Responsibilities The Milwaukee Spinal Cord Injury/Disorders (SCI/D) Center is 1 of 25 VA SCI/D Centers across the nation. The mission of the VA SCI/D program is to support and maintain the health, independence, quality of life, and productivity of Veterans with SCI/D throughout their lives. We deliver care to approximately 400 Veterans. Our SCI/D Center is a 38-bed unit equipped with ventilator capability, sleep study management, environmental control units, and an apartment that is utilized to prepare a Veteran with new onset SCI/D for discharge back to the community. Inpatient services include acute rehabilitation, sustaining medical and surgical care, respite care, and end of life care. Outpatient services include SCI/D specialty care, primary care, preventative medicine, adaptive sports medicine, baclofen pump management, acupuncture, healing touch, wound management, home care, psychology and social work services, physical therapy, occupational therapy, recreation therapy, nutritional and pharmacy services, and telehealth. We authorize care in the community and monitor SCI/D Veterans who reside in long-term care facilities within a 100-mile radius from the Medical Center and make referrals if beyond. Major Duties: Providing direct patient care which includes Obtaining and documenting Vital Signs, Intake & Output, and other required data. Reporting of abnormal values or a change in the patient's condition. Following the plan of care by providing appropriate level of assistance with activities of daily living. At a minimum, this includes dressing and undressing, eating/feeding, toileting, bathing, showering, shaving, oral care, hair care, perineal care and restorative care: transferring, ambulating, positioning, range of motion and use of assistive devices. Performing patient procedures such as specimen collection, glucometer use, bladder scanner, use of mobility devices, and provision of continuous protective observation (CPO and 1:1 CPO) for suicidal ideations. Responding to patient/resident call lights or requests for assistance in a timely and courteous manner. We are seeking a qualified, experienced Nursing Assistant with an interest in hospice and a desire to provide dignified care to patients at the end of life. Aurora Zilber Family Hospice is an 18 bed Inpatient unit that provides intense symptom management for hospice patients in a beautiful, home-like setting. This is a Night shift position, 4 8-hour shifts per 2 week pay period from the hours of 11 PM to 7:30 AM. Occasional weekend rotation, every other holiday.
Pay Range
$20.40 - $0.60
Major Responsibilities:
* Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
* Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
* Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately.
* Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
* Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
* Completes various errands as directed including pick-up and delivery of trays, charts, specimens, and reports. Maintains and orders stock for nursing area. Cleans equipment and work areas.
* Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection.
* Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
* Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
* Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
* Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
