+ The Nurse Practitioner (NP) will provide clinical support to our Street Medicine team. NP will provide direct medical care to unsheltered, homeless individuals with comorbid physical, mental health, and substance use disorders in a wide variety of settings including encampments and on the street.
+ The NP will work as part of a multidisciplinary team consisting of behavioral health, community health workers, medical assistants, and a nurse to develop a treatment plan and provide ongoing care to a panel of high-risk homeless patients.
+ Participates in strategic planning within overall needs in function of growth and clinical enterprise of Street Medicine.
+ Facilitates relationships with other practitioners and learners.
+ Provides ongoing education to staff, learners and community on health and homelessness.
+ Participates in the development of new protocols for street medicine along with the director of clinic operations and medical director.

+ This position is a grant-funded position and may be eliminated if funds are no longer available.
**Job Requirements**
**Minimum Qualifications**
+ Must possess and maintain a current Registered Nurse license AND certification as a Nurse Practitioner with the state of California.
+ Must possess and maintain a current American Heart Association Basic Life Support (BLS) certificate
+ Current/unrestricted DEA certification.
+ 2+ years of NP experience working in a medical office or hospital setting.
+ NP experience in assessing/managing those with substance use disorders, mental illness, and trauma care.
+ Experience with EMRs
+ Must work independently with little supervision
+ Ability to be culturally sensitive and work with a diverse patient population

The Nurse Practitioner(NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarceratedpatientsin accordance withestablished policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director ordesigneetomonitorthe provision of clinical services. **Key Responsibilities**

Education

+ Must have the patience and tact to deal with the inmatepatientpopulation and ability to work effectively in the corrections environment.
+ Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage.

POSITION SUMMARY
- The Hematology/Oncology Nurse Practitioner/Physician Assistant functions as an independent healthcare provider who, working collaboratively within a multidisciplinary health team, is responsible for providing comprehensive care to patients in a primarily outpatient setting.

- Demonstrate proficiency in procedural skills pertinent to the practice area.
- Provide care that is patient and family-centered, compassionate, appropriate, and effective for health promotion, illness prevention, and treatment of disease.
- Effectively counsel and educate patients and their families on disease processes, treatment plans, and symptom management.
