Maxim Healthcare Services is seeking a Registered Nurse (RN) to assume responsibility and accountability for the application of the nursing process and the delivery of patient care. The Registered Nurse (RN) demonstrates the ability to make clinical judgments in an effective and efficient manner under the direction of the Director of Clinical Services.
Responsibilities
- Utilizes the nursing process to assess, plan, implement and evaluate patient care.
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
- Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
- Documents the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
- Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard.
- Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
- Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor including establishing priorities.
- Other Registered Nurse (RN) duties as assigned.
Requirements
- Current Registered Nurse (RN) License for the state in which the nurse practices.
- Current Health Certificate (per facility Registered Nurse (RN) contract or state regulation).
- Current PPD or Chest X-Ray.
- Current BLS card.
- One year prior Registered Nurse (RN) experience preferred.

Summary
We are looking for an experienced and compassionate Psychiatric Nurse Practitioner to provide high-quality mental health care across various settings, including Skilled Nursing Facilities (SNF), Long-Term Care (LTC), Assisted Living (AL), and Independent Living (IL). This role emphasizes work-life balance and offers a fulfilling opportunity to make a difference in the lives of adults and geriatrics in senior living communities.
As a Psychiatric Nurse Practitioner, you'll work closely with a multidisciplinary team to ensure continuity of care and tailored psychiatric support across the post-acute and long-term care continuum.
Essential Duties & Responsibilities
- Psychiatric Care and Rounding : Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
- Collaborative Care Coordination : Partner with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive, coordinated care.
- Patient Evaluation and Management : Assess and manage the mental health needs of residents, perform physical exams, and gather complete medical histories.
- Diagnostics and Therapeutics : Order and interpret lab and diagnostic tests, make preliminary diagnoses, and develop effective therapeutic care plans for mental and physical health.
- Medication and Treatment Management : Prescribe and administer medications and treatments; provide injections as necessary.
- Patient and Family Education : Educate patients and families on medications, care plans, and preventive mental health practices.
- Acute Care Collaboration : Work alongside physicians and facility teams to address acute psychiatric and medical needs promptly and effectively.
- Graduation from an accredited Nurse Practitioner program and ANCC certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
- Active DEA license.
- BLS and ACLS certification.

This role requires travel to and from one or more assigned facilities. POSITION REQUIREMENTS & COMPETENCIES:
* Graduate from an accredited registered nursing program
* Licensed as a RN in the state of practice
* At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing
* Home health experience preferred
* Ability to make home visits and provide nursing care in the home environment
* Ability to complete necessary documentation appropriately, legibly and timely
* Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
* Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation
