Responsible to the Unit Director/Manager/designee for overall nursing practice. Provides professional nursing care to treat human responses to life-threatening problems, based on the American Association of Critical Care Nursing (AACN) Standards. This includes the responsibility for assessment of the patients' needs, development of a plan of care, implementation of nursing measures and evaluation of the effectiveness of the nursing care and other treatments to protect, maintain, and restore health for the pediatric through geriatric age population. The Critical Care nurse recognizes cardiac arrhythmias and life-threatening emergency situations and takes appropriate action. ESSENTIAL FUNCTIONS Assessment is ongoing for all critically ill patients to promote optimal outcome.Conducts a comprehensive health history and physical assessment.Prioritizes patient problems/needs according to the actual/potential threat. Develops and implements a plan of care based on ongoing assessments. A problem list is developed, prioritized and updated to meet critically ill patients' needs, which includes discharge planning and appropriate referrals. Manages life-threatening complications of critically ill patients per Mount Nittany Medical Center policies and procedures and based on AACN standards. Interventions are established to prevent life-threatening complications, maintain airway and ventilation, hemodynamic stability, and stable heart rhythm. Accepts responsibility and accountability for maintaining knowledge and skills in critical care practice.Maintains an awareness of professional issues.Updates knowledge and clinical skills on an ongoing basis.Remains current on concepts, trends, scientific advances, biomedical equipment and ethics relating to Critical Care Nursing by updating knowledge and clinical skills on ongoing basis.Ensures the minimum qualifications of position are met at all times including licensure, certification and registration.Completes mandatory education and participates in annual competency fair. Assists with temporary pacemaker insertion and ambulance transport as needed

Responsibilities

The Patient Care Manager -Hospice assists Executive Administrator/Executive Director/Director of Clinical Operations with all patient care activities to ensure the delivery of safe and efficient patient care which adheres to relevant standards of accepted nursing, regulatory, medical practice, and payer source criteria.

Responsible for compliance with hospice policies and procedures and with local, state, and federal regulatory requirements governing the practice of the assigned team.

Actively promotes and directs assigned team regarding quality of care and safety of patients and staff.

Engages in thorough problem resolution and complaint investigation.

Provides in-services to staff to promote the health and safety of the patient, to familiarize staff with regulatory issues and agency policy and procedures, and to address any identified educational needs.

Education and ExperienceLicense Requirements

* Current RN licensure in the state of practice and one year of clinical experience.
* Current CPR Certification.
* Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Job Duties
+ Responsible and accountable for performing a wide range of clerical duties and direct patient care activities, depending on the need of the unit. The performance of these tasks in an efficient and timely manner is essential to enhance the care team's ability to provide quality care. Meets the qualifications to provide care for patients in the specific age range in their assigned clinical area.
+ Provides patient care as directed by the licensed nurse, within the nursing assistant scope of practice, in meeting the multidisciplinary care plan established for each patient.
+ Identifies, observes and documents the physical needs of the patient according to established standards.
+ Reports changes in patient condition to licensed personnel in a timely manner.
+ Obtains and maintains the integrity of specimens in accordance with policy and procedure, according to physician order.
+ Provides for or assists patients with activities of daily living and fulfills designated and delegated responsibilities for ongoing continuum of care.
+ Provides one to one patient safety watch as assigned following established policies and procedures.
+ Assists in problem solving, trouble shooting and taking corrective measures in care and maintenance of equipment and supplies.
+ Responsible for actively participating in patient safety improvement and management program.
+ Serves as a preceptor and mentor for new staff and students in a positive manner.
+ Performs clerical duties for admissions, transfers, discharges, and deaths according to standards and facilitates patient throughput activities.
+ Transcribes and processes orders according to unit standards.
+ Releases and confirms ordered tests to appropriate departments.
+ Coordinates transportation of patients to multiple tests.
+ Communicates patient related information promptly to the appropriate care team member.
+ Assists in ordering supplies.
+ Communicates with food services to ensure appropriate patient diets.
+ Operates, problem solves, and takes corrective measures for clerical equipment.
+ Includes scheduling maintenance of computers, fax machines, phones, etc.
+ Participates in obtaining and communicating electronic information as pertinent to the nursing unit.
+ Staff have access to non-controlled medications storage and supply areas and can transport non-controlled medications as necessary to meet the needs of the clinic or unit.
+ Assists with transporting non-controlled medications to and from Pharmacy and within the unit as needed.
+ Assists with transporting blood products to and from Blood Bank as needed.
