PURPOSE AND SCOPE:
Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with organization policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP).
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Patient Related
Education:
Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
Welcome assigned patients and inquire as to their wellbeing since their last treatment.
Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.
Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
Monitor patients’ response to dialysis therapy.
Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure
Obtain Hemostasis and apply appropriate dressings.
Evaluate the patient prior to discharge.
Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
Responsible for calculating and entry of individual patients’ dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.
Perform and document any intervention for unusual patient status and document patients' response to intervention.
Evaluate patients’ vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.
Obtain all prescribed laboratory testing and prepare specimens for collection.
Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.
Monitors patients performing self-care under the supervision of RN.
Staff Related
Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.
Education/Communication:
Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices
Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
Initiate Solution Delivery System (SDS) system.
RECORD KEEPING:
Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
Enters all treatment data into the designated clinical application in an accurate and timely manner.
Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure
omitted entries are completed or corrected by appropriate staff.
Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
Collect, label, appropriately prepare and store lab samples according required laboratory specifications.
Ensure collection of lab specimens by appropriate lab courier.
INFECTION CONTROL:
Assists in collecting information for infection control audits.
Supports staff and patient Adherence to infection control practices.
Follows infection control Policies and Procedures
Participates in and reinforces infection control education of patients and families under the direction of the CM or designee as allowed by state law.
TRANSITIONAL CARE UNIT:
Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care
Reinforces all education and care related matters as it relates to the new patient as allowed by state law
Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.
Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.
Sets and preforms a treatment on all machines used in the transitional care unit.
Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.
Assists with coordination of transition to patient’s modality choice.
Assists with supply ordering and inventory for TCU

This position requires frequent, prolonged periods of standing and the employee must be able to bend over. The employee may occasionally be required to push and/or pull equipment, exerting up to 15 pounds of force. The employee may be required to lift and to lower solutions on a frequent basis of up to 30 lbs., and on an occasional lift basis up to 40 lbs., as high as 5 feet. There is a two-person assist program and "material assist” devices for the heavier items.
EDUCATION AND LICENSES:
High School diploma or G.E.D. required.
Must meet Center for Medicaid/Medicare Services (CMS)-approved state and/or national certification requirements within the required state or CMS timeline.
All appropriate state licensure, education, and training (if any) required.

Job Description:
The Patient Care Technician assists with the conduct of clinical trials through overseeing the care of clinical trial subjects while housed on unit. Assist with the collection of biological samples, perform venipuncture, electrocardiograms, tube labeling, sample processing, documenting in subject charts, and other related tasks in accordance with the doctor’s written orders. In executing these responsibilities, the Patient Care Technician is guided by Good Clinical Practices (GCP), International Conference on Harmonization (ICH) guidelines, Health Insurance Portability and Accountability Act (HIPAA) and company standard operating procedures and policies.
Responsibilities:
- Perform regular safety and status checks on clinical trial subjects housed on unit as determined by physician orders.
- Perform appropriate research protocol procedures as delegated which may include but are not limited to vital signs, blood collection and processing, ECGs, and point of care laboratory tests.
- Perform basic lab procedures as delegated including but not limited to preparation of specimen tubes and labels, blood specimen collection, centrifuge operation, storing and shipping of lab specimens, accountability of specimens and notification of courier for specimen pick-ups.
- Ensure that biological specimens are disposed of appropriately.
- Maintain a clean, safe work environment for both fellow employees and clinical trial subjects.
- Engage the clinical trial subjects in group activities or one-on-one activities while housed on unit.
- Assure subjects have linens, toiletry items, and other sundries needed while the subject is housed on the unit.
- Serve meals and provide snacks to clinical trial subjects while subjects are housed on unit.
- Study, learn and comply with ERG site standard operating procedures, and other policies, practices and regulations where applicable.

Qualifications- Meets the training requirements of the State
- Completed a minimum of 75 hours of classroom and supervised practical training 16 hours of classroom must be completed prior to a mandatory 16 hours of practical training
- Minimum of six months experience including providing personal care
- At least 18 years of age
- Ability to read and follow written instructions and document care given
- Understands hospice philosophy, comfortable providing care to the terminally ill
- Satisfactory references from previous employers
- Ability to work with little direct supervision
- Secure with issues of death/dying
- Provides a calm manner when in a patient's home
- Possess and maintains current CPR certification
- Demonstrates tact, patience, and good personal hygiene
- Successfully completes a probationary period
- This probationary period is to ensure the aide is competent in the above qualifications and is documented by the supervisor and the employee
- Strong oral and written communication skills
- Good organizational skills

**As a Hospice Aide/ Certified Nursing Assistant, you will:**
+ Report directly to the Administrator, Executive Director or Patient Care Manager Senior and function under the direction, instruction and supervision of the Registered Nurse and/or Clinical Supervisor.
+ Help the patient to maintain good personal hygiene.
+ Assist in maintaining a safe and healthy environment.
+ Prepare and serve light meals.
+ Assist in feeding patient when needed.
+ Assist the patient in ambulation.
+ Assist with the safe use of a walker or wheelchair as taught and supervised by the nurse.
+ Assist with light housekeeping tasks such as emptying trash and changing linens.
+ Assist with certain treatments as ordered on the aide plan of care and as allowable in state-specific regulations.
+ Attempt to promote patient's mental alertness through involvement in activities of interest.
+ Give basic emotional and psychological support to the patient and other members of the household.
+ Report any change in the patient's mental or physical condition or home situation to the Registered Nurse and members of the interdisciplinary team.
+ Demonstrate basic care to the primary caregiver.
+ Practice infection control measures and standard precautions.
+ Provide accurate documentation with visit itineraries
+ Attend staff meetings.
+ Participate in at least one Quality Improvement activity yearly, as assigned by supervisor.
+ Complete documentation in a timely manner as outlined in orientation.
**About You**
**Hospice Aide/ CNA Requirements:**
+ Hospice heart: compassionate, kind and empathetic to your patients and their families.
+ Comfortable providing care wherever the patient calls "home".
+ Demonstrate flexibility and composure while supporting others in a time of crisis.
+ Certified Nursing Assistant (CNA) license.
+ Six (6) months of patient care experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
+ Additional, preferred experience to help you be successful in a hospice role: hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, nursing home or travel.
+ Current driver's license and automobile insurance.
+ Current CPR certification
