PRINCIPAL DUTIES AND RESPONSIBILITIES:
As a member of the nephrology healthcare team, you will provide safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse.
• Sets up, tests, and operates hemodialysis machines for patient treatments.
• Obtains and documents necessary pre- and post-treatment vital signs and weight for each patient.
• Evaluates vascular access pre-treatment and performs vascular access cannulation.
• Evaluates intradialytic problems and provides intervention as prescribed by physician order or as directed.
• Monitors patients’ response to dialysis therapy.
• Evaluates patient prior to termination of venous access and discontinues dialysis treatment according to established procedures.
• Reports any significant information and/or change in patient condition directly to supervisor.
• Enters all treatment data into the designated clinical application in an accurate and timely manner.
• Collects, labels, appropriately prepares, and stores lab samples according to required laboratory specifications.
• Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
• Assists other healthcare members in providing patient education.
• Prepares, organizes, and efficiently uses supplies and equipment to prevent waste.
EDUCATION AND LICENSES :
• High school diploma or G.E.D. required.
EXPERIENCE AND REQUIRED SKILLS :
• Qualities and traits: dependable, compassionate, caring, supportive, collaborative, reliable.
• Previous patient care experience in a hospital setting or a related facility (preferred but not required).
• Continued employment is dependent on successful completion of the Fresenius Medical Care dialysis training program and successful completion of CPR certification.

The employee may occasionally be required to push and/or pull equipment, exerting up to 15 pounds of force. The employee may be required to lift and to lower solutions on a frequent basis of up to 30 lbs., and on an occasional basis lift up to 40 lbs., as high as 5 feet. There is a 2-person assist program and "material assist” devices for the heavier items.
The rate of pay for this position will depend on the successful candidate’s work location and qualifications, including relevant education, work experience, skills, and competencies.

6. Takes incoming calls from patients, caregivers, and facilitates professional, compassionate communication with staff, patient, and family. Responds to questions in timely manner, including use of tiger text when appropriate.
7. Acts as a liaison between mortuaries and hospice physicians to ensure death certificates are completed in an accurate and timely manner.
8. Requests Health & Physical (H&P) and inpatient records from hospitals and Skilled Nursing Facilities (SNFs).
9. Add and update mortuary, friends, and family resources in EHR as requested by IDG.
10. Work with hospice physicians to resolve and clarify codes and diagnoses with conflicting, missing, or unclear information, runs note audit reports.
11. Prepares materials for and coordinates set up for IDG meetings including the Part D Covered/Uncovered Query for MD review during meeting.
12. Generates, scans, and faxes Patient Status Reports to community physicians.
13. Generates staff bereaved lists and processes sympathy cards.
14. Performs clerical duties to support clinical teams, such as faxing, scanning, printing and copying.
15. Provides back-up support to other PCC staff as needed.

6. Possess excellent customer service skills.

Job Description
This position is responsible for the oversight and direct supervision of Medical Assistants. This position is responsible for ensuring that staff is efficient, professional and providing exceptional patient service. Ensures daily functions are properly staffed to accommodate patient flow and that operations run smoothly throughout the day. The position is responsible to ensure policies and procedures for areas of responsibility are adhered to, monitors performance of staff, and resolves patient care and departmental issues. Duties and assignments may vary at the discretion of management.
**_Responsibilities_**
+ Manage, Implement and maintain workflow processes of staff and patients in the clinic to improve patient experience.
+ Ensures that staff consistently interact with patients, clients, and staff in a manner that reflects favorably on the organization and promotes teamwork.
+ Maintains policies and procedures for areas of responsibility, ensuring compliance and consistent application.
+ Effectively supervises all medical assistant staff members. Monitors day-to-day staffing requirements for medical assistants and adjusts staffing as needed. Addresses fluctuating workloads based upon patient flow and physician schedules.
+ Assist with hiring process for potential candidates for assigned area, and training of new staff.
+ Provide Medical Assistant coverage when needed.
+ Travel to all cCARE Southern sites, including Victorville and Chula Vista.
**_Qualifications_**
+ Medical Assistant certification required
+ Minimum of 4 years of Medical Assisting experience required
+ Minimum of 2 years of Management experience required
+ Current CPR required
+ Ability to travel to each cCARE southern sites

+ Coordinates and supervises the daily activities of operations or business staff
+ Administers and exercises policies and procedures
+ Ensures employees operate within guidelines
**Anticipated salary range:** $75,600 - $113,295
