- Collect the patient or responsible party's insurance information, if applicable; collect payments and log in the EHR
- Scanning all appropriate documents or alert the provider if they need to complete the scanning
- Escort patients to the exam room. Collect and document patient's chief complaint, medical and medication history in the EHR
- Obtain vital signs and patient history and enter the data in the EHR in designated clinics with completed competencies
- Continue to process the patients in the waiting following appropriate clinic flow
- Provide waiting patients/potential patients guidance on registration, wait time, services that may be rendered, and payment methods
- Answer phones, responds to questions, complete call backs (including lab reviews) and return calls as necessary
- Clean and organize the clinic space daily. Including but not limited to: taking out trash, logging and putting away supply orders, cleaning the floor under counters and around furniture, cleaning examination rooms after patient visits, and equipment disinfection
- Participate in and prepare for off-site events as needed
- Put together weekly order for supplies for provider's approval

+ Provide skilled nursing interventions in the treatment of the patient/clients illness, rehabilitative needs and preventative care. Utilize a holistic approach in the provision of problem specific therapeutic interventions, teaching and training activities in accordance with the plan of care.
+ Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
+ Confer with the RN Supervisor regarding needed changes in the Plan of Treatment. Accept verbal orders from physician were permitted by state law/regulations/Nurse Practice Act and communicate these orders to the RN Supervisor.
+ Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care.
+ Maintain contact with patients, physicians, clinical manager(s), other members of the healthcare team in a timely manner regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.

During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. About the Position:
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

What to Expect :
- Conduct Adult/Geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
- Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up
- Provide services in a variety of venues to include: Home Visits, Skilled Facility Visits

Type: PRN - Flexible - Nurse Practitioner
PAY: Earn from $2,000 up to $0,000 per month (Based on an average of 20-100 completed visits per month, and pay of 100 per completed assessment). Compensation range is based on availability and assessment completion.
Location: Home, SNF, and other community environments in Pine Mountain, Georgia
Hours : Days, Evenings, Nights, Weekends, Flexible Scheduling

Qualifications:What is Required:
- Master’s Degree OR commensurate experience and satisfactory completion of NP licensure
- Current RN and NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority
- Board certified by the AANP, ANCC or the AACCN in a Matrix approved specialty
- Current BLS, ACLS or CPR certification
- 1 year experience as a Nurse Practitioner preferred, new grads encouraged to apply.
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills with patient, clients, and colleagues
- Comfort and flexibility with frequent change
Travel Requirements:
- Travel required to meet patients where they live
- Ability to travel - Valid state driver license, able to drive a car, proof of adequate automobile insurance coverage for the state of residence

JOB SUMMARY :
As a Nurse Practitioner or Physician Assistant at Physicians Care, you will play a crucial role in providing exceptional medical care to patients of all ages with a wide variety of acute and chronic medical conditions. Working collaboratively with a team of healthcare professionals, you will diagnose, treat, and educate patients while ensuring the highest standard of care and service. Our full-time Providers work a rotating 36-40 hour/week schedule, consisting of 12-hour weekday shifts and every-other weekend. Our PRN (per diem) team members are scheduled on an as-needed basis.
KEY RESPONSIBILITIES:
The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive. Other duties may be assigned.
• Provide exemplary customer service to patients, visitors and coworkers.
• Provide convenient, friendly patient care to all who come through our door seeking health related services.
• Provide leadership, direction, and assistance during emergencies while maintaining a comforting and reassuring affect for patients.
• Working environment subject to varying fast paced and high stress conditions, including long and variable hours, changing locations, and continuous and frequent contact with others.
• Performs medical assessment, treatment, management and follow up of clinic patients including, but not limited to: primary care, urgent care, occupational health, DOT physicals and workers compensation.
• Performs medical procedures including, but not limited to: laceration repairs, nail removal, pap tests, injections, splinting and abscess incision and drainage.
• Establishes and monitors a medically appropriate level of care for clinic patients.
• Oversees, directs and supports on site laboratory, x-rays, injections, and collections for direct access services, if applicable.
• Maintains clinical core competency.
• Works with Center Leadership and other staff to ensure operations are consistent with medical policy and professional standards.
• Cooperates and participates in the development, implementation and revision of policies affecting medical practice, quality of care and patient satisfaction.
• Participates in and assists with in-service and continuing education programs for physicians, nurses and other personnel employed by or associated with Company.
