What You'll Do
- Leading and evolving a compliant, high-performing Care Management program aligned with regulatory standards.
- Coaching and mentoring a tenured team (including 3 Case Managers, 1 PRN, and a Clerk) with clarity, accountability, and professional development in mind.
- Serving as a trusted liaison across departments and with community partners to support safe transitions of care.
- Participating in LOS meetings and Patient Care Conferences to ensure efficient, patient-centered outcomes.
- Reinforcing standards and practices that support regulatory compliance and consistent performance.
- RN and Bachelor's required; Master's preferred
- 3+ years of leadership in Case/Care Management with Utilization Review experience (hospital experience required)
- Case Management Certification strongly preferred
