**Summary:**
Under the direction of a professional nurse, the Student Nursing Assistant (SNA) assists the patient with activities of daily living, transfer and mobility skills, skin, bowel and bladder programs, and maintains a clean, safe environment for the patient. **Responsibilities:**
- Properly applies safety devices.
- Assures that patients are set up and ready for meals and assists with feeding as appropriate.
- Performs accuchecks 30 minutes before meals or as directed by the RN and reports and documents blood sugar levels to the RN.
- Immediately inform RN of any change in patient's condition; call RRT if needed.
- Maintains a therapeutic relationship to provide optimal customer-focused care.
- Communicates effectively with patients at level appropriate to age and cognitive status, showing sensitivity to cultural diversity.
- Interacts effectively with peers and other hospital staff to foster positive working relationships.
- Computer documentation is accurate, timely and at point-of-care when possible.
- Notify charge nurse whenever leaving the unit.
- Assists patients with personal hygiene and activities of daily living promoting maximal independence.
- Accurately calculates and documents calorie count, intake and output and patients weights.
- Assists patients with transfers and mobility activities observing appropriate techniques, precautions and assistive devices.
- Assists patients with bladder and bowel elimination programs and collection of laboratory specimens.
- Enhance patient satisfaction by hourly rounding and prevention of call light needs. Improves response to call lights when they occur.
- Work collaboratively with other members of the health care team to insure high levels of patient care.
- Follows CDC guidelines for hand washing and isolation precautions.
- Needs to be on the floor at all times during shift report unless directed by the Charge RN.
- Works as a team with nurse to ensure that the patient feels safe and well cared during hospital stay.

Duties

VHA Nursing Assistants perform care as delegated by a Registered Nurse to patients in the Mental Health (PTSD) department. Nursing Assistants will assist medical personnel in the care and treatment of patients, including direct patient care. They perform a wide variety of duties aimed at increasing the comfort and mental health of the patient. They perform patient care and take measures to address the patient's bio-psychosocial needs, personal hygiene, physical comfort, nutrition, hydration, and safety. They will encourage patients with self-care and promote independence, while reporting to the Registered Nurse.

Duties Include but are not limited to:

  • Provides direct care for complex veterans and makes observations of subtle changes in resident's condition, i.e. behavior shifts, mood, skin coloring, appetite, etc. and takes action accordingly within the NA's assigned role.
  • Documents care given, observations and responses on appropriate tools or forms.
  • Responds appropriately to subtle patient changes by contacting the appropriate nursing personnel.
  • Monitor and documents patient intake and output reports deviations timely and accurately.
  • Performs nursing procedures within scope of practice and according to policies and standard procedures to care for the veteran for such procedures but not limited to: care simple dressing, application of braces and prosthetic devices.
  • Performing and documenting prescribed medical treatment and interventions for patients, such as diabetic urine testing and blood testing, simple dressing changes.
  • Works on small group activities with veterans/residents according to the patients/resident treatment plan.
  • Recognizes the residents' autonomy and promotes independence by means of respect, limit setting and individualized treatment.
  • Actively participates in Treatment Planning Meetings.
  • Utilizes equipment to promote the veteran self-care, such as; lift equipment, hydro sound tubs, and veteran/resident wandering system.
  • Verbally provides patient information to the treatment team for inclusion in the veteran/residents' plan of care, participates in treatment team meetings for resident care planning.
  • Enters information about the residents' condition in the chart, utilizing appropriate formats.
  • Participates in maintaining a clean, safe environment for the patient/resident.
  • Removes defective equipment, reports hazards, observes safety regulations, contributes to infection control, and utilizes proper aseptic technique in performance of duties.
  • Manages distributed patient behavior and by appropriately intervenes and request assistant from other health care members.
  • Appropriately initiates basic life support, takes actions appropriate action.
  • Other duties as assigned

Basic Requirements:

  • Experience, Education, or Training: Six months of general experience. GS-3:

None beyond the basic requirements.

