Shift Structure
- Wound rounds with partner facility wound care team
- In-person rounding shifts start in the morning in accordance with facility wound round schedule with flexibility within reason;
- Shifts are typically daily Monday-Friday
- Charting/documentation can be completed off-site;
- This is an in-person position
Essential Functions & Responsibilities
- Provider is responsible for rounding and managing the wound care of the population at the assigned SNF
- The provider is responsible for collaborating and rounding with wound care nursing staff to assess and treat patients residing at SNFs with wounds
- Provider will provide high quality, patient-centered, comprehensive wound care services to patients residing in SNFs
- Provider is responsible for timely documentation of all patient care/encounters
- The provider will be available for phone/secure messaging consultations with facility nursing staff
- Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy™
- PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems
Physical Requirements
- Must be able to move inside a facility frequently and briskly in order to see patients, meet with managers and facility staff, and attend to patient needs.
- Punctual in-person attendance (typically between 7 a.m. and 1 p.m. is required (for rounders).
- Continuous operation of app-based EMR/notes system either through a computer or on a mobile device (iPad, Phone etc.).
- Ability to ascend and descend stairs in a facility.
- Some ability to position oneself to obtain records, tools, medical equipment and other items that may be in locations that are below the knee or above the shoulder.
- Effectively communicate with patients, managers, and Facility staff.
- Ability to observe details in close range (within a few feet of the observer) in order to provide accurate and appropriate medical recommendations, treatment, and to provide timely updates to Managers regarding facility or patient concerns.
- Must be able to lift or move at least 25 pounds.
Requirements and Qualifications
- Candidate must hold a valid PA/NP license in the state in which they will practice
- Candidate must hold a valid NP/PA degree from an accredited institution
- Candidate must have an active DEA license (If new graduate, Theoria will assist with your application)
- General Post-Acute/SNF knowledge or experience preferred
- Candidate must be familiar with EMR systems and charting practices

Korzen Health seeks a full-time Wound Care Nurse Practitioner (NP) to lead the Wound Care Clinic program in the Central Michigan market area, in collaboration with Healogics and MyMichigan Medical Center.
For over 25 years, Healogics has been treating patients with chronic wounds from over 600 hospital-based wound care centers across the country. Currently, Healogics treats over 300,000 wound care patients annually and has healed nearly 4 million wounds. Healogics is the worlds leading expert in wound healing.
This position is based in our outpatient wound care clinic and is exclusively dedicated to procedural wound care. Our center operating hours are Monday through Friday, 8:30 AM to 4:30 PM, with no nights, on-call duties, or weekend work. All patients are seen by appointment only; walk-ins are not accepted. The clinic is integrated with MyMichigan Hospital, and the physician will be employed by Healogics.
This is a procedure-oriented environment primarily serving geriatric patients. The successful candidate must be Board Certified/Board Eligible in their core specialty and possess a strong procedural background. Recent experience with procedures such as suturing, incision and drainage (I&D), or other types of procedures is required, and procedural case logs will be requested as part of the credentialing process. Wound Care and Hyperbaric Oxygen (HBO) Therapy training are available. A full clinical team will be in place from the start date.
Scope of Practice Highlights
- Full Team support includes Case Manager, Hyperbaric Oxygen Therapy Nurses, Office Manager, and Front/Back End Support.
- We offer advanced wound-care treatments to promote healing, including: Cellulitis therapy, compression therapy, hyperbaric oxygen therapy, infection management and prevention, wound cleaning, and wound dressing.
- Teams will average approximately 18 to 22 patients per day, with a majority receiving wound care and less than 5% receiving Hyperbaric Oxygen Therapy.
- Standard work hours are Monday through Friday from 8:30 AM to 4:30 PM, totaling 40 hours per week. There are no night shifts, on-call responsibilities, or weekend hours.
- This is a 4 day a week job, 32 hours a week

NP degree
Must be Board-Certified
Clear and Active State Medical License and a DEA License

Float to various clinics during and after your training
You must have a flexible schedule and be able to work mornings, evenings, weekends, and holiday
What you can expect:
Build meaningful and long term relationships with patients and their family in an intimate outpatient setting.
Be a part of a team that supports and relies on each other in a positive environment.
Deliver care to patients who are often dealing with multiple co-morbidities which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge.
Oversee a group of patients with the support of PCTs to conduct patient observations, measuring stats and machine set up.
You will work with your head, heart and hands each day in a fast paced environment.

Requirements:
Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree
You might also have experience in the following that can be helpful but not required
Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience
Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN)
