Seeking a compassionate and professional PRN Nurse (RN or LPN) to conduct home visits and provide high-quality, individualized care to clients in their residences. Key Responsibilities:
- Conduct in-home patient assessments, including health history, physical exams, and evaluations of the home environment
- Monitor patient conditions, vital signs, and report changes to the supervising nurse or agency
- Assess health status and develop individualized, comprehensive care plans tailored to client needs
- Document care provided and health status updates with accuracy and efficiency
- Educate patients and families on disease prevention, medication management, and overall health maintenance

- Current and valid RN or LPN license in the state of South Carolina
- Minimum of 1 year of clinical nursing experience (home care preferred)

- PRN (as-needed) availability required
- Home visits required ; each visit lasts approximately 30 minutes
- Minimal documentation required following each visit
