The clinic provides a broad scope of services including the diagnosis and treatment of workers compensation injuries and illnesses, urgent care, surveillance and regulatory exams, and preventative services. RBH offers adults, as well as child and adolescent patients in our communities, the ability to receive care at multiple level of services including: inpatient, residential, partial hospitalization, intensive outpatient and outpatient services. The three primary hospitals are located in Wisconsin, and numerous clinics spread from coast to coast in Wisconsin and 9 other states, including California, Colorado, Florida, Georgia, Illinois, Minnesota, Pennsylvania, Tennessee, and Washington. In this role, with the support of a collaborating physician, the provider will primarily serve as a hospitalist in our inpatient and residential departments, and working alongside our psychiatric providers in a multi-faceted treatment team model of providing care.
Services :
Attend Patients at the Site:
- AHP will be expected to attend patients and assist with patient coverage at the locations assigned by Employer, this may include multiple locations, multiple levels of care or providing virtual services to sites within the scope of AHP licensure.
- Conduct medication management sessions with patients,
- Conduct patient medical evaluations and assessments,
- Conduct history and physical
- Create patient treatment plans and manage updates,
- Perform patient and family education,
- Participate in team and unit meetings,
- Create discharge plan creation,
- Participating in the established call schedule.
- Take emergency call for medical care units
- Round on patients as assigned.
- Serve on Hospital committees as assigned

- Review of all denials of service from all payors that are deemed for medical reasons.
- Interface with referring parties, the psychiatrist, primary care Physicians or AHPs, to coordinate care, aftercare.
- Market Employer as appropriate, to include off-site meetings and marketing activities.
- Review potential admissions, pending cases, and declined admissions, as needed, to assure Employer policy execution and compliance with EMTALA.
- Serve an advocate for the organization.
- AHP will provide support, information, education and services
- Participate in Employer's quality assurance and performance improvement activities and meetings.
- Take an active role in identifying new program development opportunities for patient care services and Employer.
- Participate in community education of behavioral health issues.
Physical/Mental Demands :

Summary

**Required Qualifications:**
+ Certified Nurse Practitioner through a national board
+ For NPs: Graduate of an accredited master's degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), Adult-Gerontology Acute Care Nurse Practitioners (AG AC NP), Adult/Family or Gerontology Nurse Practitioners (ACNP), with preferred certification as ANP, FNP, or GNP
+ Active and unrestricted license in the state which you reside
+ Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
+ Ability to gain a collaborative practice agreement, if applicable in your state
+ Ability to complete physical requirements of the job including moving a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions
+ Availability to work 24 hours per month, with expectations that 16 of the 24 hours/month could be during off-hours (after 5 pm, on weekends, and/or holidays) not to exceed 960 hours in a calendar year
+ Driver's license and access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area

The registered nurse plans, organizes, directs, and coordinates services and is experienced in nursing, with emphasis on community health education & experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Patient Care
Develops and initiates the plan of care on admits to the agency. Assesses patient's condition, re evaluates, and updates as necessary. Initiates proper preventative and rehabilitative nursing procedures and provides services that are ordered by the physician as indicated in the plan of care. Provides skilled nursing care. Provides patient, caregiver, and family counseling. Provides patient and caregiver education. Prepares clinical notes. Communicates with the physician who is responsible for the plan of care and other health care practitioners (as appropriate) related to the current plan of care. Obtains necessary physician orders and collaborates with physician as patient needs dictate. 75% of all charting to be done in the patient's home. Re evaluates the need for continued care on an ongoing basis and completes recertification as necessary. All weekend visit documentation must be completed by that Monday at 8:00 AM. All visit documentation for non-weekend visits must be completed within 24 hours of the visit. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. Participates in other duties as required by the agency.
- Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.

Participating in the HHA's quality assessment and performance improvement program and HHA-sponsored in-service training. Participates in on-call duties as defined by the on-call policy. Participates in all mandatory meetings, training, and in-service education; Ensures that arrangements for equipment and other necessary items and services are available. Instructs, supervises and evaluates home health aide care provided every two (2) weeks. Supervises Licensed Practical Nurses (LPNs)/ Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)/Certified Nursing Assistants (CNAs), as directed.
