+ Perform services for patients including bathing using basin-less bath products, oral hygiene, oral feeding and care of hair
+ Participate in hourly rounds
+ Transport patients to and from various treatment centers when necessary, and assist transporters in moving patients in and out of stretchers and wheelchairs

We are looking for a motivated, enthusiastic Nurse Practitioner or Physician Assistant to join our outpatient private practice part time in Elkhart, IN. Join our knowledgeable, well-established, and friendly multidisciplinary team and support staff! We are looking for a compassionate Nurse Practitioner or Physician Assistant, who enjoys helping others, is a great communicator, and a team player. The ideal candidate for this role has a passion for holistic and functional medicine with a focus in clinical procedures including hormone replacement therapy as well as a knowledge/background with IV infusions.

Duties:
• Patient exam/evaluations- focused exams (HPI, medical history, review pertinent medical records/imaging)
• Order and review appropriate tests- Evaluate clinical data
• Diagnosis and Treatment plan formulation
• Perform treatments to include Joint injection/trigger point injections (hips, knees, shoulders), IV Therapy and Hormone Therapy (will provide training)
• Patient education on diagnosis, treatment plan options and expectations, patient goals, and post care treatments
• Prescribing and following up with medications and peptides
• Documentation- accurate and timely
• General oversight of the nursing staff
• Collaboration with the multidisciplinary team for comprehensive care
Qualifications:
• Graduation from an Accredited Nurse Practitioner or Physician Assistant program
• Nurse Practitioner/Physician Assistant license in IN
• Experience in regen med, joint injections, hormone therapies and IV therapies
• Strong background or interest in Holistic and Functional Medicine (Preferred)
Schedule:
• Part time (2 days per week)
Salary (range):
• $725-$850/day (DOE) plus potential bonus compensation
