**Job Summary and Qualifications**
The Patient Care Technician (PCT) delivers high quality, patient-centered care by performing a variety of delegated basic patient care services related to common nursing functions and activities of daily living. Under the supervision of the registered nurse (RN), the PCT assists the nursing staff in providing patient care in accordance with the PCT's skills/training and the needs of the Department, and consistently assists the patient care team with maintaining a clean, well-organized, and safe patient care environment. The PCT complies with all hospital policies/procedures and observes the State Board of Nursing rules for unlicensed personnel.
**What you will do in this role:**
+ Participates in the implementation of nursing care plans by performing direct and indirect patient care under the supervision of the licensed nurse.
+ Obtains and records initial and ongoing patient information throughout the cycle of care, including admissions data, vital signs, height and weight, blood pressure, oxygen saturation, intake and output, calorie counts, and other patient information as outlined by policy.
+ Assists with mobility by regular turning and positioning, dangling, and ambulating patients. Assists with use of walkers, crutches, canes, and wheelchairs. Transfers patients to and from other units, departments, or entities. Assists with discharges. Demonstrates proper body mechanics.
+ Obtains or assists in the collection of specimens for diagnostic or waived tests. Ensures accuracy of all labels and timely transfer of specimen.
+ Assists patients with activities of daily living to include eating and drinking, bathing, shaving, oral care, grooming, and elimination. Changes bed linens as needed.
+ Serves as Monitor/Telemetry Tech for the unit as needed.
+ Engages in evidence-based best practices that enhance the patient experience, including SAFETY rounding, communication fundamentals/AIDET, Teach-Back, Narrating Care, etc.

PURPOSE AND SCOPE:

Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with organization policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP).

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Patient Related

Education:

  • Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.

Treatment:

  • Welcome assigned patients and inquire as to their wellbeing since their last treatment.

  • Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.

  • Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment

  • Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).   

  • Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.   

  • Monitor patients’ response to dialysis therapy. 

  • Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.   

  • Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure

  • Obtain Hemostasis and apply appropriate dressings.   

  • Evaluate the patient prior to discharge.

  • Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification   

  • Responsible for calculating and entry of individual patients’ dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.   

  • Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.   

  • Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification   

  • Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.   

  • Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.

  • Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.

  • Perform and document any intervention for unusual patient status and document patients' response to intervention.

  • Evaluate patients’ vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.

  • Obtain all prescribed laboratory testing and prepare specimens for collection.   

  • Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.   

  • Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.   

  • Monitors patients performing self-care under the supervision of RN.

Staff Related

  • Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.

Education/Communication:

  • Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices

  • Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.

  • Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.

  • Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).

  • Initiate Solution Delivery System (SDS) system.

RECORD KEEPING:

  • Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.  

  • Enters all treatment data into the designated clinical application in an accurate and timely manner.   

  • Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure

omitted entries are completed or corrected by appropriate staff.

  • Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.

  • Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.   

  • Collect, label, appropriately prepare and store lab samples according required laboratory specifications.   

  • Ensure collection of lab specimens by appropriate lab courier.

   

INFECTION CONTROL:

  • Assists in collecting  information for infection control audits.   

  • Supports staff and patient Adherence to infection control practices.   

  • Follows infection control Policies and Procedures   

  • Participates in and reinforces infection control education of patients and families under the direction of the CM or designee as allowed by state law.

   

TRANSITIONAL CARE UNIT:

  • Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care 

  • Reinforces all education and care related matters as it relates to the new patient     as allowed by state law

  • Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.   

  • Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.

  • Sets and preforms a treatment on all machines used in the transitional care unit.

  • Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.

  • Assists with coordination of transition to patient’s modality choice.   

