Base pay in range will be determined by applicant's skills and experience. Role is also eligible for team based bonus opportunities. Temporary associates are not benefits eligible but may participate in the company's 401(k) program.

At Geriatric Specialty Care of Reno, we're expanding our model for caring for seniors that provides high-quality, comprehensive in-home healthcare services to seniors all across Northern Nevada. From private residences and long-term care to assisted living facilities, our team of nurses, physicians, advanced practice providers, and care providers visit patients where they're most comfortable-creating a seamless, convenient, and personalized process for seniors and their caretakers.
The Role
The Provider is responsible for the diagnostic evaluation, management, and therapeutic health care of primary care patients of PPH residing in assisted living communities, residential care homes and private residences. This care includes-
* Obtain history and physical on new patients
* Order/interpret diagnostic tests
* Diagnose and treat chronic conditions and episodic illness
* Perform Advance Care Planning
* Accurate and timely documentation in the EHR
* Coordinates with other members of the care team, both internal and external partners
In addition to the above, the Provider is responsible for the annual comprehensive health assessments in accordance with health plan partners and Medicare guidelines. The assessments may be done in the home or clinic setting. This service includes:
* Conducts Health Assessment, including past medical history, review of systems, medication reconciliation, and physical exam
* Review and discuss the need to close care gaps and quality measures
* Perform/review standardized screening exams
* Identify all existing medical conditions
* Accurately and timely documentation of assessment and conditions to include quality MEAT Statements
* Identifies patients in need of chronic care management
* Identifies patients in need of pharmacy assistance
* Document Elements of Personal Prevention Plan and health issues to follow up with PCP
The Provider is also responsible for ensuring a safe and smooth transition of patients following an acute or skilled stay or ED visit. The care included is-
* In-home post-acute visit including history and physical and medication reconciliation
* Short-term management of episodic illness related to acute or skilled admission
* Care coordination with team members for the implementation of a safe discharge plan
* Identify barriers/obstacles to safe, self-management in the community and work with care team to overcome or identify alternatives
What We're Looking For
* Current Nevada APRN license, credentialed with Medicare, Medicaid and health plans PPH is actively contracted with (Senior Care Plus, Prominence, OPTUM,) or has the ability to be enrolled with any health plans.
* Minimum two years experience in specialty care area (Geriatrics) in private physician office, clinic setting or community-based program preferred.
* Demonstrates clinical competence including strong communications skills and medical knowledge.
* Remains current in licensure according to state regulation.
* Positively represents the practice by communicating to all visitors, patients and staff in an enthusiastic, friendly and professional manner.
* Provides a positive environment and experience with patients

ResponsibilitiesThe Palliative Care Nurse Practitioner/APRN provides direct patient and family care through palliative care consultation to ensure specialty services to patients with serious illness and a prognosis of one year or less who are not ready or eligible for hospice care.
- Demonstrates effective and ethical use of approved evidence-based palliative medicine practices in the delivery of all patient
care based on physical, emotional, spiritual, psycho/social, ethical, educational, safety and related criteria, appropriate to the ages of patients served. - Demonstrates knowledgeable clinical expertise by incorporating palliative care body of knowledge to formulate an effective plan of care.
- Gathers pertinent information systematically from all sources, including medical records, family history, and caregivers, and thorough specialty physical, emotional, psychosocial, and spiritual clinical patient assessment.
- Completes a comprehensive assessment utilizing the palliative care domains of assessment, interprets findings, conducts medication reconciliation, and formulates problem-related diagnoses, initiates plan of care utilizing therapeutic interventions, and evaluates the results of these actions for adequate outcome responses.
Education and Experience
Experience Requirements
- Minimum of five years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred.
License Requirements
- Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
- Current certification by a national body such as ANCC, NBCHPN, or AANP.
- Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation.
- Current CPR certification required.
License Desired
- Palliative Care ARNP preferred.
Skill Requirements
- Knowledgeable regarding the interdisciplinary team concept.

We are seeking a dedicated Hospice Nurse Practitioner to perform Face-to-Face (F2F) encounters essential for hospice eligibility certification and recertification. In this role, you will gather comprehensive clinical information, perform detailed assessments, and collaborate closely with hospice physicians and interdisciplinary teams to ensure quality patient care and regulatory compliance.
Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.
Salary Range – $99,000.00 - $110,000.00
Key Responsibilities:

Perform and complete physical assessments for hospice F2F encounters within CMS-established regulatory timeframes.

Document clinical findings accurately and attest F2F encounters in patient medical records.

Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements.

Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians.
-
Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families.
-
Provide clinical recommendations to the IDG based on F2F assessment findings.
-
Conduct assessments in the patient’s place of residence, including home, skilled nursing, or other settings.
-
Notify the hospice agency promptly if unable to complete F2F visits as scheduled.
-
May provide F2F coverage for multiple provider numbers with approval.
Qualifications – What You’ll Bring:

Master’s or higher degree in Nursing

Completion of an accredited graduate-level nurse practitioner education program

Current registered nurse license in state of practice

Current advanced practice nurse license or certification

National Nurse Practitioner certification (ANCC or AANP)

Collaborative agreement in place as required

Current CPR certification

Valid driver’s license and automobile insurance

Strong understanding of hospice care principles and terminally ill patient support systems

Excellent patient assessment and clinical evaluation skills

Familiarity with LCD criteria and hospice documentation requirements

Knowledge of Medicare, Medicaid, ACHC, and other regulatory guidelines

Strong oral and written communication skills

Compassionate and sensitive to end-of-life issues impacting patients and families

Ability to provide emotional support during times of crisis

Flexible and adaptable with ability to manage diverse tasks efficiently

Effective communicator across diverse socioeconomic and cultural backgrounds

Cooperative and tactful team member adhering to OSHA, state, and federal hospice regulations
