As a Nurse Practitioner, you will play a key role in delivering comprehensive healthcare services to patients via telehealth. You will assess, diagnose, and manage a diverse range of medical conditions while collaborating with other healthcare providers to ensure optimal patient outcomes. This position requires excellent communication skills, clinical expertise, and a passion for patient-centered care.
Key Responsibilities:
- Conduct virtual patient assessments and consultations via video or phone calls.
- Diagnose and treat acute and chronic health conditions.
- Develop and implement individualized care plans based on patient needs and preferences.
- Prescribe medications and order diagnostic tests as needed.
- Educate patients about their health conditions and treatment options.
- Collaborate with multidisciplinary teams to coordinate care and follow-up for patients.
- Maintain accurate and timely documentation of patient encounters in electronic health records (EHR).
- Stay updated on best practices, telehealth regulations, and advancements in nursing and healthcare.
- Participate in quality improvement initiatives to enhance patient care and satisfaction.
Qualifications:
- Current Nurse Practitioner (NP) certification and state licensure.
- Minimum of 1years of clinical experience, preferably in a primary care or urgent care setting.
- Strong knowledge of telehealth technology and virtual care best practices.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and manage a flexible schedule.
- Familiarity with electronic health record systems and telemedicine platforms.
- BLS and ACLS certifications preferred.
