**Licensed Practical Nurse/Licensed Vocational Nurse**
**Part Time, Multiple Schedules Available**
This is a fantastic role to continue your nursing career path and work within a setting that provides variety and rewarding challenges. You will have high-impact work by providing nursing services, training, and supports to address the medical or psychiatric needs of individuals with a brain injury, intellectual and developmental disability, a substance use disorder, and/or has an emotional or behavioral disability.
+ Monitor the health conditions of individuals served and ensure quality services are provided.
+ Administer medications and provide documentation as well as oversee and monitor staff members who administer medications.
+ Provide prescribed medical treatment by physician's orders and personal care services.
+ Conduct assessments, implement health care services to be included in each Individual Service Plan (ISP), and respond to changes of condition.
+ Provide training to staff members including universal precautions, infection control, and medication administration.
+ Serve as liaison and advocate with other medical and professional service providers as well as work with external agencies to coordinate services and ensure consistent treatment of individuals receiving care.
+ Provide training for individuals served with topics including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation.
**_Qualifications:_**
+ Associate's Degree in Nursing or related field, or state certificate
+ One year of experience working with population served in a nursing capacity
+ Current state LPN or LVN licensure for state of residence
+ Current driver's license, car registration, and auto insurance
+ Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
+ Ability to effectively communicate with staff, individuals served, and families

**How you make a difference**
The Licensed Practical Nurse (LPN)plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education anddemonstratedcompetencies.
**Key Responsibilities**
+ Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed.
+ Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures.
+ Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines.
+ Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel.
+ Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care.

Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions.Documents medication regimen and updates medication profile according to established procedures.Treats patients and caregivers in the highest and most effective manner.Implement all available actions to prevent avoidable hospitalizations and ER visits. Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel.What is Required?Graduate of an accredited Certificate, Diploma, or Associate School of NursingCurrent State License as a Licensed Vocational / Practical Nurse1 year of experience in a clinical care settingExperience in a hospice or home health environment is preferred50% travel requiredSit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.Valid driver's license and insurance and reliable transportation to perform job tasksYou will report to the Clinical Team Manager.We value the unique skills of veterans and military spouses.
