**Qualifications**
+ Graduate of a Baccalaureate Degree from an accredited school of nursing (National League of Nursing)

**Work Schedule:**
Monday - Friday, 9 hours between 7:00am and 5:00pm, including a one hour lunch
**Core Duties:** The Family Nurse Practitioner (FNP) works as an independent health care provider of a primary care team. The practitioner is self-directed and has a high level of independence. The FNP may have a defined enrolled population of adult and geriatric patients aged 18 years and older, uses the results of health assessment tools and data to assist patients in obtaining an appropriate level of care and resources. As a member of the Medical Treatment Facility (MTF) Care Team, coordinates patient care through a continuum facilitating the achievement of optimal outcomes in clinical, quality and cost-effectiveness arenas. Provides direct clinical care to patients requiring the intervention of a mid-level provider and providing patient education and medication guidance as needed to patients. Uses population health guidelines to ensure patients receive appropriate clinical preventive services. Reviews diagnosis and medical records for active-duty members to determine pre-deployment and post deployment fitness and health concerns. Evaluates and responds to telephone consultations/MHS Genesis In-Between encounters from patients and other staff members. Provides risk factor counseling and preventive services. Prioritizes identified risk factors and triages/refers patients to Specialty Services, Health and Wellness Center (HAWC), or other prevention programs as appropriate. Uses and develops protocols in conjunction with existing clinical guidelines. Develops tools, monitors metrics to evaluate and continuously improve the effectiveness of programs and the health of the defined population. Knows scope of practice for support staff and offers opportunities for their personal and professional development. Guides support staff in gaining breadth and depth of experience in order to enhance delivery of patient care.
+ Conduct medical assessments to determine deployment status in a private setting.
+ Conduct pre-deployment review of health record for any conditions that may impact deployment status.
+ Review the Electronic Health Record (EHR) to identify current prescribed medications and discuss current use of over-the-counter medications and supplements with the patient.
+ Verify all medications are compatible with Area of Responsibility (AOR) restrictions; initiate waivers as applicable and notify Public Health and Deployment Waiver Manager.
+ Provide health education and information and recommend medical referrals/follow-up for deployment-related health concerns as indicated and/or aligned with appropriate Clinic Practice Guidelines (CPGs).
+ A complete and legible copy of DRHAs 1-5 must be documented in the deployer's EHR.
+ Ensure Service Members have access to comprehensive counseling on the full range of methods for contraception, pregnancy, pregnancy prevention and menstrual suppression, as applicable and directed by DHA-PI and prescribe specific deployment medications as indicated by Public Health and deployment location.
+ Make deployment determination and indicate clearance status in the ASIMS DMC module. Update ASIMS DD Form 2766 IAW AFMAN 41-210.
+ Coordinate referrals with deployer's PCM via T-CON, as applicable. In most cases a referral to the Internal Behavioral Health Consultant (IBHC) in Primary Care is appropriate for an initial referral for mental health care (where the Behavioral Health Optimization Program (BHOP) is available).
+ Complete the Health-Care Provider portion of the medical waiver packages for deployment limiting conditions, as directed by BOMC standard work process.
+ Capture active component DRHA workload using the provider's Medical Expense and Performance Reporting System code.
**Performance Expectations:**
+ Provides comprehensive primary, preventative, and therapeutic health care service to patients and family members empaneled to the MTF; to included, but not limited to, the Flight and Operational Medicine Clinic, Occupational Health Clinic, Base Operational Medicine Clinic, Family Health Clinic, or other privileged Nurse Practitioner positions within the MTF.
+ Performs comprehensive physical assessments of patients to include baseline data such as past medical and surgical history, family history, family history, social history, and immunization history.
+ Performs complete assessments on patients of adult age. Performs pre-employment physicals, and sports physicals. Secures a health and development history from the patient or patient record findings and makes critical evaluations.
+ Performs pre-deployment and post-deployment assessments of active-duty personnel. Performs PHA/Reserve Component PHA exams, overseas clearances for dependents, and medical evaluation board summaries on military personnel in accordance with annual requirements.
+ Performs or requests special screening and development tests and laboratory tests and interprets the results. Orders, performs, collects and interpret laboratory tests to include but not limited to EKGs, wet preps, wet mounts, hema-occults. Orders and interprets the following tests performed by outside laboratories including but not limited to routine blood chemistry, hematology, microbiology, bacteriology, virology, toxicology, pathology, and cytology. Orders and interprets the findings of radiological studies, including but not limited to CT scans, MRIs, X-rays, VQ scans, nuclear medicine scans, and other tests that may be necessary for the diagnosis and treatment of a patient. Consultations will be requested on the interpretation of abnormal studies.
+ Knowledgeable of a variety of pharmacological agents used in patient treatment, the desired effects, side effects, and complications of their use as well as the accurate administration of the pharmacologic agent, including dosage calculations as required.
+ Establishes medical diagnosis for common short term or chronic stable health problems to include but not limited to the diagnosis of short-term health problems such as injuries and infections.
+ Independently manages chronic stable health problems including but are not limited to asthma, chronic obstructive pulmonary disease, hypertension, diabetes, arthritis, upper respiratory infections, sexually transmitted diseases, and dermatological or orthopedic conditions.
+ Prescribes and monitors pain management regimen for patients with chronic pain.
+ Performs procedures to include but not limited to biopsies such as shave biopsies, minor suturing, toenail removals, and joint aspirations if specified in the credential and privilege lists.
+ Possesses knowledge of patient education principles and behavioral readiness to change. Possesses knowledge of disease management programs and clinical practice guidelines (e.g. VA/DoD CPGs), and HEDIS Measurements.
+ Teaches patients and family member's ways to maintain or improve their health status. Uses patient education materials as appropriate.
+ Recognizes scope of practice/limitations, consults with a physician and or medical specialty services as needed, and refers patients to Disease Management, Case Management, Health and Wellness Center (HAWC) or other services as required.
+ Accesses electronic medical records or patient paper charts to track members at risk for disease. Incorporates current knowledge of disease and condition management into daily work activities to include evidence-based primary, secondary, and tertiary prevention.
+ Works with TRICARE/Humana Service Center to coordinate referrals with RMC as appropriate. Ensure referrals include the appropriate level of severity/urgency of the care needed. Maintain accurate knowledge of TRICARE benefits and capabilities in local area network.
+ Knowledgeable of the Medical Standards Directory (MSD) and how/when to refer patients to the Airmen Medical Readiness Optimization (AMRO) board.
+ Applying medical, fitness and profiling standards IAW AFIs 48-123, 36-2905 and 10-203 (including profiling and duty restrictions).
+ Performs Base Operational Medical Clinic assessments to include but not limited to: Deployment Risk Health Assessments (DRHA) IAW DHA policy, Separation History Physical Exams (SHPE), and Mental Health Assessments (MHA). Secures a health and development history from the patient or patient record findings and makes critical evaluations.
**Professional Qualities:**
+ Participates in professional development activities and maintains professional affiliations. Attends conferences and workshops related to duties.
+ Maintains current knowledge of practice standards, disease management, preventive screening standard/requirements, and current trends in population-based health management and facilitates compliance. Duties and responsibilities: Serves as faculty and administrator responsible for the Doctor of Nursing Practice Programs in the College of Nursing.

