We're looking for someone to join our team as a Case Manager LVN ECM who:

Will co-manage and collaborate on patient cases with Care Coordinators (CCs) and Outreach Workers to ensure that patients receive high-quality care, understand their responsibilities as participants (Pediatric/Adult) and that appropriate orders are reviewed and completed in the Electronic Medical Record (EMR).

Essential Functions:
  • Perform patient care functions as prescribed by provider following established protocols, policies, and procedures within their scope of education, training and responsibilities.
  • Assist medical personnel with special procedures, minor surgical procedures and/or diagnostic exams.
  • Assess and monitor patients' conditions and notify provider for disposition.
  • Prepare and administer medications as directed by provider order or standing orders, following established protocols.
  • If, State certified, initiate and maintain Intravenous solutions as prescribed by provider, following established protocols.
  • Maintain equipment and supplies. Identify and arrange for equipment needing repair.
  • Apply principles of aseptic technique and infection control.
  • Monitor patient flow and assign/direct other personnel as needed.
  • Provide medical information and education to patients, following established protocols and policies.
  • Document pertinent patient information, assessment and nursing procedures, following established protocols. Maintain patient confidentiality.
  • Documentation must be dated, legible and with the appropriate signature to all entries (first initial, last name and title). All pre-printed prompts are to be answered.
  • On a temporary basis, may be required to work at any satellite facility.
  • Demonstrates clinical knowledge and skill in the care of the newborn, infant, toddler, child, adolescent, adult and geriatric patients ranging from 0-100+ years of age.
  • Facilitate the practice of the assigned providers.
  • Perform routine clerical functions as assigned (making appointments, telephone calls, etc.

You'll be successful with the following qualifications:
  • Graduation from an accredited School of Vocational Nursing.
  • Current California Vocational Nurse license required.
  • Current I.V. certification preferred.
  • Current Basic Life Support card required.
  • Must adhere to Clinica Sierra Vista's employee health/immunization requirements or provide a valid exemption request for subsequent approval.

Position: Patient Care Technician - Temporary - Shift

Compensation: The estimated pay for this position is $21.00 to $25.8274. This reflects only a portion of the total compensation package for this position. Additional compensation may be available for this role through differentials, incentives, and bonuses.

Definition: Under direction, provides a variety of direct and indirect patient care in an in-patient setting.

Distinguishing Characteristics: Incumbents follow clearly established procedures to assist licensed, professional nursing and medical staff in an in-patient setting; including performing general clerical functions and caring for patient's personal needs, safety, and comfort.

Essential Functions:

  • Assists with patient observation and care; checks and records vital signs and pain levels; collects specimens; measures and records intake and output of food and fluids; and records results.
  • Passes and observes food trays; assists or feeds patients.
  • Assists patients with personal hygiene and grooming.
  • Answers call signals; makes beds; cleans and cares for equipment.
  • Adheres to isolation precautions/practices, medical asepsis and infection control; observes oxygen precautions.
  • Transports patients as needed for admission, discharge, and procedures; transports specimens to the lab.
  • Performs general clerical duties; operates a variety of office equipment.
  • Assists with admitting, transferring and discharging patients by inputting data into the Electronic Medical Record, schedules appointments and follows up on authorizations.
  • Answers telephones and provides basic information to patients and/or other interested parties.
  • Coordinates the delivery of equipment and/or other services for patients following discharge.

Other Functions: Performs other job related duties as required.

Employment Standards: Successful completion of a State of California Department of Health Services approved Nursing Assistant training program OR one (1) year of an education program leading to an Associate's Degree or Baccalaureate degree in Nursing, OR two (2) years of experience as a Nursing Attendant in an acute care facility or in a skilled nursing facility for psychiatric patients.

Additional Requirements:

  • Appointees not possessing the American Heart Association Provider Basic Life Support (BLS) card at time of hire must successfully complete appropriate training and qualify for the RQI Provider certification within 60 days of employment. As a continued condition of employment, employee must maintain RQI Provider certification and competency.
  • Employees must maintain all health requirements designated by Kern Medical Center.
  • Depending on assignment incumbents may be required to possess and maintain specific certificates of competency as a condition of employment.
  • Some assignments may require: Basic Dysrhythmia certification.

Physical Requirements: Physical requirements for this classification require incumbents to stand, bend, squat, kneel, push and pull intermittently throughout the majority of the work day. Incumbents must be able to frequently lift and carry, partially carry, or fully support a patient while transferring to and from a bed, wheelchair, or gurney, or while walking.

Knowledge of: Basic nursing care and techniques; medical terminology and metric measurement.

Ability to: Perform basic nursing care; read and record medical terminology and metric measurement, maintain medical records, follow oral and written instructions; and maintain effective relations with medical staff and the public; stand, bend, squat, kneel, push and pull throughout the majority of the work day; lift, carry, partially carry, or fully support patients while transferring to and from a bed, wheelchair, or gurney, or while walking.

Supplemental: A background check may be required for this classification.

As a Medical Assistant Med Office Rep at Dignity Health Medical Foundation clinics, now part of CommonSpirit Health, you'll keep the day running smoothly and efficiently for your patients and clinical team. You'll take the lead on proactively preparing patient charts- ensuring all information is obtained and up-to-date for those seen in our facility. You can expect to have multiple patients daily depending on the number of physicians are on-site.

**Some of your daily activities will include:**
+ Assisting in preparing patients for examinations or procedures, assisting with procedures, relaying instructions from the physician to a patient or authorized person, and collecting patient data
+ Administering medications specifically directed by the physician and performing accurate vital sign measurements and documentation.
+ Maintaining a clean, organized, and safe environment, and performing patient care services that support the physician / provider's practice under the clinical supervision of the Physician, Registered Nurse, Nurse Practitioner, Physician's Assistant, or Licensed Vocational Nurse.
+ Comfortable managing medical records- charts and EMR
+ Communicate and document any patient concerns to physicians, physician's assistants, or nurses
+ Work with physicians, insurance providers, and pharmacies to obtain authorizations for patient medication, including refills

+ High school diploma (or equivalent) required
+ Satisfactory completion of a formal Medical Assistant program pursuant to the Division of Allied Health Professions or military training that is equivalent to an accredited Medical Assistant program (determination by Dignity Health Medical Foundation H.R. department in conjunction with the State Division of Allied Health Professions) required
+ Excellent interpersonal, organizational, and customer service skills required
+ Medical terminology, familiarity with CPT and ICD-10 coding procedures and reference tools, and pharmacology appropriate to the Medical Assistant scope of practice required
+ Current BLS certification required (must be certified through the American Heart Association)
Preferred Qualifications:
+ Six (6) months of experience in an outpatient setting as a Medical Assistant preferred
+ Certification must be obtained through one of the private agencies approved by the Medical Board of California:
+ American Association of Medical Assistants (preferred)
+ American Medical Technologists
+ American Medical Certification Association
+ California Certifying Board of Medical Assistants
+ Multi-skilled Medical Certification Institute, Inc. (NCCT)
+ Familiarity with an electronic practice management system preferred
