Job Summary Assists clinician(s) in providing coordinated services & case management for adult clients with mental health and/or substance abuse issues. Key Responsibilities 1. Prepares for and conducts social skill building and other therapeutic activities in order to meet the goals of each individual Treatment Care Plan, which is designed to meet the age-specific needs of each client. 2. Provides direct care, under the supervision of a licensed/credentialed clinician, by co-leading therapy groups and performing assigned case management activities to assist in achieving established client outcomes. 3. Observes clients and communicates to the Treatment Team about the effectiveness of the Treatment Plan. Documents client/family responses as well as collateral contacts. Identifies variances and contributes suggestions to the revision of the Treatment Plan. Initiates further interventions as determined by the Treatment Team. 4. Implements established behavioral management plans for a program or for individual clients. Ensures a safe environment, ensures client’s agreement with and adherence to the plan, implements rewards and consequences, and provides appropriate clinical support to patients/families to meet established goals of the plan. 5. Assists with transportation arrangements, as approved by the Program Manager, to help clients attend program and meet aspects of their treatment plan which can’t be met on site. 6. Utilizes community resources and makes appropriate referrals in support of the treatment plan and the continuing care plan a client begins after the discharge process and afterwards. 7. Connects with area resources through service, health and employment agencies, professional groups, committees, and consortiums in order to be well-versed in accessing necessary services for each client upon discharge. 8. Participates in treatment conferences, staff meetings, and in-service training as required to ensure a quality care experience for clients. 9. Participates in the facilitation and coordination of education groups, and community meetings. 10. Performs other duties as may be assigned by the Program Manager. * Associates in Drug and Alcohol Recovery Counseling (DARC), Human Services, or related field OR relative experience as outlined below * Bachelor's Degree in Social Work, Psychology, or related field is preferred * In lieu of education requirement, at least two (2) years of experience in a behavioral health or substance abuse setting may be considered * Familiarity with Alcoholics (or Narcotics) Anonymous 12-Step Program * CPR, Crisis Prevention Institute (CPI) & First Aid training required (exception: PHP program) * Medication Certification required. **JOB SUMMARY**
This job, as part of a physician team, ensures that utilization management responsibilities are performed in accordance with the highest and most current clinical standards. The incumbent reviews escalated cases electronically and using Medical Policy criteria sets to evaluate the medical necessity and appropriateness of the requested treatment of service. Depending on the nature of the case, telephonic peer to peer discussions may be required. The incumbent ensures compliance to NCQA, URAC, CMS, DOH, and DOL regulations at all times. In addition to utilization review, the incumbent participates as the physician member of the multidisciplinary team for case and disease management. They will advise the multidisciplinary team on cases, particularly high-risk cases, through the team structure. Additionally, the incumbent may be assigned special projects to help support and improve the care of our members
**ESSENTIAL RESPONSIBILITIES**
+ Conduct electronic review of escalated cases against medical policy criteria, which may include telephonic peer to peer discussions, to determine medical necessity and appropriateness. Complete initial determination of cases, review of appeals and grievances, and other reviews as assigned. Compose clear and concise rationales for member and provider determination notifications all while adhering to required compliance standards (NCQA, URAC, CMS, DOH, and DOL regulations, etc.). Ensure that all aspects of the medical management process are consistent with community standards of care.
+ Participate as a member of the CMDM multidisciplinary team. Attend huddles and grand rounds. Advise multidisciplinary team on cases that require physician expertise.
+ Participate in protocol and guidelines development to ensure consistency in the review process.
+ Actively manage projects and/or participate on project teams that require a physician subject matter expert.
+ Other duties as assigned.

St. Vincent's Medical Center in Bridgeport is a robust community teaching hospital with over 3,200 employees. With a 473-bed capacity and a 76-bed inpatient psychiatric facility in Westport, we offer comprehensive services that include a large multispecialty provider group and specialized care for all ages.
Job Summary: As a Clinical Services Leader, you will play a crucial role in managing patient care activities and overseeing assigned staff, ensuring the highest standards of care are maintained.
Key Responsibilities:
- Provide direction and support to unit staff, overseeing admissions and conducting patient assessments.
- Develop and evaluate nursing care plans while documenting patient progress.
- Facilitate the development and implementation of nursing initiatives and objectives.
- Serve as a liaison between the management, staff, and other areas, identifying and resolving care delivery issues.
- Offer hands-on nursing support and provide coaching, mentoring, and education to the nursing team.
- Drive improvement in patient quality and safety outcomes at the care delivery point.
- Maintain fiscal responsibility and effectively manage organizational resources.
- Collaborate with team members to develop performance improvement indicators based on established standards and best practices.
- Other duties as assigned.
In this role, you will adhere to the ANA Scope and Standards of Nursing Practice, the ANA Code of Ethics, and the ANA Social Policy Statement.
Qualifications:
Licensure / Certification / Registration:
- Current Registered Nurse licensure from the Connecticut Department of Public Health.
- Certification as a BLS Provider from the American Heart Association or American Red Cross, obtained within 60 days of hire.
Education: A Bachelor of Science in Nursing (BSN) is required.
