresis procedure and plasma collection, including donor set-up, monitoring and disconnect in compliance with Standard Procedures and FDA guidelines, as needed.
+ Monitors the safety and care of the donor during the plasmapheresis process.
+ Maintains alertness and awareness to any reaction a donor may have during or after the plasmapheresis process and notifies appropriate medical staff.
+ Ensures accurate recording of donor data in the electronic donor information management system as outlined in the SOPs.
+ Records incidents that occur during the pheresis process, such as machine alerts and alarms, volume variances and donor adverse events.
+ Maintains a clean, orderly, and efficient work environment and ensures sufficient operating supplies are available as needed.
+ Records any required documentation accurately and in compliance with company SOPs.
+ Maintains data integrity and confidentiality of all personnel, donor, and plasma collection center information.
+ Monitors donors for possible issues with their donation, understands how to handle donor adverse events and collection exceptions.
+ Explains plasma collection center policies & rules to new donors about the donation process, proper nutrition, and any other information pertinent to the donor or donor safety.
+ Works with plasma collection center's staff to meet production goals.
+ Ensures all donor questions are answered timely, accurately, and professionally and alerts management of any issues or concerns.
+ Assists plasma collection center for audits.
+ Follows all SOPs and regulatory policies and procedures, including the employee handbook.
+ Remains current on SOPs changes.
+ Follows all Health Safety & Environmental and Occupational Safety and Health Administration (OSHA) policies and procedures.
+ Operates computer systems including National Donor Deferral Registry (NDDR) system and Cross Donation Check System (CDCS) within approved guidelines.
+ Understands and performs timely proper computer entry.
+ Documents comments on donor data including exceptions.
+ Answers the telephone in a courteous and friendly manner and provides information to caller(s) or transfers calls to appropriate personnel, as needed.

Detailed Duties and Responsibilities:
  • Provide primary-care services in accordance with Eventus WholeHealth site-specific protocols and consistent with the standard of care for the specialty of family medicine, geriatric medicine, and/or internal medicine as it pertains to long term care facility work.
  • Review and sign MAR's as requested by facilities, sign all orders specific for a supervising Physician while at the facility and or within 24 hours if faxed or emailed to include phone orders. Review and address labs within 24 hours of them being sent to you. Manage all Scheduled medication prescriptions specific to your facility's patients and specialty and as per Eventus Whole Health's Scheduled Medication Rx Policy.
  • Provide facility with adequate time each visit to allow for paperwork and admin duties.
  • Work with facility to manage systems. Attend care plan meetings and QA/PI (Quality Assurance/Performance Improvement) meetings as requested by facility and or supervisor.
  • On Call: As of this date, all providers are expected to be available to their facilities between the hours of 8AM and 5PM by text or phone on the scheduled days of work. We have an ECare team that supports us. Triage Policies and OnCall duties and scheduling will be modified as needed and all providers are expected to adhere to the newest agreed upon triage schedule.
  • Submit copies of documentation and billing records to Eventus WholeHealth according to the billing schedules.
  • Establish and maintain open and positive communications with facility staff and administration. Provide verbal and/or written instruction or feedback regarding medications and other pertinent caregiver information. Meet with Administrator or designated facility care coordinator equivalent prior to the start of your patient care visits each day and prior to leaving facility each day.
  • Be knowledgeable of and adhere to Eventus Whole Health standards, policies and procedures. Be aware of and adhere to all legal and regulatory agencies' rules and guidelines and professional ethical standards. Comply with all regulatory agencies governing health care delivery.
  • Maintain patient confidentiality at all times including appropriate use of Cell phone, emails text messaging, patient charts and EMR. Agrees to abide by and be knowledgeable of HIPAA rules and regulations and CPT coding guidelines.
Qualifications/Education:
  • Licensed MD or DO
  • 3 years in Long Term Care/Geriatric Care preferred.