Administrative Duties (30%)
Responsible for all aspects of the DNP program, including, but not limited to supervising faculty, leading continuous quality improvement efforts, maintaining accreditation requirements, and developing/updating curricula and or new concentrations.
Serves as a liaison between the College of Nursing and other University departments as well as local, state, and national agencies and professional organizations.
Ensures program is compliant with guidelines/regulations of the University, regional accreditation (SACSCOC) standards, national accreditation (GOA and CCNE) standards, and state regulatory agencies (LSBN), and/or other agencies as required.
Explores and leads appropriate workforce-need-driven program expansion efforts that will add to the imminence of the college and university.
Provides visionary leadership to shape the strategic direction of the DNP program, ensuring alignment with the College of Nursing's vision, mission, goals, and strategic plan.
Ensures program tuition/fees/budgetary decisions are made in a timely manner that is beneficial to all stakeholders.

Works with the Senior Director of Nursing and the CONSAH Dean to develop program support funding.
Collaborates with nursing faculty to review and revise curricula and programs to ensure that offerings are relevant, rigorous, and meet the needs of the healthcare community.
Ensures sufficient numbers of qualified doctoral faculty to lead quality programs.
Evaluates all program faculty and staff in accordance with NSU's faculty evaluation policies and established faculty outcomes.
Role-models the professional behaviors expected of all DNP faculty.
Creates semester schedules and directing faculty workload assignments that are equitable and in accord with the College of Nursing's workload policy.
Creates a passionate, respectful, transparent workplace environment for all program faculty and staff.
Leads concentration coordinators in program development, recruitment, and retention activities
Encourages and supports scholarship and innovation in program faculty.
Teaching Duties (20%):

Scholarly Activities and Professional Development (20%):
Actively participates in University and college of nursing committees, including acting as chair of the DNP Program Curriculum Committee (DN P PCC)
Actively participates in scholarly activities and other creative activities, such as, scholarly research; writing books, book chapters, and/or articles; practice; podium or poster presentations.
Maintains appropriate expertise and competencies through continuing professional education and development, as well as through the pursuit of scholarly activities.
Participates in relevant state and national professional development activities to stay abreast of requirements and up-to-date knowledge required from DNP graduates.
Student Engagement: Advising and Recruiting (15%):
Creatively recruits students for all concentrations within the DNP Program.
Ensures the program provides quality advising and instruction in online, distance, and face-to-face settings.
Advises applicants interested in the DNP program and advises currently enrolled students.
Works with the alumni office to develop a doctorate graduate alumni base.
Service (10%):
Attends College of Nursing and/or University functions and committee meetings as assigned.
Representing the CON to internal and external stakeholders at college, university, state, regional and national events.
Represents the College of Nursing and University in a professional manner.
Participates in activities that promote the College of Nursing, including those that may occur in the evenings, out of town and on the weekend.
Other (5%):
Responsible for maintenance of records and files pertinent to the College of Nursing.
Maintains confidentiality in accordance with federal laws and program policy.

The Nurse Practitioner holds primary responsibility to conduct clinical assessment(s) of PH members, documents and codes encounters in the authorized electronic medical record system(s), in an effort to improve our plan member's quality of life and to facilitate optimal health outcomes through cost-effective healthcare. **Primary Responsibilities:**
+ Conduct assessments in a variety of settings, such as member homes and clinics, on health plan members with emphasis on accurate and appropriate documentation
+ Checking vitals, conducting a physical exam that includes monofilament test and foot exam (as appropriate)
+ Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
+ Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
+ Communicate findings in your assessment that will be used to inform the PCP of potential gaps in care
+ Educate members on topics such as disease process, medications, and compliance

The salary range for this role is $104,500 to $156,000 annually based on full-time employment. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement.
