    1. Understands and practices universal precautions with all residents
    2. Performs all assigned tasks/procedures and reports to charge nurse tasks not completed
    3. Assists in the preparation for admission of residents
    4. Assists in and accompanies residents during admission, transfer and discharge procedures
    5. Provides postmortem care

    9. Takes and records temperature, pulse, respiration, weight, height, blood pressure and intake and output measurements
    10. Adheres to policies and procedures of the Department of Nursing
    11. Participates in socialization activities on the unit and assists residents to activities
    12. Turns and positions as ordered and/or as needed, making sure no rough surfaces are in direct contact with the body. Lifts and turns with proper and safe mechanics and with available resources
    13. Checks for reddened areas or skin breakdown and reports to an RN or LPN
    14. Ensures that residents are dressed properly and assists as necessary. Ensures that clothing is properly stored in dressers or on hangers. Ensures that all residents are clean and dry at all times
    15. Checks unit for adequate linen and supplies
    16. Restrains residents properly when ordered. Understands reason for restraint and suggests less restrictive measures if possible
    17. Accompanies residents to appointments as directed
    18. Provides reality orientation in daily care
    19. Prepares residents for medical staff ordered diet, removes food trays and assists with meals or feeds residents
    20. Distributes drinking water and other nourishment to residents
    21. Performs general care activities for residents in isolation, adhering to the center's Infection Control Policies
    22. Answers residents' call lights, anticipates residents' needs, and makes rounds
    23. Assists residents with handling and care of clothing and other personal property (including dentures, glasses, contact lenses, hearing aids and prosthetic devices)
    24. Collects and transports specimens as directed, following Infection Control P and P
    25. Reports (and records when appropriate) changes observed in condition or behavior of residents
    26. Knows the Nursing Care Plans and contributes when new information is available
    27. Assists in maintaining a safe and clean environment, reports equipment that is out of order, reports other problems to the charge nurse, maintains appropriate handling of clean and soiled linen and stores supplies on the unit
    28. Establishes and maintains interpersonal relationship with residents, family members, and other center personnel, while assuring confidentiality of resident information
    29. Practices careful, efficient and non-wasteful use of supplies and linen, and follows established charge procedures for resident charge items
    30. Ability and willingness to be assigned work on different units according to staffing needs
    31. Ability to work in settings where patients' needs, actions, and response to others are widely varied, where unpredictable reactions and/or emergencies occur, and where your immediate, professional, and compassionate action is required

Provides each of the assigned patients with routine daily nursing care and services in accordance with the patient’s assessment and care plan, and as directed by the nurse supervisor.

KEY RESPONSIBILITIES:

  3. Assist Residents in and out of bed and into wheelchair with the correct use of the transfer belt and practices proper body mechanics while moving/transferring patients

  4. Assists Residents in bathing: to include bed baths, tub baths, and showers

  5. Assists Residents in dressing or undressing, and personal grooming e.g., oral/denture care, brushing hair, trimming fingernails and toenails, skin care and shaving

  6. Assists Residents to and from activities as requested

  7. Assists Residents to dining area for meals; passes food trays to individual Residents in accordance with diet cards on tray and needs; documents intake of nourishment, and can identify the four basic food groups on a sample breakfast, lunch, and dinner Tray

  8. Assists with admissions, discharges, and transfers as requested; understands and utilizes care plans

  9. Cleans bedpans and empty catheter bags, provides external/indwelling catheter care, provides incontinent care as need for Residents.

  16. Removes bedding and replaces with clean linens per schedule or condition, and as needed

  17. Takes vital signs of patient, e.g., temperature, blood pressure, and respiration and records information and documents/reports information regarding the patient’s health, I&O, any change of medical condition, and care given

KNOWLEDGE, SKILLS, ABILITIES:

• Communicates well with patients/residents/clients and family members providing warm and friendly greeting and an approachable demeanor to families, visitors, patients/residents/clients

Job Summary

Assists professional nursing personnel in providing patient care in assigned area. Assists patients with activities of daily living, provides for personal care, emotional support and performs more complex clinical skills under the direction of professional nursing personnel.

