As a LPN or RN at Whites Creek Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:
- Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
- Participate in admissions, discharges and transfers of residents as required.
- Assess resident's conditions, prepare, and administer medications per physician orders.
- Initiate, review, and update care plans.
- Report any changes in a resident's condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
- Perform other assigned duties given by clinical or administrative leadership.
- Maintain confidentiality of protected health information, including verbal, written and electronic communications.

EssentialDuties and Responsibilities
+ Apply the nursing process to assess, plan, implement, and evaluate care for adult patients with diverse medical and surgical diagnoses
+ Conduct thorough patient assessments, including vital signs, physical exams, and health histories
+ Administer medications, treatments, and IV therapies as prescribed
+ Monitor and document patient responses to interventions and changes in condition
+ Collaborate with healthcare providers to develop and adjust individualized care plans
+ Prepare patients for diagnostic tests, procedures, and surgeries
+ Provide preoperative and postoperative care, including wound care, pain management, and mobility support
+ Educate patients and families on disease processes, medications, discharge instructions, and self-care
+ Maintain accurate and timely documentation in the electronic health record (EHR)
+ Adhere to infection control, safety, and quality standards
+ Respond promptly to emergencies such as rapid deterioration, falls, or code situations
+ Support patient advocacy and uphold patient rights and confidentiality
MinimumRequirements
+ Current Registered Nurse License in the state of practice
+ Minimum one year of professional nursing experience; Med-Surg experience preferred

+ Current BLS certification (ACLS preferred)
+ TB screening and health clearance (as required by contract or regulation)
+ Compliance with all federal, state, and local regulations
