**Optum Pediatrics** has an immediate opening for a friendly, patient focused and detailed oriented **Patient Care Coordinator** to join our team. The **Patient Care Coordinator** is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Supports the teams in meeting financial, clinical and service goals.
**Working Hours:** Office is open 7 days a week **,** Monday through Friday between the hours of 8:00 am to 8:00 pm. Rotating Saturdays and Sundays between the hours of 9:00 am to 1:00 pm. A 40-hour work week to be determine by the practice manager upon hire.

**Primary Responsibilities:**
+ Obtain accurate and updated patient information, such as name, address, insurance information
+ Perform insurance verification on the date of service
+ Obtain patient signatures for required documents
+ Upload a valid Government issued state ID of patient and patient insurance cards
+ Accurate and complete occupational medicine registration workflow
+ File and maintain medical records
+ Performs telephonic responsibilities to include confirming and schedule appointments, relaying lab and imaging results to patients, discussing patient questions to providers
+ Answer telephone calls promptly and courteously
+ Collects co-pays or outstanding balances on the date of service
+ Perform referral documentation promptly
+ Performs certain follow-up services for patients in a prompt and courteous manner, such as scheduling follow-up appointments and obtaining copies lab results or specialist reports
+ Adhere to the standards identified via Sparq regarding Optum Employee Policies
+ Work cohesively with fellow employees to achieve specific team goals
+ Keep abreast on current medical requirements relevant to their position, which includes maintaining patient confidentiality and abiding by all HIPAA and OSHA requirements

+ Participate in planning, coordinating, and implementing care in conjunction with the multidisciplinary team in accordance with the Plan of Care

**Required Qualifications:**
+ High School Diploma/GED (or higher)
+ 1+ years of experience working in medical office position performing duties such as scheduling appointments, checking patients in/out, insurance verification, collecting co-pays, and maintaining medical records
+ Intermediate level of computer proficiency (multi-tasking through multiple applications including Microsoft Outlook, Excel, and Word)
+ Ability to travel 10 % of the time to cover other offices as needed
+ Ability to work a 40-hour week flexible schedule
+ Monday through Friday between the hours of 8:00 am to 8:00 pm.
+ Rotating Saturdays and Sundays between the hours of 9:00 am to 1:00 pm
**Preferred Qualifications:**

Job Responsibility
+ Assists with the unit's admission and discharge processes.
+ Establishes good interpersonal relationships with families/visitors and hospital staff.
+ Reports vital information to all levels of the healthcare team, as needed.

+ Maintains good communication with co-workers for proper team functioning.
+ Communicates pertinent patient observations to members of the health care team.
+ Performs direct patient care as assigned for designated group of patients.

+ Apprises the Registered Nurse and health care team of any observed significant changes in the patient's condition.

+ Maintains a safe, comfortable and therapeutic environment for patients and families in accordance with hospital standards.
+ Functions in an emergency situation as a team member with guidance from supervisory personnel.
+ Assists with restrained patients according to established policy.
+ Performs grade I and II decubitus care, as directed.
+ Performs activities of daily living appropriate to patient's condition.
+ Uses proper body mechanics, obtains help and/or uses appropriate equipment when lifting, moving or transferring patients from bed to chair, putting them on stretchers and assisting them into wheelchairs.
+ Performs delegated patient care procedures.
+ Obtains and labels specimens, and performs certain diagnostic tests consistent with established procedures, as directed.
+ Performs phlebotomy procedures.
+ Performs telemetry and electrocardiograms, as directed.
+ Uses clinical knowledge in the care and management of patients based on diagnoses and the unit assigned.
+ Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions.
Job Qualification
+ High School Diploma or equivalent required.
+ 1-3 years of relevant experience, required.

Job Summary:
The official job title is Lead Care Manager (LCM).
The Lead Care Manager (LCM) role involves developing personalized care plans, coordinating member services, and collaborating closely with members and families, as well as Primary Care Providers to ensure they receive necessary medical treatment and support. The LCM will consult with members to determine their needs, develop individualized action plans, and work with care teams to manage the member experience effectively. Providing emotional support, resolving administrative issues, and ensuring timely access to care are key aspects of the position. Compassion, healthcare knowledge, and exceptional customer service are essential qualities for assisting members in becoming self-sufficient in health. The LCM will work with a diverse population of members enrolled in the Enhanced Care Management program, which may involve one or multiple members from the population of focus section below.
Responsibilities:
• Interview members to assess medical and social determinant of healthcare gaps and provide education about their condition and medication, while developing individualized care plans.
• Respond to member inquiries and concerns, ensuring adherence to hospital and legal requirements.

• Individuals with Serious Mental Health and/ or substance use disorder needs: Adults who meet the eligibility criteria for participation in, or obtaining services through Specialist mental health services or the Drug Medi-Cal organized delivery system or Drug Medi-Cal program.

Education and Experience:

PRINCIPAL DUTIES AND RESPONSIBILITIES:
As a member of the nephrology healthcare team, you will provide safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse.
- Sets up, tests, and operates hemodialysis machines for patient treatments.