Essential Functions

  • Consults with and keeps supervisor informed of unit activities, needs, and problems related to patient care.
  • Provides general nursing care such as positioning the patient, lifting, turning, applying/utilizing special equipment, assisting in the use of bed pan, urinal or commode, ambulating the patient, pre operative and post operative care, and application of binder or anti embolic stockings, bed bath, oral/hair/back care, changing bed linens, cleaning over bed table, and bedside stand, straightening patient room and other general care necessary during the shift.
  • Takes and records temperature, pulse, respiration, weight, height, blood pressure, and intake and output measurements accurately. Documents in a timely manner.
  • Prepares patient room and bed for admission and transfers and ensures all necessary equipment is in room. Maintains an attractive and comfortable environment for patients with special consideration to cleanliness of room, ventilation, and lighting.
  • Assists in patient admission, transfer, and discharge procedures. Assist with handling and care of patient belongings and other personal property.
  • Answers patient call lights, telephone, and paging system and responds as appropriate.
  • Under the direction of the Registered Nurse, performs routine treatment procedures such as enemas, application of appliances for heat and cold.
  • Able to release, remove, and reapply restraints to patient under the direction of an RN. Ensures patient is in proper position and alignment.
  • Assist Registered Nurse and/or LVN/LPN with complicated patient treatment procedures.
  • Serves and removes patient meal tray in timely manner. Assists with feeding and/or preparing items such as opening milk container, cutting food for patient.
Knowledge/Skills/Abilities/Expectations
  • Must read, write and speak fluent English.
  • Must have good and regular attendance.
  • Excellent oral and written communication and interpersonal skills
  • Basic computer knowledge.
  • Able to organize tasks, develop action plans, set priorities and function under stressful situations.
  • Ability to maintain a good working relationship both within the department and with other departments.

Qualifications

Education
  • Completion of course for nursing assistants.
Licenses/Certification
  • Current Nursing Assistant Certification
  • BLS Certification
Experience
  • Minimum six months’ experience in acute or long-term care facility.

Position is considered light CNA duties

Alternating weekly hours: 40/week, 32/week

POSITION SUMMARY

Personnel in this position function under the direct supervision of the Director of Nursing. He or she assists the clinical and nursing teams in patient care, helps patients by supporting daily living needs, and provides comfort, transportation, and vital sign monitoring.

QUALIFICATIONS

  • High School Diploma preferred
  • Good understanding of addictive process and recovery skills
  • Minimum of one-year experience in the addiction field preferred
  • Individuals in recovery from substance abuse must have a minimum of two years of continuous chemical abstinence and must be active in support groups

PERFORMANCE EXPECTATIONS

  1. Perform admissions and orientation of the patient to the unit. Assists with patient discharges
  2. Takes patient vital signs and documents according to facility policy
  3. Observes patient behaviors and documents in accordance with facility policy
  4. Conduct patient check of belongings upon admission
  5. Supervise urine collection of patients
  6. Conduct crisis intervention with the supervision of Director of Nursing/ADON or Charge Nurse
  7. Escort patients to appointments as indicated by supervisor. Accompanies patients on outings and to nightly meetings and may require driving facility vehicles
  8. Documents in chart any contact with patients in accordance with department and facility procedure
  9. Serves as a liaison between clinical staff and patients, keeping staff informed of high-risk issues or potential patient problems
  10. Regularly communicates with other team members and interface in a positive constructive and helpful manner to promote cohesiveness, reduce conflict and provide for resolution
  11. Monitors and identifies potential problems and discusses them in the daily multidisciplinary treatment meeting
  12. Participates in meetings and treatment teams as required
  13. Assists with Crisis Intervention as directed by Director of Nursing or Charge Nurse. Able to understand and react effectively to the unique needs of the patients
  14. Monitors meal times and bathroom breaks (ED Unit only)
  15. Follow through with special assignments and responsibilities as requested by the Director of Nursing
  16. Provides support for activities of daily living by assisting patients with activities they are unable to perform themselves
  17. Maintains patient stability by checking vital signs and weight, testing urine, and recording intake and output information
  18. Provides patient comfort by utilizing facility resources, transporting patients, answering patient questions and requests, and reporting observations of the patient to the nursing supervisor
  19. Documents actions by completing forms, reports, logs, and records
  20. Maintains work operations by following current policies and procedures
  21. Protects patient privacy and organization's value by keeping patient information confidential
  22. Adheres to professional standards, facility policies and procedures, federal/state/local requirements, and Joint Commission standards
  23. Updates job knowledge by participating in education opportunities, reading professional publications, participating in professional organizations, and maintaining certification and licensure
  24. Other duties as assigned
