PRINCIPAL DUTIES AND RESPONSIBILITIES:
As a member of the nephrology healthcare team, you will provide safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse. 

•    Sets up, tests, and operates hemodialysis machines for patient treatments.
•    Obtains and documents necessary pre- and post-treatment vital signs and weight for each patient.
•    Evaluates vascular access pre-treatment and performs vascular access cannulation.
•    Evaluates intradialytic problems and provides intervention as prescribed by physician order or as directed.
•    Monitors patients’ response to dialysis therapy.
•    Evaluates patient prior to termination of venous access and discontinues dialysis treatment according to established procedures.
•    Reports any significant information and/or change in patient condition directly to supervisor.
•    Enters all treatment data into the designated clinical application in an accurate and timely manner.
•    Collects, labels, appropriately prepares, and stores lab samples according to required laboratory specifications.
•    Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
•    Assists other healthcare members in providing patient education.
•    Prepares, organizes, and efficiently uses supplies and equipment to prevent waste.
 

EDUCATION AND LICENSES :
•    High school diploma or G.E.D. required.

EXPERIENCE AND REQUIRED SKILLS :
•    Qualities and traits: dependable, compassionate, caring, supportive, collaborative, reliable.
•    Previous patient care experience in a hospital setting or a related facility (preferred but not required).
•    Continued employment is dependent on successful completion of the Fresenius Medical Care dialysis training program and successful completion of CPR certification.

The employee may occasionally be required to push and/or pull equipment, exerting up to 15 pounds of force. There is a 2-person assist program and "material assist” devices for the heavier items.

The rate of pay for this position will depend on the successful candidate’s work location and qualifications, including relevant education, work experience, skills, and competencies.

Responsibilities

1. Generates and monitors staff data integrity reports for staff compliance, including Patient Task Report, Unverified Services Reports and other reports as requested.

2. Generates, processes and tracks all physician recertification's and Do Not Resuscitate (DNR) / Physician Orders for Life-Sustaining Treatment (POLST) to ensure timeliness and accuracy.

3. Works in collaboration with the Medical records staff to ensure all orders printed by Medical Records are presented to Provider at IDG meeting for completion as needed.

4. Compiles, updates and maintains components of the clinical record from admission to discharge including changes in patient status, level of care, location, mortuary, CDAP and family and friends resource if requested by qualified field staff or clinical leader.

5. Prepares correspondence and reports for team and other requests such as therapy referrals and packets for NP/MD visits.

6. Takes incoming calls from patients, caregivers, and facilitates professional, compassionate communication with staff, patient, and family. Responds to questions in timely manner, including use of tiger text when appropriate.

7. Acts as a liaison between mortuaries and hospice physicians to ensure death certificates are completed in an accurate and timely manner.

8. Requests Health & Physical (H&P) and inpatient records from hospitals and Skilled Nursing Facilities (SNFs).

9. Add and update mortuary, friends, and family resources in EHR as requested by IDG.

10. Work with hospice physicians to resolve and clarify codes and diagnoses with conflicting, missing, or unclear information, runs note audit reports.

11. Prepares materials for and coordinates set up for IDG meetings including the Part D Covered/Uncovered Query for MD review during meeting.

12. Generates, scans, and faxes Patient Status Reports to community physicians.

13. Generates staff bereaved lists and processes sympathy cards.

14. Performs clerical duties to support clinical teams, such as faxing, scanning, printing and copying.

15. Provides back-up support to other PCC staff as needed.

16. Monitors and tracks clinical staff Unscheduled Paid Time Off (USPTO) in Excel Staff Schedule.

17. Performs special projects or other assignments as requested by Clinical Manager.

18. Reviews monthly reports on medical supply use and ensures the medical supply closet is stocked.

Qualifications

1. Current California LVN (Licensed Vocational Nurse) Licensure.

2. High School Diploma.

3. Working knowledge of medical terminology required.

4. Working knowledge of HIPAA regulations.

5. Demonstrates proficiency in typing skills and use of Microsoft Office programs; ability to learn new software applications.

6. Possess excellent customer service skills.

7. Demonstrates ability to communicate effectively with staff, patients, physicians and community agencies, both verbally and written.

8. Ability to manage time efficiently.

**_What Practice Management contributes to Cardinal Health_**
This position is responsible for the oversight and direct supervision of Medical Assistants. This position is responsible for ensuring that staff is efficient, professional and providing exceptional patient service. Ensures daily functions are properly staffed to accommodate patient flow and that operations run smoothly throughout the day. The position is responsible to ensure policies and procedures for areas of responsibility are adhered to, monitors performance of staff, and resolves patient care and departmental issues. Duties and assignments may vary at the discretion of management.
**_Responsibilities_**
+ Manage, Implement and maintain workflow processes of staff and patients in the clinic to improve patient experience.
+ Ensures that staff consistently interact with patients, clients, and staff in a manner that reflects favorably on the organization and promotes teamwork.
+ Maintains policies and procedures for areas of responsibility, ensuring compliance and consistent application.
+ Effectively supervises all medical assistant staff members. Monitors day-to-day staffing requirements for medical assistants and adjusts staffing as needed. Addresses fluctuating workloads based upon patient flow and physician schedules.
+ Assist with hiring process for potential candidates for assigned area, and training of new staff.
+ Provide Medical Assistant coverage when needed.
+ Travel to all cCARE Southern sites, including Victorville and Chula Vista.
**_Qualifications_**
+ Medical Assistant certification required
+ Minimum of 4 years of Medical Assisting experience required
+ Minimum of 2 years of Management experience required
+ Current CPR required
+ Ability to travel to each cCARE southern sites
**_What is expected of you and others at this level_**
+ Coordinates and supervises the daily activities of operations or business staff
+ Administers and exercises policies and procedures
+ Ensures employees operate within guidelines
**Anticipated salary range:** $75,600 - $113,295
