What You'll Do as a Hospice Aide:

Observe and report changes in the patient's physical or mental condition

Offer emotional and psychological support to patients and families

Maintain accurate and timely documentation of visits

Attend team meetings and participate in Quality Improvement (QI) activities

Practiceinfection control and follow safety protocols at all times
About You

We are looking for a wound care experienced Nurse Practitioner to join our medical group in Worcester MA. This position can be part time, with the potential for full time athrs per week depending on case load, locations covered, providing essential wound care services at assisted living facilities and skilled nursing facilities. Wound care certification or substantial wound care experience preferred, and we can help with wound care certification if needed. Rounds and schedule will vary depending on patient need and census volume but will be 1-2 days per week (8-hour days), locations are in Athol MA and Worcester (1 hr apart) with potential to expand in the future. Ideally the incoming NP can provide coverage at both locations.

Responsibilities:
• Deliver comprehensive, patient-centered wound care to residents of assisted living facilities, patients of skilled nursing facilities and home care.
• Perform comprehensive wound assessments and develop treatment plans.
• Implement wound care therapies, monitor patient's response to treatment, and adjust treatments as necessary.
• Communicate effectively with patients, families, and facility staff regarding treatment plans and progress.
• Document all treatments and care provided accurately and thoroughly in electronic medical records.
• Comply with all federal and state regulations regarding wound care treatment and patient privacy.
Requirements:
• Active unrestricted NP (Nurse Practitioner) license in MA
• Wound care certification or substantial wound care experience preferred. (we can aid with training/certification if needed)
• A minimum of 1 year of clinical experience post-graduation.
• Proficiency in electronic medical records systems.
• Excellent communication, patient care, and team collaboration skills.
• Ability to travel to different assisted living facilities and skilled nursing facilities
Schedule:
• Flexible schedule Part time (1-2 days) with potential to go full time in the future
Salary (range):
• $60- $75/hour depending on experience