  • Assists with supply ordering and inventory for TCU

+ Follows the patient and family-centered care standards and provides direct patient care under the direction of a registered nurse. Documents, where applicable, care administered, diagnostic measurements, treatments and procedures in accordance with established policies and procedures. Responds to the call light system, contacting nursing personnel as appropriate, and follows through with meeting patient needs.
+ Pull/prints daily reports per unit standard, which may include daily shift report, charge nurse reports, handoff reports and/or My Daily Care Plan. Assists with calls to physicians and their office, bed management, transfer services or other floors, coordinating with Operations Administrator as needed. Organizes the unit workflow, paperwork as needed, proactively problem-solves, anticipating needs, and managing multiple ongoing priorities with minimal supervision.
+ Coordinates the requests to Facilities Management and Biomed regarding department needs or broken equipment. Rounds through the unit, per unit expectations, to check for environmental or equipment needs, and other assistance that may be needed. Serves as an in-house courier which may include retrieving blood, hand-delivering labs, tele boxes, etc.
+ Contributes towards improving department scores for patient satisfaction through peer-to-peer accountability, i.e., bedside shift handoff/report.
**QUALITY/SAFETY ESSENTIAL FUNCTIONS**
+ Reports observations, conditions and problems of patients to licensed personnel to achieve desired patient outcomes. Provides for the privacy of patients and families, keeping the safety of the patient in mind.
+ Provides appropriate hand-off at change of shift at patient bedside to communicate and collaborate, promoting patient-centered care.
+ Conducts hourly rounding to ensure patient needs are met (four P's). Monitors and addresses noise level of unit to improve or impact patient satisfaction. Responds to patient calls promptly. Reports near misses and collaborates with the interprofessional health care team to improve patient safety. + Contributes towards improving quality and safety scores, through peer-to-peer accountability. Supports initiatives to prevent conditions such as pressure injuries, patient falls, and hospital-acquired infections.
**FINANCE ESSENTIAL FUNCTIONS**
+ Assists with patient and staffing needs (floats) across the service line or hospital as competencies allow. Utilizes time between heavy workloads efficiently.
+ Utilizes resources with cost effectiveness and value creation in mind. Self-motivated to independently manage time effectively and prioritize daily tasks, minimizing incidental overtime, assisting coworkers as needed.
**GROWTH/INNOVATION ESSENTIAL FUNCTIONS**
+ Offers innovative solutions in performance improvement projects and shared governance activities. Follows up on action items as necessary to ensure completion of assignments.
+ Seeks opportunities to identify self-development needs on knowledge base, skill level and decision-making, as necessary, especially in areas of question, from preceptor and assigned licensed personnel and takes appropriate action. Ensures own career discussions occur with appropriate management. Completes and updates the My Development Plan on an on-going basis.

**EDUCATION**
+ High School diploma or equivalent education (examples include: GED, verification of homeschool equivalency, partial or full completion of post-secondary education, etc.)
**WORK EXPERIENCE**
+ Eighteen months of Certified Nursing Assistant (CNA), Certified Medical Assistant (CMA) or Patient Care Assistant (PCA) experience
+ One year of secretarial/office, college/vocational training, or six months of internal cross-training within Houston Methodist
+ Prior health care experience and/or medical terminology preferred
**LICENSES AND CERTIFICATIONS - REQUIRED**
+ BLS - Basic Life Support or Instructor (AHA)
**KNOWLEDGE, SKILLS, AND ABILITIES**
+ Demonstrates the skills and competencies necessary to safely perform the assigned job, determined through on-going skills, competency assessments, and performance evaluations
+ Sufficient proficiency in speaking, reading, and writing the English language necessary to perform the essential functions of this job, especially with regard to activities impacting patient or employee safety or security
+ Ability to effectively communicate with patients, physicians, family members and co-workers in a manner consistent with a customer service focus and application of positive language principles
+ Performs basic patient care activities of daily living (ADL's) within unit specific defined limits and job scope, such as assistance with patient elimination, hygiene, comfort, safety, nutrition, and progress activity
+ Ability to operate and troubleshoot equipment (fax, PC, printers, telephones, etc.)
+ Proficient computer application skills