GS-4:

One year of specialized experience as a nursing assistant or experience in another medical field (LPN, health technician, hospital corpsman, etc.) performing technical and nursing skills requiring knowledge of the human physical and emotional states, therapeutic communication, and technical skills required for basic and intermediate treatments;

OR

Education: Successful completion of 2 years above high school in an accredited institution with courses related to nursing care, health care or in a field of study appropriate to the specialization of the position, such as education in a program for psychology, psychiatric, or operating room technicians;

AND

In addition, the candidate must demonstrate the following Knowledge, Skills, and Abilities (KSAs):

1. Ability to provide a range of patient/resident care and contribute to the completion of the nurse care plan and recovery of patients/residents.

2. Ability to observe patients/residents or resident's physical or emotional status of refer deviations and/or problems to supervisor in a timely manner.

3. Ability to properly use equipment, materials and supplies in simple diagnostic and treatment procedures such as bladder scan, continuous passive motion device, blood glucose monitoring machine, specimen collection, etc.

4. Ability to communicate effectively with patients/residents, their families and other health professionals.

GS-5:

One year of progressively responsible assignments and experience equivalent to the GS-4 level which demonstrates knowledge, skills, and abilities that are directly related to the specific assignment. In addition, the candidate must demonstrate the professional KSAs below;

Education: Successful completion of a 4 year course of study above high school leading to a bachelor's degree that included 24 semester hours of courses related to health care or possession of a bachelor's degree;

3. Ability to recognize and react to emergent patient/resident care situations and intervene while waiting for assistance. For example, recognizing need for basic life support, controlling bleeding and assisting with behavior crisis, etc.

Provides basic, quality patient focused care in clinical setting under daily direction and supervision of Registered Nurse (RN) in accordance with the standards of practice within guidelines of corresponding state regulations. Performs delegated tasks in patient care setting that supports environment of patient care and assures patients and family are treated in a holistic manner with dignity and respect.

Major Responsibilities:

  • Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
  • Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
  • Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately.
  • Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
  • Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
  • Completes various errands as directed including pick-up and delivery of trays, charts, specimens, and reports. Maintains and orders stock for nursing area. Cleans equipment and work areas.
  • Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection.
  • Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
  • Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
  • Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.

Licensure, Registration, and/or Certification Required:

  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 45 days, and
  • Nurse Aide in good standing as indicated with the State Registry in which the team member practices.

Education Required:

  • Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED), or
  • High School Graduate.

Duties

VA Nursing Assistants (NA) perform care as delegated by a Registered Nurse (RN). Nursing Assistants will assist medical personnel in the care and treatment of patients, including direct patient care. They perform a wide variety of duties aimed at increasing the comfort and mental health of the patient. They perform patient care and take measures to address the patient's bio-psychosocial needs, personal hygiene, physical comfort, nutrition, hydration, and safety.   • Compiles necessary chart forms for admissions, transfers, discharges and expirations. Obtains appropriate copies of medical records for transfers to nursing homes and other health care facilities. Verifies medical records contain complete and accurate patient demographic information, patient identification and registration in the medical record, including appropriate level of care.
  • Establishes and maintains positive customer relations and contributes to a harmonious work environment. Quickly and courteously acknowledges requests, responds to call lights, demonstrates proper phone etiquette and fosters communication between the health care team.
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Position Requirements
+ Master's degree in business, informatics, nursing or other clinical specialty or related field required.
+ Eight (8) years of clinical healthcare experience required.
+ Four (4) years of progressive health care management experience is required.
+ Current state of Illinois nursing, nurse practitioner, or physician's assistant licensure
+ Demonstrated effectiveness in managing, directing departmental operations and management/supervisory personnel, evaluating, training and motivating performance. Ability to accomplish results through others. Demonstrated ability to delegate effectively and to establish clear guidelines for accountability. Ability to build consensus and provide strong leadership in a team environment, with the highest of professional and personal integrity.
