Job Summary:
An unlicensed caregiver who maintains a safe and clean environment for patients and Health Care team members. Transport patients with proper technique. Transport equipment, supplies and specimens. Provide environmental sanitation as required.
Essential Responsibilities:
+ Assists in preparation of the unit, patient rooms, and beds for receiving patient admission, returning surgical patients, transfers, emergencies, or discharges. Provides hygiene care, which includes providing or assisting in bed baths, oral hygiene, hair care and shampoo, back care and rubs, perineal care, dressing patient, and changing bed linen.
+ Prepares patients for meals, serves and removes food trays, and assists with meal or feeds patients. Distributes water and nourishment to patients. Performs other duties as assigned by the RN as patient care needs change. Orients patients and significant others to call lights, bed controls, side rail usage, bathrooms phones, TV waiting areas, and visiting hours. Assists and accompanies patient and their belongings (including personal property) in the admission, transfer, and discharge process. Answers patient call lights, anticipates patient care needs, and makes rounds to assigned patients on an ongoing basis. Communicates with patients in a courteous and helpful manner. Establishes and maintains communicates with the health care team. Performs data collection and documentation of TPR, BP, wt, ht, I/O, activity, diet, and hygiene in an ongoing, timely, accurate, and concise manner.
+ Demonstrates the use of good body mechanics and provides for patient safety when transferring, ambulating, turning, lifting, and positioning patients. Acts appropriately in emergency situations. Performs routine treatments and patient care duties listed on the skills inventory form and under the direct supervision of the RN.
+ Identifies and reports changes in the patients condition to the RN within an appropriate time frame. Maintains verbal and written information in a confidential manner. Assists in straightening patient rooms, cleaning overbed tables and bedside stands, keeping patient care areas clean at end of each shift. Collects and transports specimens for delivery to appropriate departments per hospital standards.
+ Completes work order for equipment that is out of service and leaves copy in the appropriate designated place/unit for follow up. Adheres to Infection Control Policy including but not limited to: a) demonstrates body substance precautions by wearing gloves and other protective attire to avoid body substance contact when appropriate; b) adheres to the KFHPW hand washing policy; c) complies with the proper handling of waste; d) maintains a clean work area. Provides postmortem care under the direction of the RN. Demonstrates initiative and responsibility for improving skills and expanding knowledge through continuing education or on the job learning experience. Consistently demonstrates an appearance consistent with current KFHPW dress code policy.
Basic Qualifications:
Experience
+ Minimum one (1) year of acute care or extended care.
+ Per the National Agreement, current KP Coalition employees have this experience requirement waived.
Education
+ High School Diploma or General Education Development (GED) required.
License, Certification, Registration
+ Nursing Assistant Certificate (Washington) required at hire
+ Basic Life Support
Additional Requirements:
+ Working knowledge of medical terminology.
+ Demonstrated interpersonal skills.
+ Demonstrated basic technical nursing skills.

Under minimal supervision, the **Patient Care Coordinator (General Surgery Department)** will coordinate and schedule all procedures into the scheduling system for all providers, technicians, rooms and equipment when applicable. Works as a liaison between physicians/office staff, patients and the service department to provide accurate and complete schedules for procedures. **Primary Responsibilities:**
+ Schedules procedures appropriately. May interact with various scheduling tools
+ Fills empty spaces on schedules as cancellations occur from patient waiting list
+ Identify scheduling conflicts, notify appropriate people, and rearrange schedules to eliminate conflicts
+ Calls patients from specialty referral list to arrange for preventive screenings when applicable
+ Accurately takes basic historical data and documents to facilitate procedural scheduling and follows protocols and guidelines for appropriate procedure/test
+ Communicates patients concerns of change in health status to MD or MA staff
+ Communicates clinical information with other clinic disciplines regarding procedures. (Anticoagulation clinic, Anesthesia, Hospital, radiology, interpretive services, etc.)
+ Assists with patient recalls
+ May interact directly with patients, physicians, and staff daily to schedule and coordinate rooms, equipment, supplies and procedural/testing schedules as well as coordination of billing for appropriate services
+ May perform insurance pre-authorization when applicable
+ May submit charges for services rendered when applicable
+ Provides established written instructions and instructs patients regarding appropriate procedures on preparations for procedures, medication and dietary restrictions for procedures
+ Instruct patients to the proper location for tests
+ Asks appropriate questions regarding insurance/referral needs
+ Reviews paperwork from primary care office to assure patient is appropriately referred for screening or other procedure
+ Reviews medical history with patients before scheduling procedures
+ Determines from basic historical information per protocol whether patient may go directly to procedure or will need office consult first
+ Following written parameters, instructs patients on how to alter medications (insulin, Coumadin, etc.) in preparation for procedure
+ Arranges for and verifying interpretive services are available for procedures
+ Check voicemail often throughout the day
+ Answer phone calls as they come into the department
+ Prioritize phone calls and voicemail
+ Communicate with clinical staff regarding urgent phone calls. Gather pertinent information for taking clear, complete telephone encounters (TEs) according to standard work (SW) and direct patient calls to clinical staff according to clinical policy

Working with Executive Director, supports and facilitates implementation of existing or changes to Agency's policies, regulations, process, or procedures.13. Consults with VSO and internal/external stakeholders as needed when developing solutions or addressing issues.14. Working with Executive Director, sets strategic and operational goals for branch. The Therapist of Mental Health Services located in the Post-Intensive Behavioral Health Treatment Facility (Bridge Point) Discharge Housing program provides clinical counseling services to young adults' intensive behavioral health care to community-based living. They provide clinical care by conducting mental health assessments, short and long-term therapy, case management, and crisis intervention.

The Therapist is a key member of the Behavioral Health team and plays a crucial role in providing support to the community.

essential duties

Clinical Services Tasks

  1. Provide evidence-based and culturally responsive therapeutic interventions and services to young adults transitioning out of intensive behavioral health treatment.
  2. Administer mental health assessments, provide individual and group counseling sessions.
  3. Maintain awareness of crisis intervention best practices and provide crisis support to clients, as needed.
  4. Diagnose mental health conditions in accordance with the most recently published Diagnostic and Statistical Manual of Mental Disorders (DSM).
  5. Collaborate with the housing team to provide clinical input into individualized care plans and support alignment between behavioral health and housing stability goals.
  6. Provide clinical consultation, coordination, and referrals in collaboration with discharge planners, housing case managers, and other care team members.
Administrative Tasks
  1. Attend weekly/bi-weekly individual meetings with Clinical Supervisor to review cases.
  2. Participate in employee case consultations and division meetings.
  3. Maintain awareness of the divisions within the agency and its internal resources.
  4. Improve knowledge about current best practices for various diagnoses and mental health concerns through research.
  5. Maintain case records in an accurate and timely manner per all WAC, COA, Medicaid and King County requirements.
  6. Collaborate with internal and external providers to support successful transitions fromintensive care to long-term stability in the community.

  1. Education and Experience
    1. Master's degree in human services field.
    2. Licensed with the State of Washington as an LICSW/LSWAIC. LMHC/A, or LMFT/A.
    3. Minimum of 1-year relevant work experience with children, youth, and/or families.
    4. Demonstrated knowledge of MS365 Power Platform.
  1. Additional Requirements
    1. Must satisfactorily pass criminal history check.
    2. Must satisfactorily pass tuberculosis (TB) test.
    3. Must have reliable transportation.
    4. Must successfully complete the following health certification requirements within 30 days of employment: CPR, First Aid and Bloodborne Pathogens.
    5. Proof of COVID-19 vaccination is required.
